| Objective: This paper takes 31 residency training bases in Anhui Province for the class of 2018 to 2020 as the research object,and studies and analyses the residents’ satisfaction with the residency training work and the main influencing factors of residency standardised training in Anhui Province in terms of the residents’ understanding and support of the residency training policies of the state and training bases,the teaching teachers and the residents’ own ability improvement,and proposes countermeasures based on the research results,hoping that It is hoped that this study will serve as a reference for the innovation and reform of medical education in Anhui Province and the improvement of the quality and policies of standardized residency training.Methods: The article used literature analysis to search some authoritative Chinese and English databases,and reviewed the literature and materials related to standardized residency training at home and abroad to understand the historical development and current situation of standardized residency training at home and abroad.The reliability and validity of the questionnaire were tested on 38 indicators in three dimensions,including policy and management,teachers’ qualifications,and self-capacity enhancement.The electronic questionnaire was sent to 31 residency training bases in Anhui Province from 2018 to 2020 through Questionnaire Star to investigate the work of residency training,and the data obtained were analyzed and processed by using Excel2016 and SPSS 23.0 software.Through multivariate ordered logistic regression analysis,the satisfaction of residents and the influencing factors were studied to provide a basis for effective suggestions and countermeasures.Results: The Cronbach’s α coefficient value of the questionnaire reliability test was0.981,which was considered a good internal consistency of the questionnaire.the KM0 value was 0.980 and the 2 value of Bartlett’s spherical test was 59676.367 with a degree of freedom of 703,which reached a significant level(p<0.001),indicating a high validity of the questionnaire.The results of the survey showed that residents in training in Anhui Province from the class of 2018 to 2020 were satisfied with the standardised residency training.(1)Satisfaction level of understanding and support of the national and training base’s residency training policies.Satisfaction level of residents’ participation in teaching activities in training bases or departments(5 points)was the highest(43.64%),support for national policies on residency training(5 points)(23.74%),satisfaction with clinical teaching in training bases(5 points)(21.44%),support for training base policies(5 points)(20.90%),satisfaction with including residency training in performance appraisal(5points)(19.75%)is high;the dissatisfaction level(1 point)with the training base’s treatment guarantee is the highest(17.15%),the dissatisfaction level(1 point)with the training base’s inclusion of practice qualification(12.02%),seniority(10.57%),education(9.11%),social security(8.81%),merit evaluation(6.97%),and the training base’s inclusion in the treatment system,and the dissatisfaction level(1 point)with the training base’s dissatisfaction with process management(1 point)was high(7.43%).(2)Satisfaction with teaching by lead teachers.The residents’ ratings of the teaching awareness,teaching level,teaching content,teaching tasks,teaching methods and teaching effectiveness of the lead teachers were in a relatively balanced range,with general satisfaction(3 points)ranging from 27.64% to 31.70%,more satisfied(4 points)ranging from 37.83% to 41.73% and very satisfied(5 points)ranging from 20.21% to22.74%.The highest level of dissatisfaction(1 point)in the ratings related to the qualifications of the teachers who lead them was the innovation in teaching methods(5.28%),while the highest level of satisfaction(5 points)was the extent to which the teaching tasks were implemented according to the training syllabus(22.82%).(3)Satisfaction level of residents’ own competence improvement.Residents considered that they were very satisfied with the improvement of their own competencies in humanistic care(30.32%),doctor-patient communication(29.56%),teamwork(29.25%),labour discipline(29.10%),clinical operation skills(27.57%),history taking(27.11%),medical ethics(26.95%),basic theory learning(26.49%),undertaking clinical workload(26.11%),diagnostic ability of common diseases(26.03%),and auxiliary diagnosis(24.81%)were rated as very satisfactory.This was followed by critical care case management(19.60%)and learning new knowledge and techniques(19.45%);those who rated themselves as very satisfied in teaching junior residents or interns(17.23%),research ability(15.31%),review writing(15.01%),and learning English(13.17%)were relatively less satisfied.A relatively high percentage of students rated themselves as 1(very dissatisfied)in the areas of teaching junior residents or interns(7.35%),research skills(13.55%),writing reviews(13.17%)and learning English(13.40%)to improve their own skills.Conclusion: After years of development,the standardized training of resident doctors in Anhui Province has been effective.However,there are also some shortcomings:the training bases present weak management in terms of process management,and the quality of training needs to be further improved.The performance assessment mechanism is not yet perfect,and the training is not highly motivated.The funding input is insufficient,and the policy of treatment guarantee is not yet perfect.The standardization and innovation of teaching of resident trainers are not sufficient,and the teaching effect is not satisfactory.There are relatively few opportunities for residents to learn English,and the level of writing reviews is not high.The learning of new knowledge and techniques is neglected,and the ability to handle critical and severe cases is not strong.There is less participation in scientific research and teaching,and the ability to teach scientific research is weak.In the future,we should strengthen the policy propaganda of residency training,improve the performance assessment mechanism,treatment guarantee and faculty construction,and strengthen the comprehensive training of residents’ own abilities in English learning,review writing,learning new knowledge and new technology,handling critical and emergency cases,and scientific research and teaching ability,so as to improve the work quality of residency training and promote the improvement of residency training work in Anhui Province. |
