Research On The Development Dilemma And Resolution Of Institutional-based Integration Of Medical Care And Elderly Care

The elderly group is one of the groups that need the most attention and care in society.The happiness of the elderly is related to the happiness of every family in a society and the harmony and stability of it.At present,actively coping with population aging has become a national strategy,and promoting high-quality development of elderly services is one of the important tasks to realize this national strategy.For a long time,China’s medical and health resources and elderly care service resources have been in a state of mutual separation,the elderly care function of family has been low,and elderly care institutions have been small and weak,most of which can only provide basic care services,such as basic life care,assistance in meal,bath,toilet,etc,and the content and quality of elderly care services provided by which cannot well meet the needs of the increasing number of elderly people.Promoting the mutual integration of medical resources and elderly care resources is an important step to promote the high-quality development of elderly care services.With the development of elderly care practice,three models of elderly care have gradually formed in China: home-based elderly care,community-based elderly care and institution-based elderly care.In the process of promoting the integration of medical resources and elderly care resources,relying on the three existing elderly care models,the integration of medical care and elderly care has also gradually formed three models:the home-based integration of the medical care and elderly care,the community-based integration of medical care and elderly care,and the institution-based integration of medical care and elderly care.The institution-based integration of medical care and elderly care involves the most subjects and the most difficult challenges.This thesis discusses the difficulties and reasons of promoting the development of the institution-based integration of medical care and elderly care,and tries to put forward targeted countermeasures and suggestions.This thesis is conducted according to the general research idea of “what-why-how”,starts from the supply of services of institution-based integration of medical care and elderly care,sorts out the characteristics and current situations of the three stages of institution-based integration of medical care and health care,which include its conception and germination,establishment and in-depth development,focus on the four types of institution-based medical care and elderly care arising in practice,and argues that the current dilemmas in the development of institution-based integration of medical care and elderly care in China mainly lie in: the lack of motivation for medical institutions to set up elderly institutions within medical institutions,the lack of strength of medical institutions within elderly institutions,the formality of cooperation and agreement between medical institutions and elderly care institutions,the difficulties of social forces to set up institutions integrating medical care and elderly care.This thesis concludes that although the four types of institution-based integration of medical care and elderly care are different in form,there are many similarities in the involved multiple subjects which supply services of integration of medical care and elderly care,and the intersubjective interest game relationships,so it is necessary to analyze the reasons for the development dilemmas of the four types in a comprehensive manner.These reasons are mainly reflected in the following: the development concept needs to be updated,the policy protection system is not sound,the role and responsibilities of relevant subjects have not been clarified,and the fully developed market environment has not yet been formed.In view of the above reasons,this thesis proposes countermeasures to solve the dilemmas of the development of institution-based integration of medical care and health care from four dimensions: insisting on its scientific development concept,improving its the policy guarantee system,clarifying the roles and responsibilities of its relevant subjects,optimizing its benign developing market environment,and helping its long-term development in four dimensions,including concept leading,improving guarantee,clarifying roles and creating environment.
