Clinical Observation On The Therapeutic Effect Of Radix Bupleurum Decoction On Type 2 Diabetes Mellitus Complicated With Metabolic Syndrome Of Liver And Stomach Depression Heat And Stasis

Objective This study was to observe the clinical efficacy and safety of Radix bupleurum decoction in the treatment of type 2 diabetes mellitus(T2DM)combined with metabolic syndrome(MS)caused by liver and stomach depression and heat and blood stasis,providing theoretical basis for the treatment of traditional Chinese medicine for T2 DM combined with liver and stomach depression and blood stasis.MethodAccording to the inclusion criteria,60 patients with T2 DM combined with MS(liver and stomach stasis with heat)admitted to the Endocrinology outpatient Department of Lu’an Hospital of Traditional Chinese Medicine from December 2021 to December 2022were selected and divided into treatment group and control group with 30 patients in each group by random number table method.All patients were given basic treatment,including health education,diet control and exercise.The control group was given metformin hydrochloride sustained release tablets.In the treatment group,Radix bupleurum decoction was added to the control group.4 weeks was a course of treatment,and 2 courses were observed.The changes of TCM syndrome score,blood glucose(FBG,2h PG),blood lipid(TG,HDL-C,LDL-C),body mass index(BMI),insulin resistance index(HOMA-IR)and other indicators in the two groups after different treatments were observed,and SPSS23.0 statistical software was used to analyze the data.Results(1)TCM syndrome score and efficacy: After treatment,TCM syndrome score of both groups was decreased compared with before(P<0.05),and the improvement of TCM syndrome in the treatment group was better than that in the control group(P<0.05);The total effective rate of treatment group was higher than that of control group(P<0.05).(2)Glucose metabolism indexes: FBG and 2h PG were decreased in both groups,with differences(P<0.05);The improvement degree of FBG in the treatment group was better than that in the control group,the difference was statistically significant(P<0.05),but there was no significant difference in the improvement degree of 2h PG between the two groups(P>0.05).(3)Insulin related indexes: both groups could reduce HOMA-IR(P<0.05),and the reduction of HOMA-IR in treatment group was better than that in control group(P<0.05).(4)Lipid metabolism indexes: TG and LDL-C were decreased and HDL-C was increased in both groups(P<0.05);The treatment group was better than the control group in terms of lowering TG and LDL-C(P<0.05),but there was no statistical significance in the increase of HDL-C between the two groups after treatment(P>0.05).(5)Obesity index: both groups could reduce BMI,with differences(P<0.05),and there was no statistical significance in reducing BMI between the two groups after treatment(P>0.05).ConclusionUnder the premise of basic treatment,the addition of Radix bupleurum decoction can improve the clinical symptoms of patients with liver and stomach stasis and blood stasis type 2 diabetes mellitus complicated with metabolic syndrome,reduce fasting blood glucose and blood lipid,improve insulin resistance,and is superior to simple Western medicine treatment in regulating the disorder of glucose and lipid metabolism and increasing insulin sensitivity,with positive clinical efficacy and good safety.
