Objective: To observe the clinical effect of electro acupuncture at nerve stem stimulation points on dysphagia after stroke.Methods: Subjects were divided into two groups: the group using speech swallowing therapy instrument alone and the group using speech swallowing therapy instrument plus nerve stem stimulation points.The control group: the patient was lying flat,fully exposed the neck,wiped the neck clean and kept the neck skin dry.The electrode piece of the swallowing speech therapy instrument was attached to the upper part of the hyoid bone and the upper part of the thyroid cartilage,with a frequency of 0-10 m A,which was suitable for the patient to tolerate and the electrode piece was removed after 20 minutes.Treatment group: acupuncture treatment: Select points on the basis of the control group: select the stimulation points of hypoglossal nerve,recurrent laryngeal nerve,Jinjin,Yuye,and throat wall.Location and acupuncture method: location of the stimulation point of the hypoglossal nerve: the midpoint of the line between the greater angle of the hyoid bone and the mandibular angle.Sting method: directly prick 2-3 cm towards the root of the tongue.Location of the stimulation point of the recurrent laryngeal nerve: a transverse finger below the laryngeal node,beside the cricoid cartilage.Stabbing method: stab 1.5-2cm backward and slightly inward at an angle of 70 °.Do not leave a needle when pricking Jinjin,Yuye and throat wall bleeding.Connect the two-point needle handle to the electrode wire adjust the waveform to the density wave and the intensity is based on the patient’s tolerance.After 30 minutes,take the needle,once a day,six times a course of treatment,and continue the next course of treatment after a day’s rest.A total of four courses of treatment are used to evaluate the treatment effect.Before and after 4 courses of treatment,the patients in the two groups were evaluated on the recovery of swallowing function of patients with dysphagia before and after treatment by Wada drinking water test standard swallowing function scoring method and swallowing function evaluation.Result: 1.There was no significant difference between the two groups before treatment(p>0.05).2.Comparison within groups: after systemic treatment,the patients in the treatment group had significant improvement in the Watian drinking water test standard swallowing function ability score and ingestion and swallowing function evaluation(p<0.05).In the control group,the scores before and after treatment were also significantly improved(p<0.05).3.Inter-group comparison: after treatment the patients in the two groups had significant improvement in the Watian drinking water test the standard swallowing function score and the evaluation of ingestion and swallowing function(p<0.05).The improvement degree of the treatment group was better than that of the control group(p< 0.05).4.Comparison of curative effect: The total effective rate was 96.3% in the treatment group and 76.9% in the control group.The curative effect of the treatment group was better than that of the control group(p<0.05).Conclusion:1.The simple use of speech swallowing therapy instrument and speech swallowing therapy instrument plus electro acupuncture at nerve stem stimulation point has an effect on the recovery of patients with dysphagia after stroke.2.The nerve stem stimulation point electro acupuncture can effectively improve and improve the swallowing problem of patients,and improve the feeding ability of patients,and its clinical effect is better than that of using the speech swallowing therapy instrument alone.
