BackgroundType 2 diabetes is caused by genetics,environment,age,lifestyle,etc.In recent years,theage of diabetes is getting smaller and smaller,which directly or indirectly affects the work and life of all kinds of people.Many complications of diabetes patients cannot be cured under current medical conditions,but it is of great clinical significance to monitor the biochemical markers related to the progression of complications in the early stage.Studies have shown that the level of amino-terminal brain natriuretic peptide precursor and D-dimer is closely related to the occurrence and development of diabetes complications.ObjectiveTo explore the relationship between amino terminal brain natriuretic peptide precursor(NT-proBNP),D dimer and vascular complications of type 2 diabetes,to provide sufficient basis for clinical prediction of the occurrence and development of vascular complications of type 2 diabetes.MethodsAccording to the type of complications of diabetes,the clinical diagnostic indexes of diabetes with macroangiopathemia and diabetes with microangiopathemia were studied.According to the diagnosis and exclusion criteria of type 2 diabetes formulated by the World Health Organization and China’s Clinical Guidelines for the Prevention and Treatment of Diabetes Complications 2019 Edition,the electronic medical record system of Henan Provincial People’s Hospital was used to search for patients with type 2 diabetes who visited the endocrinology department of Henan Provincial People’s Hospital from February 2020 to November 2021.84 diabetic patients with macroangiopathopathy and 50 diabetic patients with microangiopathopathy meeting the criteria were selected as the study group,and 55 diabetic patients with simple diabetes were selected as the control group.Various clinical data were collected,and the age,gender,diastolic blood pressure,systolic blood pressure,diabetes course,NT-proBNP,D-dimer,WBC,CRP and liver function,kidney function,lipid and other related indicators were recorded.SPSS25.0 software was used to compare the above indicators of the three groups of patients,and then NT-proBNP and D-dimer of the three groups of patients were correlated with the above indicators,and then specific indicators were selected according to the analysis of the data for ROC curve analysis,to determine its diagnostic value for type 2 diabetes vascular complications.Use Prism8.0 software to do the diagram.All test levels were P<0.05,indicating statistical significance.Results1.Comparison of basic data among the three groups: age,diabetes course,WBC and CRP indexes were statistically significant,and the above indexes in the diabetic vascular complication group were higher than those in the simple diabetic group(P<0.05),while there were no statistically significant differences in gender,diastolic blood pressure,systolic blood pressure and Hb A1 c among the three groups.2.Single factor pairwise comparison among the three groups: there were statistically significant differences in NT-proBNP,D-dimer,TP,ALB,BUN,Scr,RBP,Cys C,HDL,LDL between the simple diabetes group and the diabetes combined with macrovascular disease group(P<0.05).There were significant differences in NT-proBNP,D-dimer,ALB,DBIL,BUN,Scr,RBP and Cys C between the simple diabetes group and the diabetes combined with microangiopaplasia group(P<0.05).There were significant differences in NT-proBNP,D-dimer,ALB,HDL and LDL between the diabetic group with macroangiopathopathy and the diabetic group with microangiopathopathy(P<0.05).3.Pearson correlation analysis showed that the level of NT-proBNP was positively correlated with the duration of diabetes,CRP,FIB,BUN,Scr,UA,RBP and Cys C(r>0,P<0.05),and negatively correlated with TP and ALB(r<0,P<0.05).The level of D-dimer was positively correlated with the duration of diabetes,CRP,FIB,BUN,Scr,UA and Cys C(r>0,P<0.05).4.ROC curve analysis results of NT-proBNP,D-dimer in the diagnosis of diabetic complications: the area under the curve of NT-proBNP and D-dimer were 0.870 and 0.754,respectively;The sensitivities were 98.8% and 83.6% respectively.The specificity was77.6% and 65.2%,respectively.The area under the curve of NT-proBNP and D-dimer combined diagnosis was 0.871.The sensitivity was 98.2%;The specificity was 76.1%.The P value was less than 0.001,indicating that the detection of NT-proBNP,D-dimer alone or combined detection of NT-proBNP,D-dimer has certain practical diagnostic value for the complications of type 2 diabetes mellitus.BUN,RBP and Cys C also have auxiliary diagnostic value.ConclusionNT-proBNP and D-dimer are closely related to the vascular complications of type 2diabetes mellitus.The detection of NT-prob NP and D-dimer alone or in combination has a high test efficiency for the occurrence of type 2 diabetes mellitus complications.
