ObjectiveThe purpose of this study was to research on the relevance between the disease forecast of acute pulmonary embolism patients and the detection levels of blood serum NT-proBNP,D-dimer and ST2.whether the integrated detection of these three elements could enhance the assessment value of the disease forecast of acute pulmonary embolism patients.MethodsTo choose 43 patients diagnosed with acute pulmonary embolism and tally with the research criteria in Xiangdong hospital affiliated to hunan normal university from 1~stt January 2013 to 31~stt December 2018 as analysis objectives.Among these 43 patients,62.8%of them are male and 37.2%are female with an age range from 42 to 78 and an average of 61.63±9.172.Relevant elements such as blood serum NT-proBNP,D-dimer,ST2,troponin,myocardial enzyme CK-MB,arterial partial pressure of oxygen,platelet count,hemoglobin,ESR(erythrocyte sedimentation rate),lithic acid,arterial blood systolic pressure have been conducted statistical analysis.Divide these 43 APE patients into Group A(n=17,non-massive pulmonary embolism group)and Group B(n=26,massive pulmonary embolism group),then separate Group B into Group B1(n=11,large area non-thrombolytic group)and Group B2(n=15,large area thrombolytic group)and detect serum NT-proBNP,D-dimer,ST2,troponin I and arterial partial pressure of oxygen three months before and after the treatment.According to the different prognosis,43APEpatients were divided into the group with good prognosis(C1 group)and the group with poor prognosis(C2 group).Results1.Content of NT-proBNP,D-dimer and ST2 in the massive pulmonary embolism group(Group B)is distinctively more than that in non-massive pulmonary embolism group(Group A)(p<0.05),which is of statistical significance.2.Detection result of independent sample T manifests that after three months of treatment,content of NT-proBNP,D-dimer and ST2 in the group of negative condition after the forecast(Group C2)is distinctively higher that in the group of positive condition after the forecast(Group C1)(p<0.05),which is of statistical significance.3.We could draw this conclusion after the prognostic correlation analysis of various biological indicators and acute pulmonary embolism patients:NT-proBNP,D-dimer,ST2,troponin,arterial partial oxygen pressure and arterial systolic pressure are of oblivious relation with the disease forecast of APE patients,p<0.05,difference of which is of statistical significance.While myocardial enzymes CK-MB,platelet count,hemoglobin,uric acid and erythrocyte sedimentation rate are not of oblivious relation with the disease forecast of APE patients,p>0.05,difference of which is not of statistical significance.4.ROC Trace Analysis:the ROC curve AUC of NT-proBNP,D-dimer and ST2 in serum equals to 0.835,which was larger than the area under ROC curve of any single acute pulmonary embolism test,p<0.05,difference of which is of statistically significance.Conclusions1.NT-proBNP,D-dimer and ST2 are components of prognosis in patients suffering acute pulmonary embolism,the higher the figure of which is,the worse the forecast condition becomes.2.The integrated detection of serum NT-proBNP,D-dimer and ST2 has higher sensitivity and specificity in evaluating the prognosis of patients with acute pulmonary embolism than any single index,but the integrated detection of serum NT-proBNP,D-dimer,ST2 as well as troponin is of much higher sensitivity and specificity.
