The Clinical Experience And Medication Rule Of Professor Tang Meiwen In Treating Peptic Ulcer

Objective:The purpose of this study is to analyze Professor Tang Meiwen’s clinical experience,medication rules and diagnosis and treatment methods in the treatment of peptic ulcer(PU)by collecting and processing the clinical cases of Professor Tang Meiwen in the treatment of peptic ulcer(PU),with the help of the traditional Chinese medicine inheritance assistant platform(V3.0)as the carrier,and grasp its core prescription,compatibility rules,etc.,so as to better grasp the clinical thinking of the tutor in the treatment of peptic ulcer,It can further promote the inheritance and innovation of traditional Chinese medicine in treating peptic ulcer.Methods:By collecting,sorting out and analyzing the clinical cases of Professor Tang Meiwen treating peptic ulcer,including 138 clinical cases,the included cases will be analyzed by information classification,disease proportion,TCM syndrome type,applicable prescription,drug use rule,four qi and five flavors,meridian effect,etc.through the auxiliary platform of traditional Chinese medicine inheritance(V3.0),and the used drugs will be correlated and clustered,The prescriptions,associated drugs and core drug combinations of the included cases were sorted out,and the clinical experience and medication rules of Professor Tang Meiwen in the treatment of peptic ulcer were summarized.Results:1.Patient information:There are 138 patients in this study,of which the TCM syndrome types of peptic ulcer cases included can be divided into:spleen and stomach deficiency cold syndrome,spleen and stomach damp-heat syndrome,liver and stomach stagnation heat syndrome,liver and stomach disharmony syndrome;Among them,there are 78 male patients and 60 female patients,and the age is mainly between 50 and 59 years old;There were 38 cases of gastric ulcer,82 cases of duodenal ulcer and 18 cases of compound ulcer;The symptoms of stomachache,belching,acid regurgitation and dry mouth and bitter mouth were more frequent;Among them,the syndrome of damp-heat in spleen and stomach accounted for the largest proportion;Among them,the tongue pulse is red,with tooth marks on the edge,yellow and greasy fur,and the frequency of pulse number is more.2.Prescription data analysis:The prescriptions used in this study include 101 kinds of traditional Chinese medicine;From the perspective of the "four gases" of drugs,the three drugs of temperature,cold and calmness are used more frequently,and the two drugs of heat and cold are used less frequently;From the perspective of the "five flavors" of drugs,bitter,sweet and spicy drugs are used more,and sour and salty drugs account for less;From the perspective of "meridian tropism",drugs are used more frequently in the spleen,stomach,lung,liver and heart meridians;The gallbladder and kidney meridians also play a very important role;Among them,the association rule is used for analysis.The number of support is 55,and the confidence level is 0.8.104 association rules and 71 groups of high-frequency drug pairs are obtained;Six core drug combinations can be obtained from systematic cluster analysis:combination 1:white peony-bupleurum chinense-tangerine peel-roasted licoricecuttlebone-bergamot-corrugated-atractylodes macrocephala;Combination 2:roasted licorice-orange peel-poria cocos-cuttlebone-orange fruitdandelion-Scutellaria baicalensis-codonopsis pilosula;Combination 3:orange peel-cuttlebone-white peony-corrugated fruit-gardenia-bupleurum-dandelion-scutellaria baicalensis;Combination 4:cuttlebone-orange peelmagnolia officinalis-corrugated fruit-atractylodes macrocephala-codonopsis pilosula-white peony-platycodon grandiflorum;Combination 5:roasted licorice-cinnamon twig-jujube-astragalus-white peony-atractylodes macrocephala-ginger-maltose;Combination 6:dandelion-scutellaria baicalensis-roasted licorice-cuttlebone-corrugated fruit-magnolia officinalis-tangerine peel-atractylodes atractylodes;From the drug use frequency,the total drug use frequency is 2384 times,and there are 20 traditional Chinese medicines with a frequency of more than 40 times(excluding 40),including:roasted licorice,orange peel,cuttlebone,corrugated seeds,white peony,atractylodes macrocephala,scutellaria baicalensis,dandelion,ginger pinellia,chaihu,codonopsis pilosula,magnolia officinalis,coptis chinensis,atractylodes macrocephala,poria cocos,fructus aurantii aurantii,bamboo Ru,ginger,fiuctus evodiae,gardenia;According to the statistics of the top five commonly used dosage points of drugs for highfrequency use,roasted licorice 10g,orange peel 10g,cuttlebone 15g,corrugated fruit 15g,and peony 15g;From the perspective of drug types,there are 17 types of drugs included.The top 10 types of drugs are:tonifying drugs,regulating qi drugs,relieving superficial symptoms drugs,removing dampness drugs,resolving phlegm,relieving cough and asthma drugs,diuretic drugs,astringent drugs,promoting blood circulation and removing blood stasis drugs,and antipyretic drugs.Conclusion:Through the auxiliary platform of traditional Chinese medicine inheritance,the cases of Professor Tang Meiwen’s treatment of peptic ulcer were analyzed,sorted out and summarized.More than half of the highfrequency drugs were traditional Chinese medicine such as tonifying the spleen,clearing heat,and strengthening the spleen,which reflected the teacher’s view that the etiology and pathogenesis of PU was spleen and stomach deficiency,and damp-heat stasis and toxin was its target.The key was the idea of seeking the cause through examination and treatment based on syndrome differentiation.Therefore,the basic principle of the teacher’s treatment of PU was to strengthen the spleen and stomach,and replenish the deficiency and remove the evil;The compatibility principle of the teacher’s treatment of PU is to regulate the spleen and stomach,and take into account the five viscera;It can clear away heat and damp,adapt to cold and heat,regulate qi and collaterals,and clean and replenish both."One injury to the spleen and stomach,five disorders interact with each other",taking Zhongzhou as the center and taking into account the whole.The prescription and medicine used in the case reflect the treatment principle of syndrome differentiation and overall thinking,that is,"the treatment must be based on the root",which is also the basis for the teacher to organize the prescription and adjust the medicine.
