| Objective: By collecting the medical records of the tutor Professor Tang Meiwen’s treatment of diarrhoeal irritable bowel syndrome,and using the method of data mining technology to conduct statistical analysis on the medical records,to explore the medication rules of Professor Tang Meiwen in the treatment of diarrhoeal irritable bowel syndrome,and summarize Professor Tang Meiwen’s treatment of diarrhoeal irritable bowel syndrome.Clinical experience in the treatment of diarrhea-predominant irritable bowel syndrome.Methods: The medical records of patients with diarrhea-predominant irritable bowel syndrome admitted by Professor Tang Meiwen from September 2019 to December 2021 in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine were collected through the inclusion and exclusion criteria.The patient’s basic information,four-diagnosis information and other medical record data collected from the outpatient clinic are established in Excel form,and the collected 177 case data are standardized.Frequency analysis,association rule analysis and cluster analysis were carried out on the data from the perspectives of efficacy classification,four qi,five flavors,meridian homing,and drug compatibility,to discuss Professor Tang Meiwen’s medication experience.Results:(1)General information: The cases included in this study included102 male patients,accounting for 57.63%,and 75 female patients,accounting for 42.37%.The age of onset of diarrhea-predominant irritable bowel syndrome is concentrated in the 40-49,30-39 and 20-29 age groups.(2)Syndrome type: 53 cases of spleen deficiency and dampness syndrome,47 cases of spleen-stomach damp-heat syndrome,44 cases of liver stagnation and spleen deficiency syndrome,19 cases of mixed cold and heat syndrome,and 14 cases of spleen and kidney yang deficiency syndrome.(3)Frequency of medication: This study involved 156 traditional Chinese medicines,with a total use frequency of 2686 times.The top 10 traditional Chinese medicines by frequency were: Atractylodes(151 times),licorice(148 times),Codonopsis(134 times),and dried tangerine peel(123times).times),Magnolia(106 times),Atractylodes(100 times),Chinese yam(91 times),Poria(88 times),White Lentils(80 times),Nutmeg(79 times).(4)In terms of efficacy: there are 18 types of traditional Chinese medicines involved,and the most frequently used type of medicine is the tonic medicine,which occurs 817 times.Qi-regulating class(271 times),heat-clearing class(264 times),and water-dampening class(232 times).(5)In terms of properties and flavors: in terms of medicinal properties,warm medicinals(1058,40.60%)are used the most,and other medicinal properties are used in the order of flat medicinals(680,26.14%),cold medicinals(637,24.49%),cooling drugs(131,5.04%),hot drugs(97,3.73%).In terms of medicinal taste,sweet medicinals(1297,33.87%)were used the most,followed by bitter medicinals(1221,31.89%),pungent medicinals(1054,27.53%),sour medicinals(211,5.51%),and salty medicinals(46,46,46,100,000).1.20%).In terms of meridians,the spleen meridian(2122,27.50%)was the most,and the meridians with the remaining frequency ≥ 500 were the stomach meridian(1380,17.88%),the lung meridian(1339,17.35%),and the liver meridian(691,8.96%),Heart Meridian(665,8.62%),Kidney Meridian(508,6.58%).(6)Association rule analysis: 156 traditional Chinese medicines are analyzed by association rules,the number of support is 70,the confidence is0.9,a total of 76 rules are generated,and a total of 66 groups of core traditional Chinese medicine combinations,of which 2 combinations of traditional Chinese medicines(that is,drug pairs)30 groups,24 groups of commonly used 3 herbs,11 groups of 4 herbs,and 1 group of 5 herbs.(7)Cluster analysis: A total of 4 core combinations were clustered,namely: Combination(1): Atractylodes,licorice,Codonopsis,dried tangerine peel,Magnolia officinalis,and white lentils.Combination(2): Licorice,tangerine peel,Atractylodes,Codonopsis,Chinese yam,Coix seed.Combination(3): Atractylodes,Codonopsis,Licorice,dried tangerine peel,dried ginger,Astragalus.Combination(4): Licorice,Atractylodes,Scutellaria,Atractylodes,Magnolia,Codonopsis.Conclusion:(1)The tutor’s treatment of IBS-D is mainly to strengthen the spleen and dry dampness,supplemented by soothing the liver,warming the kidney,clearing heat,relieving the surface,etc.It reflects that the etiology of the disease is related to exogenous evil Qi,diet injury,emotional disorder,weakness of the spleen and stomach,and fire failure of the gate of life.The key pathogenesis is spleen deficiency and dampness,which is marked by deficiency and excess,mixed with deficiency and excess,spleen and kidney yang deficiency,and cold dampness,damp heat and qi stagnation.(2)The instructor often uses core formulas such as Sijunzi Decoction,Shenling Baizhu Powder,and Lizhong Decoction to treat IBS.The tutor’s clinical prescription reflects the first healthy Zhongzhou,the same treatment of lung and intestine,and is good at using sweet and warm,pungent and bitter warm drugs to regulate the liver,spleen and kidney.It is used both warm and clear.It is mainly warm,pungent and bitter,and conforms to the physiological characteristics of the spleen and stomach. |
