Discussion On Professor Zhou Zhongying's Experience In The Treatment Of Peptic Ulcer Based On Data Mining

Objective:To explore Professor Zhou's clinical thinking and prescription characteristics in the treatment of peptic ulcer,so as to enrich and perfect Zhou's academic thought and clinical experience.Methods:By collecting and collating Zhou's clinical cases of peptic ulcer in the past 30 years,79 cases and 191 times of diagnosis were screened out,and all data were converted into Excel format to establish a database of peptic ulcer cases.SPSS 23.0 software and SPSS Modeler 18.0 software are used to analyze and process the data,and data mining methods such as frequency statistics,association rule analysis and factor analysis are used to mine the symptoms,pathogenesis,prescriptions,drugs and other contents in medical records.Finally,it is analyzed and summarized according to the clinical practice.Results:(1)Among the 191 medical records of 79 cases studied,there were 54 male patients with a total of 149 consultations and 25 female patients with a total of 42 consultations.Male patients and their times of diagnosis were significantly more than female patients.The oldest patient was 84 years old and the youngest was 10 years old,with an average age of(49.75±1.605)years.There were 60 cases of gastric ulcer with 115 times of diagnosis,7 cases of duodenal ulcer with 55 times of diagnosis,and 12 cases of other types of ulcer with 21 times of diagnosis.(2)In terms of symptoms,a total of 43 symptoms were found in the 79 cases of initial medical records,and the frequency of symptoms ranged from high to low:stomach pain,stomach distention,pantothenic acid,dry mouth,eructation,dry stool,loose stool,cold stomach,noisy of stomach,and bitter taste in the mouth(frequency>10).(3)In terms of tongue coating,thin,yellow and greasy coating is the most common among the 23 kinds of tongue coating.In terms of tongue proper,dark red is most common among the 24 types of tongue proper.In terms of pulse,the first three kinds of pulse with the highest frequency among the 16 kinds of pulse are thready pulse,thready slippery pulse and thready wiry pulse.(4)In terms of pathogenesis:damp-heat resistance in middle-jiao is the primary pathogenesis,followed by incoordination between liver and stomach,weakness of spleen and stomach,weak qi stagnation of stomach(frequency>10).(5)In terms of medication:144 kinds of traditional Chinese medicine were used in 79 cases of first diagnosis prescriptions,among which the top 10 most frequently used medicines were coptis chinensis,prepared pinellia tuber,cyperus tuber,evodia,cuttlebone,pogostemon cablin,zhi qiao,scutellaria,stalk of perillae,baked licorice.In terms of efficacy,drugs for relieving the depressed liver are the most frequently used,followed by the drugs of strengthening spleen and replenishing qi,heat-clearing and detoxifying,dampness-transforming water(total frequency>100).(6)Analysis of drug association rules:Drugs with higher frequency will be used for association analysis,and finally 52 pairs of statistically significant drug pairs will be selected.Combined with manual analysis,the common drug pairs of Zhou will be summarized.Conclusion:Zhou believes that the basic pathogenesis of peptic ulcer is "incoordination between liver and stomach,damp-heat resistance in middle-jiao".The basic treatment method is clearing away heat and dampness,regulating qi and harmonizing stomach.Other treatments include strengthening the spleen and qi,warming the middle-jiao to alleviate pain.At the same time,pay attention to protect the stomach yin during treatment,avoid using drugs that hurt the body.Zhou's commonly used medicines for peptic ulcer include:Pu-huang and Wulingzhi,Coptis chinensis and Evodia,pogostemon cablin and stalk of perillae;Rhizoma Corydalis and Muxiang,Bupleurum Root and Red Peony Root,Fritillary Bulb and cuttlebone,and so on.
