Objective:1.On the basis of literature research,the TCM syndrome classification of toufeng was preliminarily discussed based on Delphi method,in order to provide theoretical basis for the normalization and standardization of syndrome classification of toufeng.2.The characteristics of TCM syndroms of patients with toufeng were collected by cross-sectional investigation,and the distribution characteristics of clinical syndroms of toufeng were summarized.Methods:1.Literature research: " toufeng,cephalic wind,hypertension,migraine,syndrome,syndrome type" and so on were searched.According to the inclusion and exclusion criteria,the literature was screened.Statistical analysis and summary of the distribution characteristics of toufeng syndrome in the literature were carried out.2.Questionnaire survey based on Delphi method: Based on the symptoms of toufeng summarized in the previous literature research,A reasonably designed expert questionnaire mainly includes the names and manifestations of syndromes.Twenty experts in the field of traditional Chinese medicine and integrated traditional Chinese and Western medicine were selected nationwide.Questionnaires were distributed to the experts through the questionnaire star,and a total of three rounds of questionnaire surveys were conducted,in each round of questionnaire,the mean,percentage,coefficient of variation were used to evaluate the concentration and coordination degree of expert opinions,and finally the TCM syndrome criteria of toufeng were formed.3.Toufeng syndrome distribution of cross-sectional survey: on the basis of the previous research,in the form of questionnaire to collect the basic information of the patients with toufeng,clinical symptoms and lingual arteries and veins and other related information,establish information database,frequency analysis was performed by Excel to summarize the clinical syndrome distribution characteristics.Results:1.Literature research: A total of 10103 Chinese literatures were obtained by searching three major databases,and duplicate literatures were removed810 articles,6776 unrelated articles were excluded after reading the title and abstract,and 2128 articles were excluded after reading the full text.A total of 389 articles were included.There were 95 related TCM syndrome types,and 8 TCM syndrome types were finally retained in the normalization,which were: excessive liver-yang syndrome,excessive phlegm syndrome,blood stasis syndrome,deficiency of qi and blood,deficiency of liver and kidney Yin syndrome,external bundle of wind and cold syndrome,disturbance of wind and heat syndrome,Yang deficiency and cold condensation syndrome.2.Questionnaire survey based on Delphi method: In the three rounds of expert questionnaires,20 questionnaires were distributed and collected,and all of them were valid questionnaires,so the expert positive coefficient was 100%.After 3 rounds of questionnaires,5 TCM syndrome types of toufeng were finally determined,which were: liver-yang syndrome,excessive phlegm syndrome,blood stasis syndrome,qi and blood deficiency syndrome,liver and kidney Yin deficiency syndrome.The specific performance is as follows:(1)Liver-yang syndrome: Head pain,vertigo,irritability,red face,red eyes,eye distension,red tongue,yellow fur,pulse string number.(2)Excessive phlegm syndrome: Severe head pain,headache such as Mengmeng,lethargy,chest wan PI stuffy,fat tongue,white and greasy moss,pulse string slip or pulse slip.(3)Blood stasis syndrome: Head tingling,pain is fixed,headache very night,headache repeatedly,dark purple tongue,tongue has ecchymosis,sublingual veins cyanosis,pulse fine astringent or astringent.(4)Qi and blood deficiency syndrome: Headache,work aggravation,dizziness,fatigue,colorless face,spontaneous sweating shortness of breath,pale tongue,thin white coating,weak pulse.(5)Liver and kidney Yin deficiency syndrome: Headache,vertigo,tinnitus,waist and knee soft,red tongue less moss,pulse strings.3.Cross-sectional survey of the distribution of toufeng syndromes: 230 patients were investigated,most of them were over 60 years old,the ratio of male to female was about 1:1,and none of them had serious cardiovascular and cerebrovascular diseases.(1)According to the frequency of the syndrome,the order was as follows:hyperactive liver Yang syndrome(29.13%),turbidly phlegm syndrome Disturbance syndrome(26.96%),blood stasis syndrome(20.87%),liver and kidney Yin deficiency syndrome(16.52%),qi and blood deficiency syndrome(6.52%).(2)According to the frequency of symptoms: headache(100%),dizziness(53.48%),Fatigue(36.52%),dry mouth(31.30%),irritability(26.96%),chest tightness(25.65%),tinnitus(23.48%),upset(21.74%),insomnia(21.74%),etc.(3)The specific symptoms of different syndrome types in different ways,according to the frequency of high and low rank,the top 15 each kind of this syndrome symptoms are as follows:The distribution characteristics of clinical syndromes of hyperthyroidism of liver Yang: red tongue,swelling pain,irritability,insomnia,dizziness,pulse string,dry mouth,upset,bitter mouth,palpitation,eye distension,chest tightness,tinnitus,insomnia and dreams,thin yellow coat.The distribution of clinical symptoms of phlegm above disturbance syndrome: dizziness,red tongue,swelling pain,dizziness,pulse string sliding,palpitation,fatigue,insomnia,chest tightness,yellow and greasy fur,nausea,severe head pain,tooth marks,dry mouth,dark red tongue,white and greasy fur.The distribution characteristics of clinical symptoms of cerebral stasis syndrome included tingling,dark red tongue,especially at night,thin white fur,dizziness,astringent pulse,insomnia,palpitation,fatigue,impatience and irritability,dark purple tongue,chest tightness,nausea,swelling pain,dizziness,dry mouth,ecchymosis or petechia.The distribution of clinical symptoms of Yin deficiency of liver and kidney: red tongue,less fur,dizziness,fatigue,tinnitus,dry mouth,waist and knee soreness,swelling pain,upset,night sweat,fatigue,blurred vision,chest tightness,empty pain,sometimes mild or severe.The clinical distribution characteristics of qi and blood deficiency syndrome: dizziness,fatigue,spontaneous sweating,dull pain,pink tongue,thin white fur,palpitation,shortness of breath,insomnia,aggravation of work,fatigue,thin pulse,chest tightness,dry mouth,and white fur.Conclusion:1.The TCM syndromes of toufeng can be divided into 5 types,namely,hyperthyroidism of liver Yang,deficiency of qi and blood,disturbance of phlegm,Yin deficiency of liver and kidney,stasis of brain collaterals.2 According to the cross-sectional study of 230 clinical patients with toufeng,the most common syndrome of toufeng was excessive liver Yang,followed by phlegm,blood stasis and brain collateral syndrome,liver and kidney Yin deficiency,qi and blood deficiency syndrome.There was no significant correlation between the distribution of TCM syndrome types and gender,age and course of disease.3.The main clinical manifestations of toufeng are headache,dizziness,fatigue,dry mouth,irritability,chest tightness,tinnitus,upset,insomnia,etc.Different manifestations may occur according to different syndromes.
