| Objective: To explore the syndrome element characteristics and related factors of bile reflux gastritis(BRG),and further analyze the law of drug compatibility by using data mining technology,so as to provide some reference significance for clinical accurate,dialectical and flexible drug use.Methods: BRG patients hospitalized in the digestive endoscopy department of the Affiliated Hospital of Shandong University of traditional Chinese medicine from September 2014 to December 2021 were selected.By analyzing their general situation,four diagnosis information,auxiliary examination and other data,the distribution characteristics of syndrome elements were summarized by using syndrome dialectical element scale and SPSS statistics 26.0,and the relevant factors were analyzed.At the same time,extract the prescription information of patients who meet the standards,use Microsoft Excel 2019,SPSS statistics 26.0,SPSS modeler18.0 and Cytoscape to complete the analysis of drug frequency,classification,four natures,five flavors,channel tropism,dosage and combination,and summarize the core drugs,combination rules and potentially related prescriptions of traditional Chinese medicine in the treatment of BRG.Result:1.Distribution characteristics of TCM syndrome elements of BRG: in this study,BRG patients are more female than male,with the most people aged ≥ 60 years old.Reflux is mainly grade II.The lesion site is mostly gastric antrum,and the HP infection rate is 36.3%.The syndrome elements on disease location of BRG are mainly stomach,liver,spleen and gallbladder,involving heart and small intestine.The syndrome elements on disease nature of BRG are mainly qi stagnation,dampness,yin deficiency,heat,Qi deficiency and phlegm,involving blood deficiency,food accumulation,blood stasis and yang deficiency.Syndrome element "stomach" is mild in BRG with polyposis;" Spleen" is common in elderly patients,rare in patient with polyps,and common and serious in HP infected patients;"Qi stagnation" is more serious in BRG with ulcers,but less serious in BRG with polyps;"Dampness" is more common in duodenal lesions and more serious in atrophic gastritis;"Yin deficiency" is common in patients with chronic inflammation,and is more serious in patients with gastric fundus gland polyps and HP infection;The severity of "fever" was higher in women than in men;"Qi deficiency" is rare in patients with polyps,but more serious in patients with erosion and lesions of gastric fundus and gastric body;"Phlegm" is more serious in the lesions with ulcer;"blood stasis" is more serious in ulcer lesions,granular hyperplasia and duodenal lesions;"Yang deficiency" is common in patients with erosion,gastric lesions and HP infection.2.Combination law of traditional Chinese medicine in the treatment of BRG: 228 drugs were involved in the study,with a total frequency of 2162 times.22 high-frequency drugs with frequency ≥ 20%,and the top five are Glycyrrhizae Radix et Rhizoma,Atractylodis Macrocephalae Rhizoma,Codonopsis Radix,Pinellia Rhizoma and Amomi Fructus.The drugs for tonifying are the most important,followed by Qi regulating drugs,herbs of clearing away heat and digestant drugs.The four natures and five flavors are mainly sweet and warm,followed by bitter and cold.Channel tropism refer to multiple meridians such as spleen,stomach,lung and liver.The association rules obtained 25 groups of core drug combinations,which were mostly found in Crataegi Fructus,Hordei Fructus Germinatus,Medicated Leaven or their compatibility with Atractylodis Macrocephalae Rhizoma,Codonopsis Radix,Amomi Fructus and poria.8 common factors were obtained by factor analysis;High frequency drug cluster analysis takes the European square example 23 as the standard to cluster four groups of drug combinations,which are Xiangsha Zhizhu pill,Banxia Xiexin Decoction,Chaihu Shugan powder and Danggui Buxue Decoction.Conclusion:1.The syndrome elements on disease location of BRG are mainly stomach,liver,spleen and gallbladder,involving heart and small intestine.The syndrome elements on disease nature of BRG are mainly qi stagnation,dampness,yin deficiency,heat,Qi deficiency and phlegm,involving blood deficiency,food accumulation,blood stasis and yang deficiency.There is a certain correlation between various factors(gender,age,type of gastritis,gastroscopic manifestations,pathological changes,lesion location,Helicobacter pylori infection)and the distribution and severity of syndrome elements.2.Combination law of traditional Chinese medicine in the treatment of BRG: the medication is based on strengthening the spleen,soothing the liver and regulating qi acticity,taking into account phlegm and dampness,stagnant heat,blood stasis,food accumulation and other aspects,combined use of cold-natured herbs and warm-natured herbs,taking into account both tonic and dispel evil,lung and stomach coordination,treating both liver and stomach,observing pathogenesis,being comprehensive and orderly. |
