The Research On The Cases Of Professor ShanZhaowei Treating Bile Reflux Gastritis Based On Datamining

Objective:In order to summarize and research the experience of the national old TCM Prof.Shan Zhaowei's treatment of bile reflux gastritis,on the basis of the clinical cases of treating bile reflux gastritis by professor Shan Zhaowei,applying modern data mining technology to study medical cases.Method:The medical cases of professor shan zhaowei's diagnosis and treatment of bile reflux gastritis from September 2014 to December 2017 was recorded in Medcase V3.2 data record mining system.According to include and exclude criteria,333 cases(695 clinical times)were picked up to establish medical record database.Combining with the clinical treatment experience of professor shan zhaowei,frequency analysis,association rules,cluster analysis and other mining methods are used to summarize and research the laws of symptoms,tongue pulse,syndrome type and core prescriptions in medical cases.Results:All the 695 medical cases collected around one topic,there were 333 patients,including 191 females and 142 males.The average age was 48.59 years old.According to frequency analysis results,the main symptoms of high frequency of occurrence were abdominal distension(50.65%)pantothenic acid(37.70%)and dry mouth(36.83%);The patients with higher frequency of accompanied symptoms showed poor sleep(33.96%),loose stool(30.65%)and irritability(23.60%);The frequency of the light red(50.21%)and red tongue(40.58%)were higher;The frequency of thin white tongue coating(49.78%)and thin yellow tongue coating(25.47%)were higher;Among the pulse conditions,taut pulse andthreads pulse have higher frequency,accounted for 61.29%and 57.27%respectively.The main pathogenesis was weakness of spleen and stomach,liver-stomach disharmony,which accounted for 63.45%and 21.58%respectively.The Chinese medicine involved in the case is 178.The top ten most frequently the herbs agrimony respectively is:White atractylodes,Agrimonia pilosa,Semen Coicis,Oldenlandia,Radix pseudostellariae,Pinellia ternate,lily,Thunberg Fritillary Bulb,Radix Ophiopogonis,The root of kudzu vine,in all clinical time respectively 84.17%,83.60%,78.56%,60.58%,57.55%,54.10%,44.75%,40.43%,37.41%,40.43%.The relationship between the five herbs was strongest:White atractylodes,Agrimonia pilosa,Semen Coicis,Oldenlandia,Radix pseudostellariae.The top four most drug categories are medication to qi drugs,antipyretic,phlegm-reducing drugs,water-disinhibiting damp-percolating drugs.Accompanied drug categories frequency appear higher with qi drugs,in all clinical time respectively 18.65%.Cluster analysis shows that the core prescription of bile reflux gastritis is:Radix pseudostellariae,White atractylodes,Pinellia ternata,Radix Ophiopogonis,Agrimonia pilosa,Semen Coicis,Oldenlandia.Conclusion:The main pathogenesis of the disease is the weakness of spleen and stomach,liver-stomach disharmony.The main pathological factors are qi,fire,sputum,humidity and congestion.Treatment:replenishing qi to invigorate the spleen,Shugan Hewei method are the basic methods of the disease.Treatment features:He stresses paying attention to regulating spleen and stomach function,and smoothing liver.when treating illness,he often chooses prescription of smaller doses.He believes that we should use the qi-regulating drugs with caution preventing injury of yin-fluid.Choosing normal drugs,prescription light spirit,health education,prevention of recurrence.
