Objective:Combined with the management needs of nutrition and endocrinology specialists,this study aims to explore a remote "nutrition and weight" management system based on the Internet.The system was applied to the management of type 2 diabetes mellitus(T2DM)in the community,feedback and data collection were completed through the network to evaluate its effect.Method:1.Nutrition experts and endocrinology experts put forward the actual needs of the remote management system,and cooperate with software engineers to develop a remote "nutrition and weight" management system based on the Internet,and determine the remote management module and interface design by testing and optimizing the software.2.T2 DM patients were followed up in Community Health Center in Chongqing,and randomly divided into the telemedicine management group(n = 47)and control group(n =50).In the control group,routine outpatient management was adopted;in telemedicine management group remote "nutrition and body weight" management system was implemented,with the management time of 12 months.The feedback and exchange mechanism and data collection were completed through the network,and the changes of glucose,metabolism index,human body composition and diabetes cost were counted.Result:1.The telemedicine management system of nutrition and weight of T2 DM patients in community includes the "We Chat terminal" used by patients and the "management platform" used by medical staff.Through the real-time collection of data such as blood glucose,blood pressure,weight,exercise,diet,quality of life,medical expenses,medical staff can interact with patients remotely to realize the remote management of diabetic patients.2.Compared with the traditional outpatient follow-up,waist circumference,FBG,Hb A1 c,body fat index and the cost of diagnosis and treatment of diabetes in the telemedicine management group were significantly reduced,and Hb A1 c compliance rate was significantly increased(P < 0.05).Compared with that before intervention,FBG,Hb A1 c,body weight,BMI,waist circumference,body fat rate,body water content and muscle content were significantly improved in the telemedicine management group(P < 0.05),the cost of diabetes treatment was reduced(P < 0.05),Hb A1 c standard rate was significantly increased(P < 0.05),and the quality of life score was improved.In the telemedicine management group,there was a significant positive correlation between the weight loss and the frequency of uploading data(P < 0.01).Conclusion:The telemedicine "nutrition and weight" management system based on the network has perfect functions,which is helpful for medical staff to better manage T2 DM patients in the community,to significantly improve the behavior management ability of community patients,so as to obtain more stable blood glucose,higher Hb A1 c improvement and standard rate,avoid repeated hypoglycemia of patients,and ultimately reduce the economic burden of patients high quality of life of patients,therefore,it is worth further promotion in the community.
