Retrospective Score And Network Pharmacological Mechanism Of Buyang Huanwu Decoction In The Treatment Of Diabetic Nephropathy

Purpose:Through retrospective analysis,explore the general condition and efficacy of Buyang Huanwu Decoction in the treatment of diabetic nephropathy of Qi deficiency and blood stasis type(G2A1 to G3 a A2);explore the effect of Buyang Huanwu Decoction in the treatment of diabetic nephropathy by using network pharmacology Mechanism in order to better guide the clinic.Material and method:1.The medical records of patients with qi deficiency and blood stasis type diabetic nephropathy(G2A1 to G3 a A2)were collected in the outpatient clinic of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from October 2017 to December 2019 through analysis.Retrospective analysis of gender,age,tongue quality,tongue coating,pulse condition and main symptoms before and after treatment,microalbuminuria indexes,kidney indexes,and blood glucose indexes of the first-time patients were reviewed to evaluate the treatment of qi deficiency and blood stasis type diabetic nephropathy by Buyang Huanwu Decoction(G2A1 to G3 a A2)the efficacy of patients2.Search all chemical components of Buyang Huanwu Decoction traditional Chinese medicine through the TCM System Pharmacology Database(TCMSP)and Uni Prot database,screen the targets,and standardize the target gene names,and query the disease targets of diabetic nephropathy through Gene Cards and OMIM databases;Use the Venn diagram to find the potential therapeutic targets of Buyang Huanwu Decoction for the treatment of diabetic nephropathy,and use the String database to construct the target protein interaction network to obtain the key targets;use the Cytoscape 3.7.2 software to construct "Chinese medicine-chemical components-disease targets Click on the network diagram to obtain the key components of Buyang Huanwu Decoction in the treatment of diabetic nephropathy;use the Metascape database to perform GO function analysis and KEGG pathway analysis on target genes.Results:1.Buyang Huanwu Decoction combined with basic western medicine group and basic western medicine group can reduce the TCM symptom score(P<0.05)of patients with qi deficiency and blood stasis type diabetic nephropathy(G2A1 to G3 a A2),and both can significantly improve the patients Clinical symptoms(P<0.05).2.Buyang Huanwu Decoction combined with basic western medicine group and basic western medicine group to treat patients with qi deficiency and blood stasis type diabetic nephropathy(G2A1 to G3 a A2),group control: the patients' UACR,glomerular filtration rate,and creatinine are all obvious Improvement(P<0.05);control between groups after treatment:comparison in UACR,glomerular filtration rate,creatinine,etc.(P<0.05).3.In Buyang Huanwu Decoction,except Earth Dragon,there are 60 active ingredients and218 effective targets;3,487 targets for diabetic nephropathy;6 key ingredients of Buyang Huanwu Decoction for treatment of diabetic nephropathy;5 key targets.4.It was found that the treatment of DN with Buyang Huanwu Decoction mainly involves cancer pathway,diabetic complications AGEs-RAGE signaling pathway,IL-17 signaling pathway,PI3K-Akt signaling pathway,etc.Conclusion:1.Buyang Huanwu Decoction combined with basic western medicine to treat patients with Qi deficiency and blood stasis type diabetic nephropathy(G2A1 stage to G3 a A2 stage)has significantly improved symptoms and is higher than the basic western medicine group.2.Buyang Huanwu Decoction combined with basic western medicine group and basic western medicine group.Both can significantly improve the indicators of microalbuminuria,glomerular filtration rate,and creatinine in patients with diabetic nephropathy(G2A1 to G3 a A2)of deficiency of qi and blood stasis,but the improvement effect of Buyang Huanwu Decoction combined with basic western medicine group is more obvious3.Buyang Huanwu Decoction in the treatment of diabetic nephropathy may be mainly effective through kaempferol,quercetin,?-sitosterol,baicalein,luteolin,stigmasterol and other pharmaceutical ingredients,and may be mainly through AKT1,IL6,TP53,VEGFA Targets such as TNF and TNF take effect.4.Buyang Huanwu Decoction may play a role in the treatment of diabetic nephropathy through the AGEs-RAGE signaling pathway,a complication of diabetes.
