Research On Syndrome Differentiation And Medication Rule Of Chronic Nephritis Hematuria Based On Association Rules And Cluster Analysis

Purpose:Objective to search the treatment of hematuria due to chronic nephritis with traditional Chinese medicine prescriptions in recent ten years,using data mining technology to summarize the medication rule of modern physicians in the treatment of chronic nephritis hematuria.In order to provide new ideas for the treatment of this disease in traditional Chinese medicine.Material and method:According to the inclusion criteria and exclusion criteria,Wanfang,CNKI,CBM,VIP and other academic journal databases were selected to search the literatures on the treatment of chronic nephritis hematuria with traditional Chinese medicine from January 2010 to February 2020.Excel 2010,spss25.0 and SPSS Clementine 12.0 software were used to analyze the frequency of drugs.On this basis,the classification,nature,taste and meridian of traditional Chinese medicine were analyzed,and the association rules and cluster analysis of high-frequency drugs were carried out to summarize the combination of high-frequency drugs.Based on the modern pharmacology of traditional Chinese medicine,the mechanism of traditional Chinese medicine in the treatment of chronic nephritis hematuria was analyzed.Results:This study eventually included 91 prescriptions,involving 156 traditional Chinese medicines,with a total frequency of 1058 times;a total of 2567 patients with chronic nephritis and hematuria.1.In the analysis of syndrome types,the most frequent deficiency syndrome is Qi Yin deficiency,accounting for 38.72% of the deficiency syndrome,followed by spleen and kidney qi deficiency,liver and kidney yin deficiency,and the most common syndrome is blood stasis.2.In the analysis of high-frequency traditional Chinese medicine,the top ten herbs used most frequently were Astragalus membranaceus,Rhizoma Imperata,Rehmannia glutinosa,Rubia cordifolia,Cirsium japonicum,Eclipta,licorice,Panax notoginseng,cortex moutan and poria cocos.Among the 91 prescriptions,the most frequently used drugs were tonic drugs,accounting for 26.09%,followed by hemostatic drugs(24.29%),heat clearing drugs(20.23%),blood activating and stasis removing drugs(7.75%).From high to low,the four Qi distribution of drugs were cold(29.02%),flat(26.94%),warm(16.35%),mild temperature(9.92%),mild cold(9.45%),cool(8.13%)and hot(0.19%).The main flavor was sweet(43.08%)and bitter(27.28%),followed by pungent(10.57%),sour(6.83%),astringent(6.15%),light(4.37%)and salty(1.72%).The top five channels were liver channel(24.18%),kidney channel(16.16%),spleen channel(12.34%),lung channel(12.19%)and heart channel(11.49%).3.In the analysis of drug association rules,Astragalus membranaceus ? Achyranthes bidentata ranked first in the combination of the two drugs with 94.12% confidence,followed by Eclipta ? Ligustrum lucidum,Astragalus ? Atractylodes macrocephala,Astragalus ?yam;Rhizoma Imperata ? Fructus Ligustri Lucidi and Radix Astragali ranked first in the three drug association combinations,with a confidence of 93.33%;Eclipta prostrata ?Ligustrum lucidum,Rhizoma Imperata and Astragalus membranaceus occupied the first place in the four drug association combinations,and the confidence was 85.71%.In cluster analysis,high-frequency traditional Chinese medicine can be divided into three categories: Astragalus membranaceus,Rehmannia glutinosa,Eclipta,Ligustrum lucidum,Imperata,Cirsium japonicum,Rubia cordifolia;yam,Alisma orientalis,Cornus officinalis,Achyranthes bidentata,Cortex Moutan,Atractylodes macrocephala,Pseudostellaria heterophylla,Panax notoginseng;the remaining 31 herbs.Based on the modern pharmacological analysis of the efficacy of traditional Chinese medicine in the treatment of chronic nephritis hematuria,it is concluded that the drugs that affect immune regulation and anti-inflammatory are the most.Conclusion:1.The pathogenesis of hematuria in chronic glomerulonephritis is based on deficiency and excess,in which deficiency of kidney is based on deficiency of both qi and Yin,which is closely related to liver and spleen;blood stasis is the main factor of excess and damp heat and turbid dampness are seen.The main treatment is to replenish qi and nourish yin,and to remove blood stasis and stop bleeding,clear away heat and remove dampness.2.Based on the data mining of compound prescriptions in the literature,the commonly used drugs for the treatment of hematuria in chronic nephritis are: Astragalus membranaceus,Imperata root,Rehmannia glutinosa,Rubia cordifolia,Cirsium japonicum,Eclipta,licorice,Panax notoginseng,Cortex Moutan,Poria cocos,etc.In the literature,tonic drugs,hemostatic drugs,heat clearing drugs and blood activating and stasis removing drugs were mostly used.Based on the analysis of the nature,taste and meridian tropism of drugs,most of them are cold drugs,mild drugs,sweet drugs,bitter drugs and drugs that belong to liver,kidney and spleen meridians.3.Based on the analysis of the nature,taste and meridian tropism of drugs,most of them are cold drugs,mild drugs,sweet drugs,bitter drugs and drugs that belong to liver,kidney and spleen meridians.Based on association rule analysis,Astragalus ? Achyranthes bidentata,Imperata root ? Ligustrum lucidum,astragalus,Eclipta ? Ligustrum lucidum,Imperata root,Astragalus are the most commonly used combination of two,three and four drugs respectively;based on cluster analysis,the commonly used prescriptions are Erzhiwan and Liuweidihuangwan.4.Based on the analysis of modern pharmacology,the traditional Chinese medicine for treating hematuria of chronic nephritis has the most influence on immune regulation and anti-inflammatory drugs,among which the drugs for invigorating kidney and spleen are the most in immune regulation,while the anti-inflammatory traditional Chinese medicine has the function of clearing heat and cooling blood.
