Objective: In this project,after the establishment of a chronic wound model in rats,Xiangpi Shengji Ointment was used to intervene and treat,and the changes in the contents of TNF-?,IL-1?,IL-6,IL-10 and VEGF in the wound tissue were dynamically observed at three time points,To explore its micro-mechanism in the treatment of chronic wounds,and to provide relevant experimental theoretical basis for the clinical application of Xiangpi Shengji Ointment in the treatment of chronic wounds.Methods: After 7 days of adaptive feeding,18 SD male rats were stratified according to their body weight and selected as the blank group by random number table method.The remaining rats were modeled with chronic wounds using the full-thickness skin defect method such as Academician Fu Xiaobing.The model was successfully established.Afterwards,the rats were divided into experimental group,control group and model group by the same method,with 18 rats in each group.After grouping,the experimental group and the control group were treated with Xiangpi Shengji cream and recombinant bovine basic fibroblast growth factor gel respectively.The model group and the blank group were treated with external application of normal saline.At 3 days,7 days,and 14 days after treatment,6 rats were randomly selected from each group to record the wound healing rate.Wound tissues were used to detect TNF-?,IL-1?,IL-6,IL-10,The expression level of VEGF protein,and microscopic observation of histopathological changes.Results: During the experiment,the rats in each group had stable vital signs,normal mental and activity status,and normal food and water intake.There was no death in each group of rats,and the wound application position was good.The healing rate of the experimental group and the control group was higher than that of the model group at 3,7,and 14 days after treatment(P<0.05).On the third day after treatment,except for the blank group,tissue necrosis areas were observed,a large number of inflammatory cells existed locally and tissue edema,and more new capillaries were observed in the experimental group and the control group.On the 7th day after treatment,a large number of fibroblasts proliferated in the experimental group and control group,and a large number of new small and round capillaries were distributed.There were still a large number of inflammatory cell infiltration in the wound of the model group,only a few fibroblasts were visible,and the cells were scattered..On the14 th day after treatment,it was observed that the number of local inflammatory cells in the experimental group and the control group was relatively lower than before,a large number of collagen fibers and mature spindles formed fibroblasts,and capillaries were abundant.There are still more inflammatory cells in the model group,but relatively less than before,and there are fewer fibroblasts scattered and distributed.Analysis of the expression of wound factors among the groups: on the third day after treatment,the expressions of TNF-?,IL-1?,and IL-6 in the wounds of the experimental group,control group,and model group were all higher than those of the blank group,and the model group was higher than the other three groups(P <0.05).The expression of VEGF in the blank group was higher than that of the other three groups,while IL-10 was higher than that of the experimental group and the model group,and the model group was the lowest(P<0.05).On the 7th day after treatment,the expressions of TNF-?,IL-1? on the wounds of the experimental group,control group,and model group were higher than those of the blank group,and TNF-?,IL-1?,and IL-6 in the model group were higher than those of the other groups(P<0.05).The expression of IL-10 in the wound of the blank group was higher than that of the other groups,and the expression of IL-10 in the model group was the lowest(P<0.05).The expression of VEGF in the experimental group was higher than that in the blank group and the lowest in the model group(P<0.05).On the 14 th day after treatment,the protein expression of IL-1?,TNF-? and IL-6 in the model group was higher than the other three groups,and IL-10 and VEGF were lower than the other three groups(P<0.05).Trend analysis of wound tissue factors in each group: The expression of each factor in the blank group was relatively stable.In the experimental group,IL-1? showed an overall downward trend after treatment,TNF-? showed a downward trend before and after treatment,IL-6 showed a significant decline in the middle of the treatment and then was relatively flat,IL-10 showed a first increase and then a flat trend,VEGF showed a first increase and then a decline trend.In the control group,IL-1?,TNF-?,and IL-6 showed a downward trend before and after.There was no obvious downward trend before and after IL-10.VEGF showed an overall upward trend and then a downward trend.Model group: IL-1? expression showed a stable trend before and after,TNF-? expression showed a stable trend before and after the increase,IL-6 showed an upward trend before and after,IL-10 showed a first decline and then an upward trend,and VEGF showed a slow upward trend overall.Conclusion:(1)Xiangpi Shengji Ointment can effectively inhibit the expression of TNF-?,IL-1?,and IL-6 in chronic wounds,increase the expression of IL-10,and alleviate the local inflammation of the wound.(2)Xiangpi Shengji Ointment can regulate the expression of two different types of factors,pro-inflammatory factors and anti-inflammatory factors,and may have the effect of regulating the balance of these two types of factors.(3)Xiangpi Shengji Ointment can effectively increase the expression of VEGF in chronic wounds,promote the angiogenesis of wound tissues,and improve the circulation of wound tissues.(4)Xiangpi Shengji Ointment regulates TNF-?,IL-1?,IL-6,IL-10,VEGF is a dynamic process,and the overall expression trends of different factors before and after have different characteristics.(5)Xiangpi Shengji Ointment can effectively promote the healing of chronic wounds and accelerate the speed of wound healing.
