Construction Of A Qualification System For Emergency Nurse Specialists

Objective:Through literature review and specialist emergency nurses at home and abroad,and other professional nurse specialist qualifications,the relevant provisions of the construction of the emergency of specialist nurse training at the early stage of the research scheme on the basis of using nurses clinical ladder program,through the Delphi method to build a set of obeys the law of development of China’s emergency specialist nurse certification system.Methods:1.Literature review: Searching databases including CNKI,Wanfang,VIP,CBM,CINAHL Complete,Embase,Pub Med,Cochrane Library and Web of Science,Meanwhile,the websites of national and provincial health authorities,Chinese nursing association and provincial/municipal nursing association,American Nursing Certification Center（ANCC）,Australian Nursing and Midwifery Certification Council（ANMAC）,Japan Nursing Association（NA）,Canadian Nurses Association（CNA）were searched from inception to April 2021.To understand the research status of emergency nurse qualification certification institutions,certification requirements,certification procedures,assessment methods and re-certification requirements and extract relevant items,and initially draw up a draft emergency specialist nurse qualification certification system.2.Delphi method: Two rounds of consultation were conducted among 18 members of emergency committee of Chinese Nursing Association from 12 provinces/cities by Delphi method.Excel 2010 was used to double data input,and the data was imported into SPSS 26.0 software for statistical analysis.The corresponding indicators items were modified based on expert opinions and inclusion criteria（significance score ≥3.5,coefficient of variation ≤0.25 and full score ratio ≥20%）,and the qualification certification system of emergency specialist nurses was finally formed.Result:1.Literature review: A total of 3574 articles were obtained by searching related databases and management websites,and 1381 articles were obtained by using Endnote20 software for rectifying.After reading the title and abstract according to the literature inclusion criteria,31 literatures were included,covering the certification management organization of emergency specialist nurses,application conditions,assessment contents of re-certification,assessment criteria of re-certification,methods and conditions of re-certification,etc.2.Delphi method: The experts who conducted the two rounds of consultation came from 12 provinces/cities including Beijing,Shandong and Hunan,professional fields including emergency specialist nursing,nursing education and nursing management.After two rounds of consultation,the effective rate of questionnaire was 90% and 100%,the expert authority coefficient was 0.95,the Kendall’s W was 0.202 and 0.338（P < 0.05）.In the first round of consultation,4 concept definitions and 44 indicators were modified,2 indicators were merged,5 indicators were added,and 1 indicator was deleted;The second round of consultation modified 2 concept definitions and 6 indicators.The final emergency nurse qualification system includes 5 first-level indicators,15 second-level indicators and 92 third-level indicators.Conclusion: The consulting experts in this study were widely distributed geographically and had good representativeness in their professional fields.The Experts activeness,authority and coordinated are good,and the results are scientific and reliable.The qualification certification system is connected with the training program,highlighting the assessment requirements for the qualification certification of emergency specialist nurses at all levels,which can provide reference for the establishment of the assessment system of emergency nurse qualification certification in China.
