| Objective:To clarify the structure of public perception of doctor-patient relationship risk, compile the doctor-patient perceived risk questionnaire from the public perspective in line with the requirements of psychological measurement, and to analyze the demographic factors affecting the level of public doctor-patient perceived risk.Methods:(1)According to the results of literature review and qualitative interviews, the item pool of the scale was constructed.Combined with group discussion and expert advice, the initial questionnaire of doctor-patient perceived risk from the public perspective was developed.(2)Through the method of convenient sampling, the initial questionnaire of doctor-patient perceived risk was used to collect the public perceived risk data, and the data were randomly divided into two samples according to the ratio of1:1.(1)Sample 1:The items were screened by item analysis and exploratory factor analysis to form the formal version of the doctor-patient perceived risk questionnaire from the public perspective.(2)Sample 2:The reliability and validity tests of the doctor-patient perception risk questionnaire were conducted:internal consistency,split-half reliability, structural validity, content reliability, and confirmatory factor analysis were used to test the reliability and validity of the questionnaire.(3)Using the doctor-patient perceived risk questionnaire to investigate the current situation of doctor-patient perceived risk of the public. The influence of demographic sociological variables on the level of public perceived risk was discussed by using statistical methods such as correlation analysis,t-test,analysis of variance and multiple stepwise regression.Results:(1)Development of the Doctor-Patient Perceived Risk Questionnaire.(1)preliminary construction of doctor-patient perceived risk questionnaire:through research literature, qualitative interview, expert consultation, item analysis and exploratory factor analysis of the questionnaire data of 260 public, the doctor-patient perceived risk questionnaire consisting of 13 items divided into 3 dimensions(responsibility cognition dimension, individual decision-making risk dimension, and physician-patient trust risk dimension)was finally formed. The cumulative variance contribution of the 3 extracted common factors was 60.151%.(2)Validation of the doctor-patient perceived risk questionnaire:test the reliability and validity of the doctor-patient perceived risk questionnaire. The overall Cronbach’s a coefficient of the questionnaire was 0.856,and the Cronbach’s a coefficients of the responsibility cognition risk dimension, individual decision risk dimension, and physician-patient trust risk dimension were 0.734,0.806 and 0.873 respectively. The overall split-half reliability of the questionnaire was 0.713,and the split-half reliability of each dimension was 0.728,0.780 and 0.823,and the content validity(S-CVI)of the 13 items was 0.935.All indicators of confirmatory factor analysis and the fitting test of structural model were within the standard range(RMSEA(0.060)<0.08,PGFI(0.635)>0.5,(?)~2/df(1.922)<3,AGFI(0.900)>0.8,GFI(0.932)>0.9,TLI(0.947)>0.9,CFI(0.958)>0.9,IFI(0.958)>0.9,NFI(0.916)>0.9,PNFI(0.728)>0.5).(2)Influencing factors of public doctor-patient perceived risk level:(1)Correlation analysis:the score of public responsibility cognition risk dimension was significantly positively correlated with age(r=0.12),marital status(r=0.12),household registration type(r=0.13)and type of favorite movies and TV shows(r=0.12),and significantly negatively correlated with occupation type(r=-0.11)and medical payment method(r=-0.10)(p<0.05);the score of individual decision-making risk dimension was significantly negatively correlated with the number of children(r=-0.11),household registration type(r=-0.17),occupation type(r=-0.12)and medical payment method(r=-0.17),and positively correlated with the level of education(r=0.22)and the type of favorite movies and TV shows(r=0.11);the public doctor-patient perceived risk score has a significant positive correlation with education level(r=0.13),household registration type(r=0.16),the type of favorite movies and TV shows(r=0.12),a significant negative correlation with occupation type(r=-0.09),medical payment method(r=-0.12)(p<0.05).(2)Single factor analysis:except that there was no significant difference in the perceived risk scores of the public with different gender, number of children, occupation type and per capita monthly income of different families(p>0.05),there were significant differences in the scores of public responsibility cognition risk dimension among different ages, marital status, household registration types and medical payment methods; There are significant differences in the scores of individual decision-making risk dimension among the public with different education levels, marital status, household registration types and medical payment methods; There are significant differences in the scores of doctor-patient trust risk dimension among the public with different household registration types; The two influencing factors of different household registration types and the type of favorite movies and TV shows had statistical significance in the total score of doctor-patient perceived risk(p<0.05).(3)Multiple stepwise regression analysis:We take the relevant factors of the total score and the scores of each dimension and the factors with statistical significance in the analysis of variance as independent variables, and take the total score and the scores of each dimension as dependent variables for multiple linear regression analysis. Finally, the regression equation of the responsibility cognitive risk dimension includes two variables:marital status and the type of favorite movies and TV shows; The regression equation of individual decision-making risk dimension includes four variables:education level, marital status, medical payment method and the type of favorite movies and TV shows; The regression equation of doctor-patient trust risk dimension includes one variable:the type of household registration; The regression equation of the total score of doctor-patient perceived risk includes two variables:the type of household registration and the type of favorite movies and TV shows.Conclusion:(1)The doctor-patient perceived risk questionnaire from the public perspective has good reliability and validity, and can be used as a tool to evaluate the level of public doctor-patient perceived risk.(2)The public’s marital status and the type of favorite movies and TV shows are the main factors affecting the public’s perception of responsibility cognition risk; The main factors affecting the public’s perception of individual decision-making risk are marital status, the level of education, medical payment method, and type of favorite movies and TV shows; The type of household registration is a major factor influencing the public’s perception level of doctors-patients trust risk. The type of household registration and the type of favorite movies and TV shows are the main factors affecting the public’s perception level of doctors-patients risk. |
