Objective:To investigate the effect of pelvic resistance exercise therapy on the get-up movement of stroke patients with hemiplegia,and to explore the continuous effect of pelvic resistance exercise therapy on the get-up movement and on the motor function for hemiplegic patients Methods:Sixty patients with hemiplegia due to hospitalization in the department of Rehabilitation,Baoji City People’s Hospital of Baoji City and the rehabilitation department of Baoji High-tech Zone People’s Hospital from January 2018 to September 2018 were selected.All the subjects met the diagnostic criteria for cerebrovascular disease.It was confirmed by cranial CT or MRI,and the condition was stable,no other neurological diseases,no vestibular function or cerebellar dysfunction,no serious orthopedic diseases and complications affecting rehabilitation training.According to the principle of randomized control,the study group was divided into the study group and the control group.The control group received the training of getting up movement.The study group received pelvic resistance exercise therapy.The get-up time,ADL score,muscle strength,transfer ability score,and 10 m walking ability before and after treatment were compared for the two groups of patients.Results:(1)Before the treatment,the get-up time of the control group was 9.90±6.36 s,and the get-up time of the study group was 9.10±4.65 s.After one treatment,the get-up time of both groups was shorten,the get-up time of the control group was 8.03±4.79 s,and the get-up time of the study group was 6.00±3.01 s.Compared with before treatment,P<0.05,the difference was statistically significant.(2)The effects of grouping and time on get-up time were statistically significant(F =7.501,P=0.008;F=58.05,P<0.001),that is,the study group after treatment was significantly better than that of the control group.There was no improvement in the control group,and the difference was not statistically significant(F=6.683,P<0.001),and after pairwise comparison between the two groups,it was found that the get-up time was shortened significantly after four-week treatment compared with that before treatment.(3)After 4 weeks of continuous treatment,the ADL score,muscle strength,transfer ability and 10 m walking ability of patients in the control group and the research group were significantly improved than before treatment.For the forward superior pelvic muscle strength,repeated measurement anova found that after treatment,the improvement effect in the study group was significantly better than that in the control group(F=17.002,P<0.001),and both time and interaction effects were affected(F=82.754,P<0.001;F=20.992,P<0.001),the study group showed significant improvement in the effect of the forward superior pelvic muscle strength after treatment(F=66.252,P<0.001),and the comparison between the groups showed significant improvement after 1 week of treatment,while the control group showed no improvement until 4weeks of treatment.For the posterior pelvic compression muscle strength,there were significant differences in grouping effect(resistance group vs.training group),time effect and interaction effect(F=18.052,P<0.001;F=55.463,P<0.001;F=7.132,P<0.001),the pelvic muscle strength test result was significantly better in the study group than in the control group,and the study group showed significant improvement after 1 week of treatment,while the control group showed improvement after 3 weeks of treatment.For the transfer ability,the results of repeated measurement anova showed that time effect and interaction effect had a statistically significant difference on the score of transfer ability(F=96.539,P<0.001;F=3.274,P=0.023).The difference between the study group before and after treatment was statistically significant(F=18.866,P<0.001).The comparison of the differences between the groups showed that the treatment effect was better at1-4 weeks than before treatment,and the improvement effect was continuously improved.The control group was also improved after treatment,but it did not improve until 3 weeks after treatment,and the study group was significantly better than the control group.Repeated measures anova showed that time and interaction had effect on 10 m walking ability(F=147.051,p<0.001;F=5.461,p=0.005).After treatment,the effect of the study group was significantly improved(F=7.743,p<0.001).Pairwise comparison showed that there was a difference between 3 weeks of treatment and that before treatment,and betweeen 4 weeks of treatment and that before treatment,and between 4 weeks of treatment and the control group.Therefore,the treatment effect of the study group was significantly better than that of the control group.Time effect and interaction had an effect on ADL score,and the difference was statistically significant(F=145.32,p<0.001;F=6.866,p<0.001).There were statistically significant differences between the groups before and after treatment(F=7.18,p<0.001,F=3.334,p=0.012).Pairwise comparison between groups showed that the ADL in the study group was significantly improved after 3 weeks of treatment compared with that before treatment,and that ADL was significantly improved after 4 weeks of treatment compared with that before treatment and 1 week of treatment,and that the study group was significantly better than the control group.Conclusion:(1)Both get-up movement training and pelvic resistance exercise therapy can effectively improve patients’ instant get-up time,but pelvic resistance exercise is more effective.(2)Both get-up movement training and pelvic resistance therapy can continuously improve patients’ get-up time,but the improvement effect of pelvic resistance exercise is better.(3)Compared with the get-up movement training,the application of pelvic resistance exercise therapy has more therapeutic advantages for the improvement of ADL,10 m walking ability and transfer ability of stroke patients with hemiplegia.
