ObjectiveIn this study, the objective score, subjective score,DLQI score, adverse reactions and relapse rate of patients in the three groups were observed and compared, and conducted a preliminary discussion on the mechanism of treatment,in order to provide an effective clinical practice for the treatment of blood-deficiency and wind-dry limited neurodermatitis with the combination of cotton moxibustion with Siwurunfu Decoction.MethodsSeventy-five cases were selected from August 2020 to August 2021 who attended the dermatology outpatient clinics of Hanyang and Hankou Hospital of Wuhan Traditional Chinese Medicine Hospital and met the clinical evidence diagnosis and inclusion criteria of this study,7 cases were dislodged due to various factors, and 68 cases were finally collected and randomly divided into 23 cases in the treatment group(Siwurunfu Decoction combined with cotton moxibustion),23 cases in the control A group(cotton moxibustion),and 22 cases in the control Group B(Siwurunfu Decoction)22cases.In all three groups, vitamin E cream was applied topically twice daily. All three groups were treated in a cycle of 8 weeks. The objective lesion symptom scores, subjective symptom(itching and insomnia)scores and quality of life of patients in each group before and after treatment were observed and recorded, and the treatment effects of each group were analyzed statistically to evaluate the clinical efficacy of the three groups. In order to comprehensively assess the safety of the treatment regimen,it was necessary to observe and record the occurrence of adverse reactions in each group during the treatment period. In addition, two months after the treatment, patients in each group were followed up,so as to understand the recurrence.Results1.Before treatment, there were no statistical differences(P>0.05)between the three groups of patients regarding baseline data(gender, age, disease duration, lesion location, subjective and objective symptom scores, and quality of life scores),which were comparable.2.Comparison of objective symptom scores: the objective symptom scores of patients in all three groups improved significantly after treatment(P<0.05).At the end of 4 and 8 weeks of treatment, there was a difference between the combined group and the two control groups respectively(P<0.05),and there was no difference between the cotton moxibustion group and the Chinese medicine group(P>0.05),according to the data,it is obvious that the two control groups were lower than the combined group in improving the objective symptoms.3.Comparison of subjective symptom scores: after treatment, the mean of the difference in subjective symptom scores of patients in three groups improved clearly(P<0.05).At the end of 4 weekends of treatment, the subjective symptom scores of the combined group were lower than those of the cotton-laying moxibustion group, which was statistically different(P<0.05),and there was no significant difference between the combined group and the Chinese medicine group and the cotton-laying moxibustion group and the Chinese medicine group(P>0.05).After 8 weeks of treatment, there was a difference between the combined group and the two control groups and between the cotton moxibustion group and the Chinese medicine group(P<0.05),from the data,it is clear that the combined group is better than the Chinese medicine group than the cotton moxibustion group in improving the subjective symptoms.4.Comparison of life quality scores: after treatment, the DLQI scores of patients in all three groups were obvious improved(P<0.05).(P<0.05).At the end of 8 weeks of treatment, there was a difference between the combined group and the two control groups,respectively(P<0.05),and there was no difference between the two control groups(P>0.05),from the data,it is clear that the combined group has the best effect on the improvement of quality of life.5.comparison of efficacy: after 4 weeks’ treatment, the efficacy was evaluated in the three groups, and the combined group(86.96%)> Chinese herbal medicine group(77.27%)>cotton moxibustion group(69.57%),and there was a difference between the combined group and cotton moxibustion group(P<0.05),and there was no difference between the Chinese herbal medicine group and the combined group and cotton moxibustion group,respectively(P>0.05);at the end of 8 weekends of treatment, the combined group(95.65%)> Chinese medicine group(86.36%)> cotton moxibustion group(78.26%),and there was a difference between the combined group and the two control groups respectively(P<0.05),and there was no difference between the cotton moxibustion group and the Chinese medicine group(P>0.05),from the data,it is clear that the combined group has better clinical efficacy.6.There was one case(4.35%)of adverse reaction in the combined group and one case(4.35%)in the cotton moxibustion group during the treatment process, with mild symptoms, and no adverse events in the traditional Chinese medicine group, with a low incidence of adverse reactions(P>0.05).7.The recurrence rate in the combined group was lower than that of the Chinese medicine group and lower than that of the cotton moxibustion group at the end of the treatment course with a follow-up of 2 months.ConclusionThe combination of Siwurunfu Decoction with cotton moxibustion can effectively treat blood-deficiency and wind-dry type of limited neurodermatitis, which is better than single treatment in terms of reducing the intensity of itching, improving the condition of skin lesions, sleeping condition and quality of life, with good clinical efficacy, low recurrence rate and safer. Therefore, the efficacy of combining Siwurunfu Decoction with cotton moxibustion in the treatment of blood-deficiency and wind-dry neurodermatitis is objective and worthy of clinical promotion.
