| Objective:1.This study aimed to investigate the current status of the emotional intelligence,nurse-patient communication competency and job burnout of emergency nurses,and analyzed the influencing factors of job burnout.2.It aimed to explore the relationship among emotional intelligence,nurse-patient communication competency and job burnout of emergency nurses,to explore the possible mediating role of nurse-patient communication competency between emotional intelligence and job burnout of emergency nurses,so as to provide theoretical basis for nursing managers to stabilize the emergency nurse team,ensure medical quality and safety and promote the stability and development of emergency nursing in China.Methods:This is a cross-sectional study,and questionnaire survey was used to collect data.1.From May 2019 to December 2019,336 emergency nurses from 6 A-grade tertiary hospital in Shandong Province were selected by convenience sampling method of the questionnaire survey,General information questionnaire,Wong and Law’s Emotional Intelligence Scale,Nurse’s Clinic Communication Competency Scale and Maslach Burnout Inventory were used to collect the data of sociodemographic and emotional intelligence,nurse-patient communication competency and job burnout.2.SPSS 21.0 statistical software was used for descriptive analysis,one-way ANOVA,Pearson correlation analysis and multiple linear stepwise regression analysis and mediating effect verification were performed;AMOS 23.0 was used to construct the structural equation model.Results:1.The current status of the emotional intelligence,nurse-patient communication competency and job burnout of emergency nurses(1)The total score of emergency nurse’s emotional intelligence was(58.78 ± 9.98),and the average scores of the four dimensions of the scale were ranked from high to low as follows: Self-emotion monitoring ability(4.04 ± 0.71),emotion utilization(3.62 ±0.81),emotion recognition of others(3.53 ± 0.78),and emotion regulation(3.51 ± 0.79).(2)The total score of nurse-patient communication competency of emergency nurses was(169.46 ± 35.53),and the average scores of the six dimensions of the scale from high to low were as follows: Team communication ability(2.97 ± 0.95),emotional support ability(2.95 ± 0.91),basic nonverbal communication ability(2.93 ± 0.93),emotional perception ability(2.92 ± 0.93),difficult situation support ability(2.91 ± 0.89),and basic language communication ability(2.88 ± 0.91).(3)The total score of emergency nurse’s job burnout was(68.02 ± 14.85),and the average scores of the three dimensions of the scale from high to low were as follows:Emotional exhaustion(3.20 ± 1.26),depersonalization(3.18 ± 1.38)as well as low personal accomplishment(2.92 ± 1.23).According to the job burnout score,22(6.54%)had no job burnout,53(15.77%)had mild job burnout,134(39.88%)had moderate job burnout and 127(37.79%)had severe job burnout.2.Influencing factors of emotional intelligence,nurse-patient communication competency and job burnout of emergency nurses(1)Factors such as age,number of children,education background,emergency service years,professional title,position,nursing level,monthly income and marital status had statistical significance in emotional intelligence scores(P<0.05).(2)There were statistically significant differences in age,marital status,number of children,education background,emergency service years,monthly income,professional title and position in nurse-patient communication competency scores(P<0.05).(3)There were statistically significant differences in emotional exhaustion dimension scores among nurses of different age,marital status,number of children,emergency service years,monthly income,professional title,position and nursing level(P<0.05);There were significant differences in depersonalization dimension scores among nurses of different age,marital status,emergency service years,monthly income,professional title,position and nursing level(P<0.05);Different age,different marital status,different number of children,different emergency service years,different monthly income,different nursing level in the dimension of personal accomplishment score difference was statistically significant(P<0.05);There were statistically significant differences in occupational burnout scores among nurses of different age,marital status,number of children,emergency department service years,monthly income,professional title,position and nursing level(P<0.05).(4)The multiple stepwise regression analysis of influencing factors showed that emergency working years,monthly income,position,nursing level,emotional intelligence,and nurse-patient communication competency significantly affected the job burnout of emergency nurses.3.The relationship among emotional intelligence,nurse-patient communication competency and job burnout of emergency nursesThere was a significant positive correlation between emotional intelligence and nurse-patient communication competency of emergency nurses(r= 0.273,P<0.01);A significant negative correlation between emotional intelligence and job burnout(r=-0.496,P<0.01);and a negative correlation between nurse-patient communication competency and job burnout(r =-0.288,P<0.01).4.The mediating role of nurse-patient communication competency betweenthe emotional intelligence and job burnout of emergency nursesThe emotional intelligence level of emergency nurses was significantly negative in predicting job burnout(B =-0.668,P<0.001);the nurses’ emotional intelligence level directly and positively predicted the nurse-patient communication competency(B= 0.333,P<0.001).Structural equation model analysis showed that nurse-patient communication competency played a partial mediating role between emotional intelligence and job burnout [b=-0.075;95% confidence interval:(-0.018,-0.146)],and the proportion of mediation effect in the total effect was 10.09%.Conclusion:1.Emergency nurses generally have job burnout problems;the job burnout reached the level of high burnout,mainly moderate and severe.2.Emergency working years,monthly income,position,nursing level,emotional intelligence,and nurse-patient communication competency had a significant impact on the job burnout of emergency nurses.3.The emotional intelligence and nurse-patient communication competency of emergency nurses were in the middle level.Both emotional intelligence and nurse-patient communication competency can affect job burnout,the nurse-patient communication competency of emergency nurses played a part of mediating role between the emotional intelligence variable and the job burnout variable.Therefore,it is suggested that nursing managers pay more attention to emotional intelligence and nurse-patient communication competency,and reduce the job burnout by improving emotional intelligence and nurse-patient communication competency. |
