Objectives:To understand the status of subjective well-being in COPD patients with anxiety and depression;To observe the effects of positive emotion disclosure on anxiety and depression,quality of life and subjective well-being of COPD patients complicated with anxiety and depression;To compare the results of two different positive emotion disclosure methods in patients with COPD complicated with anxiety and depression.Methods:This research belongs to randomized control trials.Ninety-two patients with COPD complicated with anxiety and depression who were admitted to a hospital and community hospital in Huzhou City from October 2019 to December 2020 were selected as the research subjects strictly according to the inclusion and exclusion criteria,using the random number table method to group,according to the order of coming into group,patients were randomly divided into the group of written disclosure positive emotion and the group of individual face to face disclosure positive emotion,group 46 cases respectively,10 cases fall off in the process of intervention altogether,the final valid sample of is 82 cases.Two groups of patients were accepted routine treatment and nursing,written disclosure group was given a manual for positive emotional disclosure,individual face to face disclosure group were given face to face positive emotional disclosure intervention,the time of intervention for two groups is 30 minutes per time,once a day for the first week,then once a week,lasts 8 weeks.Baseline levels of the two groups were compared using a self-designed general information questionnaire,Self-rating Anxiety Scale,Self-rating Depression Scale,COPD Quality of Life Scale,General Well-being Scale before interventions.Acceptance scores about this week theme were collected weekly after the intervention for the first 6 weeks,Anxiety and depression,quality of life,subjective well-being of the two groups were evaluated again and their assessment of the interventions were collected through interview after 8 weeks to compare the differences of both groups before and after intervention and the differences between two groups.Data entry was double-checked after intervention.SPSS23.0 software was used for data collation and analysis.The statistical methods included statistical analysis,independent sample t-test,chi-square test,paired t-test,etc.All statistical analyses were conducted on a two-sided basis,and showed statistical significance when P<0.05.Results:1.During the intervention,4 cases in the written disclosure group were lost,6 cases in the face-to-face disclosure group were lost,and the final effective samples were 82 cases.There was no statistical difference between the general information of the subjects in the two groups(P>0.05).2.The total score of anxiety level in the written disclosure group before intervention was(57.82±4.02),the total score of depression was(59.26±3.193),quality of life score(21.45±4.778),the score of overall subjective well-being was(61.92±8.90);The total score of anxiety level in face-to-face disclosure group before was(57.92±3.735),the total score of depression was(59.80±3.99),quality of life score(22.08±4.968)and the score of overall subjective well-being was(63.50±9.568).An independent sample t-test was used to compare the scores of anxiety and depression,quality of life and overall subjective well-being between the two groups before intervention,Between the two groups,there was no statistical difference(P>0.05).The baseline level of the two groups was the consistent and comparable.3.Intra-group comparison: before and after interventions,there were respectively significant difference in the level of anxiety,depression and subjective well-being of two groups(P<0.05),but there was no significant difference in quality of life before and after interventions(P>0.05).4.After 8 weeks of interventions,the anxiety scores of the face-to-face disclosure group were significantly lower than that of the written disclosure group,and the SAS scores between the two groups were statistically different(P<0.05).After interventions,the GWB scores of the face-to-face disclosure group were significantly higher than that of the written disclosure group,there was statistical difference between the two groups(P<0.05).There was no significant difference in SDS scores and CAT scores between the two groups(P>0.05).Conclusions:1.Both of the two different ways of positive emotion disclosure can alleviate the anxiety and depression of patients with COPD combined with anxiety and depression,improve their subjective well-being,but the change of life quality still needs further exploration.2.Face-to-face positive emotion disclosure is better to improve anxiety level and subjective well-being score than writing disclosure positive emotion,but the intervention effect of depression level and quality of life is the same.3.From the perspective of cost-effectiveness,written disclosure positive emotions is more economical,simple and easy to implement,and reduces the consumption of nursing labor cost.It is suggested that it can be used according to the clinical actual situation.
