| Objective: This study retrospectively analyzed the clinical data of outpatient clinic cases,summarized the characteristics of the treatment of type 2 diabetes mellitus(T2DM)by Lianzhu Xiaoke prescription,observed its clinical effect,summarized professor Yan Yong’s experience and academic thought,provided objective basis and theoretical support for prospective study,and provided new ideas and methods for the diagnosis and treatment of this disease.Methods: A retrospective study was conducted to select the outpatients with type 2diabetes mellitus(T2DM)treated by Professor Yan Yong from 2015 to 2018 at Kaifeng Hospital of traditional Chinese medicine,the study included basic information on gender,age,Body Mass Index,disease Course,complications),laboratory examination of Fasting Blood Glucose,2 hours Postprandial Blood Glucose,Glycated hemoglobin,total cholesterol,triglyceride,high-density Lipoprotein,Low-density Lipoprotein,Glucagon,insulin resistance index,drug use,type and frequency of traditional Chinese medicine,type and dosage of Western medicine for hypoglycemic agents before and after treatment.The clinical efficacy of Lianzhu xiaoke prescription was evaluated by TCM symptoms,Blood Glucose index,blood lipid index,Body Mass Index,insulin resistance index and Glucagon.Results:(1)General Data: A total of 138 eligible cases were included in this study,of which 81 were males and 57 were females,over five years,those with a BMI between24 and 28 were more likely.Dyslipidemia was the most common complication,followed by hypertension,fatty liver,coronary heart disease and Hyperuricemia.Among the diabetic complications,the most common was diabetic peripheral neuropathy,followed by diabetic nephropathy,diabetic gastroenteropathy,diabetic retinopathy,Among the 51 patients treated with combined hypoglycemic agents,the most common was Metformin,followed by Glimepiride,acarbose,etc.(2)analysis of curative effect: The effective rate of TCM symptoms was 86.23%,and the symptoms of thirst and thirst were better than those before treatment(P<0.05).The effective rate of blood glucose was 87.69%,fasting blood glucose,2 hours postprandial blood glucose and glycosylated hemoglobin were lower than before treatment(P<0.05).Total cholesterol,triglyceride,high density lipoprotein,low density lipoprotein,body quality index,insulin resistance index and glucagon were different before and after treatment(P<0.05).(3)application of Medicine: Radix codonopsis Pilosula,Rhizoma atractylodis macrocephalae,fructus coix Lachryma,fructus Amomi were the most frequently used flavoring drugs,followed by Rhizoma Atractylodis macrocephalae,Rhizoma trichosanthis,semen coicis Amomum,etc.Among the 51 patients treated with hypoglycemic Western medicine,the number of patients treated with Trimethoprim was reduced from 9 to 6,the number of patients treated with Trimethoprim was reduced from 24 to 16,and some patients stopped or reduced their medication.Conclusion:(1)Lianzhu Xiaoke prescription has the curative effect of Lowering Blood Sugar,regulating blood fat,improving insulin resistance,reducing Glucagon secretion and reducing weight.(2)Lianzhu Xiaoke prescription can improve the clinical symptoms of type 2diabetes mellitus patients with dampness-heat Syndrome and blood stasis syndrome,such as thirst,no desire to drink,heavy head and body,shortness of breath and idle speech.(3)Professor Yan Yong holds that the core pathogenesis of T2 DM With Syndrome of dampness-heat intermingling and stasis is the accumulation of dampness-heat,the loss of spleen-energy is its initial factor,the key link is the imbalance of qi-energy,and blood stasis runs through the whole course of disease,the main therapeutic principle of purging turbid and removing blood stasis is often combined with invigorating the Spleen and invigorating Qi,awakening the spleen and removing dampness,etc. |
