"When the flow along the two Yang meridians was stagnated, it is referred to as Xiao(消) "quoted from the book - Yin Yang Bie Lun, Su Wen. Here, the two Yang meridiansrefer to the Yangming meridians, which generally the names for the meridians for stomachand large intestines. Xiao is a general term for a specific group of symptoms in which thepatient was accounted for diabetes, while stagnation refers to the accumulated dry-heatpathogens in the body. This leads to excessive heat in both the stomach and large intestinemeridians which consequently exhausts the bodily fluid, which may eventually result indiabetes. Due to the complex etiology and pathogenesis, the course of diabetes is usually long,and has the tendency in developing into other pathological complications. Up to date, thereare still no effective methods in curing diabetes. Having sieved through large amounts ofancient medical literature and current medical case studies, it is necessary to reinvestigatethe role played by the Yangming meridians and their related organs in diabetes. This project discussed the relationships between the Yang meridians and diabetes, anddivided into several sections. Firstly, a summarisation of ancient and modern medicalliterature was carried out to investigate the clinical evidence in treating diabetes. It wasfound that the traditional theoretical recognition of yin-deficiency and dryness-heat intreating diabetes is different from our current and many pathogenesis theories for diabetesare still left unexplained. The textual research on origin and retrospection of the distinctivedissertation and therapeutic methods in treatments of diabetes by Chinese doctors in the pastdynasties, were also carried out. The second part of the research includes the statistical investigation on the distinctiveindividual herbs or herbal prescriptions used for diabetes in traditional Chinese medicine.The results helped to provide preliminary evidence in relating the Stagnation of two Yangmeridians and diabetes. It suggested that in Nei Jing, two yangs referred to not only meridians and collaterals ofzang and fu viscera, but also to the techniques of acupuncture. The two yangs not onlyimplied meridians, collaterals, zang-viscera, and fu-viscera, but also implied to pathologicalsix Qi. On the other hand, Xiao means exhaustion of water and food, body fluids andreduction of muscles. Jie means stagnation of Qi and blood, and retention of pathogens.Research suggested that many diabetes patients may exhibit obvious increase in bloodsugar from five to nine o'clock in the morning, or show increased requiring quantity ofinsulin to sustain normal blood sugar phenomenon, it is called daybreak phenomenon ofdiabetes. The circulating time of stomach and large intestine meridians was also from five tonine o'clock in the morning. Any stagnation in the two meridians would prevent theessences of water and food from being distributed evenly in the body, and thus result in theincrease of blood sugar. Consequently, it expiated the concept from Nei Jing that pathogenesis of diabetes ismainly related with stomach heat. It believed that the accumulated heat in the stomach ispathological culprit, which may lead to diabetes. From the statistical investigations on diabetes prescriptions, 396 ancient recipes, 50modern recipes, and 136 private recipes were gathered. It was aimed to understand thegeneral therapeutic status of diabetes by Chinese medicine physicians throughout thedifferent dynasties. Combining the theories of traditional Chinese medicine and statisticalanalyse, this article further expiates the mechanism of two Yang stagnated is called Xiao. Itspurpose was to supplement the pathogenesis of diabetes, and provide a new platform forfuture clinic practice. It revealed the diversifying course of disease, pathological outcomeslike blood stasis, phlegm and dampness and heat toxins are related to the stagnancy of Qi inthe Yangming meridians.
