| Purposes: This topic is to collect professor Wang Yan-gang’s RE diagnosed by outpatient department,and the basic information of patients with effective treatment,gastroscope results and first-visit medical records,and traditional Chinese medicine prescriptions,with the help of “TCM Inheritance Support System(V2.5)”,a retrospective study on professor Wang Yan-gang’s experience in treating RE was conducted,summarizing the experience and academic thoughts in diagnosis and treatment of RE.Methods:1.To collate and collect RE patients treated by Professor Wang Yan-gang from January 2015 to May 2019 in the Outpatient Department of Digestive Internal Medicine of the Affiliated Hospital of Hebei University of Traditional Chinese Medicine,and to screen 302 valid medical records of RE patients according to the results of gastroscopy or TCM symptoms according to the efficacy criteria.The information includes the basic information of the patient(including name,sex,age),gastroscopy results and first-visit medical records,and Chinese medicine prescriptions(including: engraving disease,tongue,pulse,prescription).2.The above information is input into the database established by the“TCM Inheritance Support System(V2.5)” system provided by the institute of traditional Chinese medicine of the Chinese academy of traditional Chinese medicine.3.With the help of association rules in software and unsupervised entropy hierarchical clustering methods,the network visualization data show the corresponding statistical results,deeply analyze professor Wang Yan-gang’s experience in treating RE and high frequency medication,drug pairs and prescription medication rules for treating RE,and summarize and analyze professor Wang Yan-gang’s academic thoughts on treating RE.Research results:1.Basic conditions of patients: Among 302 patients,195 males and 107 females,mainly aged 41-70 years old,mainly showing heartburn and acid regurgitation.Tongue and pulse are mostly dark red tongue,cracked tongue,trembling tongue,thin yellow greasy coating,and smooth pulse string.Among them,193 cases were RE(not graded),90 cases were RE(LA-A),65 cases were RE(LA-B),and 4 cases were RE(LA-C).2.Prescription analysis: There are 186 traditional Chinese medicines in302 prescriptions,48 of which are used more than 20 times,classified into 17 categories and 13 categories respectively according to efficacy,with antipyretics and qi-regulating drugs appearing more frequently.The four qi and five flavors are mainly cold,warm,bitter and pungent.Channel tropism mainly enters stomach,lung,spleen and liver.Statistics on the dosage of drugs in the top 20 of the frequency are carried out.For example,the most common dosage of scutellaria baicalensis is 12 g,the most common dosage of artemisia capillaris is 15 g,the most common dosage of Bupleurum is 15 g,and the most common dosage of coptis chinensis is 12 g.3.Rule of prescription: Set the number of support degree to 230 and the confidence degree to 0.9,and obtain 8 core drugs.4.New party analysis: Setting correlation degree 9 and penalty degree 4,32 drug pairs with correlation coefficient ≧0.02 were obtained,28 core combinations were mined by clustering,and 14 new prescriptions for treating RE were extracted.Conclusion:1.Based on the statistical results of the TCM Inheritance Support System(V2.5),professor Wang Yan-gang believes that the disease is based on the core pathogenesis of "endogenous stagnation of heat and upward flow of stomach qi".The core treatment method is to clear away stagnation heat and regulate stomach downward flow.The treatment is mainly based on soothing liver and clearing heat,regulating qi and reducing adverse flow.It takes into account the changes of different disease conditions such as stasis,phlegm,dampness,food accumulation and yin injury,and is flexible in treatment.2.The prescription is cold and warm,clear but not cold.The taste of the medicine is mostly bitter and pungent,and the meridian tropism is mostly stomach,lung,spleen and liver.The ancient prescription is used today,and the wonderful methods are applied together,so as to reduce stomach qi,eliminate various diseases and achieve clinical efficacy. |
