Based On Data Mining To Explore Professor Wang Sumei's Experience In Treating Children With ADHD

Objective:In this study,the Traditional Chinese Medicine Inheritance Support System(V2.5)was used to conduct data mining analysis on professor Wang sumei’s Attention Deficit Hyperactivity Disorder(ADHD)medical records in the outpatient department,to summarize professor Wang’s diagnosis and treatment ideas of ADHD and to provide more methods for clinical treatment of the disease.Methods:We collected the cases of ADHD treated by Professor Wang Sumei in the Pediatric Outpatient Department of Dongfang Hospital,Beijing University of Chinese Medicine from August 2019 to December 2020.After the standardization of medical records,54 medical records(168 diagnosis times)meeting the inclusion criteria were obtained.The general information and the four diagnnostic information of the patients were entered into the platform system,and the gender ratio of men and women,age distribution,and meridian tropism of drug-induced taste were statistically analyzed.The frequency of symptoms and syndromes,drug frequency,drug correlation degree,and core drug combination in the source prescriptions were counted by frequency statistics,association rule analysis,entropy clustering and improved mutual information method.Therefore,the distribution of syndromes,medication orientation and potential new prescription of Professor Wang Sumei for the treatment of ADHD were obtained.To sum up Professor Wang’s understanding of ADHD and the ideas and medication rules for the treatment of the disease.Results:1.Among the 54 children enrolled in this study,43 were boys(79.63%)and 11 were girls(20.37%),the ratio of male to female is about 3.9:1,with the prevalence rate of boys significantly higher than that of girls.The treatment age was mainly from 7 to 10 years old,accounting for 64.82%of the total.2.In the 54 medical records studied in this study,there were 18 common symptoms,and the top 7 symptoms were Inattention,hyperactivity,impulse,dry stool,procrastination,anorexia,sleep disturbance.3.This study involves four TCM syndrome types:spleen and kidney deficiency,heart and liver fire 144 times(85.71%),phlegm fire internal disturbance 11 times(6.55%),spleen and kidney deficiency,phlegm dampness internal excess 9 times(5.36%),liver and kidney yin deficiency 4 times(2.3 8%),Among them,the children with ADHD of deficiency of spleen and kidney and hyperactivity of heart and liver fire syndrome had the largest number.4.In this study,Professor Wang Sumei used a total of 146 drugs in the treatment of ADHD,and the top 20 Chinese medicines with the highest frequency of use were:Alpinia oxyphylla Hance,Tortoise plastron,grass-leaf sweetflag,Dragonsbones,Polygala tenuifolia Hance,Cortex Moutan,Ramulus Uncariae cum Uncis,Lily Bulb,Pericarpium Citri Reticulatae,Radix Rehmanniae Preparata,Semen Cassiae,Radix Bupleuri,Radix Gentianae,Rhizoma Coptidis,Rhizoma Gastrodia,Fructus Gardeniae,Radix paeoniae alba,rhizoma pinelliae preparata poria and caulis bambusae in taenia.5.Statistics of meridian tropism,four gases and five flavors:Professor Wang’s prescriptions for ADHD mainly used cold drugs,warm drugs and neutral drugs;The five flavors were mainly sweet,pungent and bitter.Meridian tropism is mainly concentrated in the liver,heart,kidney,spleen.6.Professor Wang’s 74 commonly used Chinese medicine combinations for the treatment of ADHD include 10 Chinese medicines.26 groups were combined with two drugs,31 groups were combined with three drugs,15 groups were combined with four drugs,and 2 groups were combined with five drugs.7.By clustering analysis,28 groups of core drug combinations were obtained and 14 potential new prescriptions were mined.Conclusion:1.Professor Wang Sumei believes that the symptoms of ADHD mainly involve the heart,liver,spleen and kidney.Deficiency of spleen and kidney as well as exuberant heart-liver fire are the main pathogenesis of this disease.2.Professor Wang mainly takes cold and sweet herbs that enter the liver,heart,kidney and spleen meridians.The commonly used herbs include Alpinia oxyphylla Hance,Tortoise plastron,grass-leaf sweetflag,Dragonsbones,Cortex et Radix Polygalae,Cortex Moutan,Ramulus Uncariae cum Uncis,Lily Bulb,and Pericarpium Citri Reticulatae.3.Professor Wang uses "Jingxin anshen decoction" for the treatment of ADHD with the therapeutic principle of "tonifying kidney and invigorating spleen,clearing heart fire and pacifying liver".
