| Background and ObjectiveStroke is currently the main cause of disability in adults.Patients with stroke are often left with motor dysfunction,which greatly affects patients’ daily living ability and quality of life.The dysfunction after stroke is mainly manifested as the body control disorder,which causes the patient to be unable to stand or to increase the risk of falling,and the abnormal gait leading to the patient’s weakened or loss of walking ability.During walking,the flexion of the hip joint is the functional basis of the swing phase of the normal gait cycle.The hip flexion is mainly undertaken by the iliopsoas muscle,together with the rectus femoris and sartorius muscles.After stroke,due to the weakened ability of the central nervous system to control the lower limb muscles,the excitability of the iliopsoas muscle of the affected limb will be reduced,which is manifested as the failure of hip flexion.At present,the clinical treatments for hip flexion function of patients after stroke mainly include intensive hip flexion training of lower limbs,isokinetic muscle strength training,iliopsoas tensile vibration training,etc.However,each of the above therapies have their limitations,such as time consuming,lack of active participation of patients,and high requirements on patients’ functional status.Functional electrical stimulation cycling is a treatment method to produce functional movement in lower limb of stroke patients.The intelligent mode of functional electrical stimulation cycling training can effectively improve the motor function of patients.Therefore,this study observed the clinical effects of FES-cycling on hip flexion and lower limb walking ability in patients with early stroke by combining the surface EMG signals of iliopsoas muscle.MethodAccording to the random number table method,40 patients with early stroke were divided into experimental group and control group with 20 cases each.All patients were given conventional rehabilitation treatment.On this basis,the experimental group was combined with FES-cycling.Observe and evaluate the surface electromyography(s EMG),Fugl-Meyer Assessment of Lower Extremity(FMA),and Functional Ambulation Category Scale(FAC)before and after treatment.ResultsThere was no significant difference in the number of cases,gender,age,course of disease and nature of lesions between the two groups after statistical analysis(P>0.05),which showed comparability.The s EMG,FMA and FAC evaluation results of the experimental group and the control group before treatment showed no significant difference(P>0.05).After 4 weeks of treatment,the s EMG,FMA,and FAC evaluation results of the two groups were significantly improved compared to before treatment,with significant differences(P<0.05),and the results of the experimental group were higher than those of the control group,which was significant difference(P<0.05).ConclusionFES-cycling can improve the hip flexion function in patients with early stroke,and help to improve the walking ability. |
