| Objectives1.To build a health education program on breastfeeding for pregnant women with hepatitis B virus2.To explore the application of this health education program on the knowledge level,self-efficacy and breastfeeding status of pregnant women with hepatitis B virus.Methods1.Construction of health education program for maternal breastfeeding with hepatitis B virusFirstly,literature analysis was used to read and analyze domestic and foreign literatures and guidelines on breastfeeding of pregnant women carrying HBV,to understand the research status and influencing factors of breast-feeding of HBVcarrying primiparas at home and abroad,Analyze the design and implementation of relevant intervention programs,to provide theoretical reference for the construction of health education programs on breastfeeding for HBV-carrying women in this study.Based on qualitative interviews and theoretical basis,a preliminary draft of health education program on breastfeeding for women carrying HBV was constructed,and expert consultation was conducted.The intervention program was modified and improved according to the expert opinions,and the final test draft was formed.2.Application and effect evaluation of health education program of breast-feeding for pregnant women with hepatitis B virus.In this part of the study,a quantitative study was used to carry out the implementation and evaluation of the intervention program.Prenatal examinations were conducted in the outpatient department of a maternal and child health hospital in Henan Province from October2020 to December 2020.A total of 100 case samples were collected according to the inclusion and exclusion criteria,of which 8 cases were lost,with a loss rate of 8%.A total of 47 women in the control group and 45 women in the intervention group were included.The control group was given routine nursing,and the intervention group was given the health education program of breast-feeding for pregnant women carrying HBV on the basis of routine nursing.General maternal data were collected during the outpatient visit.Before intervention,at the time of discharge after delivery and 42 days after delivery,the clinical indicators of breastfeeding knowledge level,breastfeeding self-efficacy and breastfeeding status of pregnant and puerperal women were collected by questionnaire to evaluate the effect of intervention.SPSS25.0 was used to analyze the data,mainly using chi-square test,t test,descriptive statistics and repeated measurement analysis of variance.Results1.Construction of health education program on breast feeding for pregnantwomen with hepatitis B virusBased on the study of literature and guidelines and semi-structured intervi ews,the first draft of health education program on breastfeeding for pregnant women carrying HBV was determined.The first draft of the intervention plan was optimized through expert consultation,and the final draft of the interventi on plan was finally obtained.A total of 7 themes of the intervention were def ined.The whole process was divided into two stages,and the intervention peri od was 13 weeks.Intervention was performed every 3 weeks until 28 weeks of gestation,and then at hospitalization,postpartum,and discharge.2.Application and effect evaluation of health education program on breastfeeding for pregnant women carrying hepatitis B virus(1)Inclusion of research subjectsIn this study,a total of 100 subjects met the inclusion criteria,including 47 women in the control group and 45 women in the intervention group,excluding 8women who were lost to follow-up.There were no significant differences in age,education level,occupation and family income between the intervention group and the control group(P > 0.05 for all).(2)Comparison of breastfeeding knowledge:Two-factor repeated-measure ANOVA was performed on the breastfeeding knowledge scale scores of the subjects in the two groups at three time points before intervention,at discharge after delivery,and at 42 days postpartum.The results showed that there was an interaction effect between the time factor and the grouping factor(F=15.441,P < 0.001),that is,the grouping effect on the breastfeeding knowledge scores of the two groups(the intervention group and the control group)would change with time.Breastfeeding knowledge scores were consistently higher in the intervention group than in the control group at discharge after delivery and at 42 days postpartum(P < 0.001 for all).(3)Comparison of breastfeeding self-efficacy:Repeated measure ANOVA was conducted on the skill dimension,inner activity dimension and total score of self-efficacy,and the results showed that intervention and time effect of these three aspects had interaction(F=16.093,P < 0.001;F=21.314,P < 0.001;F=36.873,P < 0.001,respectively),the effects of grouping on skill dimension,inner activity dimension and total score of self-efficacy were positively correlated with time,and the skill dimension,inner activity dimension and total score of self-efficacy in the intervention group were always better than those in the control group at discharge after delivery and 42 days after delivery(P < 0.001 for all).(4)Comparison of breastfeeding status between the two groups:After intervention,the rate of exclusive breastfeeding in the intervention group(55.6%)was significantly higher than that in the control group(27.7%)at discharge after delivery.At postpartum 42 days,the rate of exclusive breastfeeding in the intervention group(44.4%)was still higher than that in the control group(25.5%),and the difference between the two groups was statistically significant(P=0.030).Conclusion:The breastfeeding health education program for pregnant women with hepatitis B virus can improve the knowledge level and self-efficacy of breastfeeding for pregnant women with hepatitis B virus,and ultimately improve the rate of postpartum exclusive breastfeeding,so as to provide reference for the breastfeeding intervention program for pregnant women with hepatitis B virus. |
