Objective:Investigate the status of breastfeeding self-efficacy(BSE)in second-child pregnant women, and explore the impact of demographic characteristics, social support, and marriage quality on the BSE of second-born pregnant women and the interaction between these factors. It is hoped to provide the scientific evidence for making intervention strategies, increasing the breastfeeding, and prolonging the breastfeeding time. Methods:This study was a cross-sectional study, using a convenient sampling method, and selected the second child pregnant women who attended a third-level hospital in Liaocheng from March to July 2018 as the research object.Data were collected using a questionnaire survey. The research tools were general information questionnaire,Breastfeeding Self-Efficacy Scale(BSES),Social Support Rating Scale(SSRS),Chinese Marital Quality Questionnaire, and breastfeeding knowledge questionnaire.Data entry was performed using Epidata 3.1.SPSS 20.0 software was used to process the analytical data.Descriptive statistical analysis was used to describe the general data characteristics of patients such as age, education, occupation, and monthly income.Univariate analysis and Pearson/Spearman correlation analysis were used to explore the differences between BSE in the general demographic characteristics of pregnant women and their relationships with pregnant women's social support, feeding knowledge, and marriage quality.Multiple linear regression was used to explore the related factors of BSE in second-child women. Results:A total of 299 questionnaires were distributed in this study, and 281 valid questionnaires were recovered. The effective rate of questionnaire recovery was 94.0%.The specific research results are as follows:1.BSE status of second-child pregnant women:The BSE scores of the study subjects were skewed, with a median of 54 points, the median of individual inner activity dimensions was 20 points, and the median of skill dimensions was 35 points. The three items with higher BSE scores are:11.I can always change breasts on one side and then change to the other(skills),12.1 can continue breastfeeding every time I breastfeed(personally),13.I can always satisfy my baby to drink breastmilk Needs(skills); The three items with the lowest scores are:3.I can always breastfeed my baby instead of using commercial formulas(such as milk powder)to supplement(personally),6.I can breastfeed even if my baby is crying(Skills),9.I am always satisfied with my breastfeeding experience(individually).2.Multivariate regression analysis results showed:Univariate analysis and Spearman correlation analysis found that pregnant women's monthly income,first-fed feeding methods, social support, feeding knowledge, and marital quality may be related influencing factors for BSE of second-pregnant women(P<0.05);stratified regression results showed that social support,Feeding knowledge and marital quality are important factors influencing the self-efficacy of breastfeeding in second-pregnant women;step-by-step regression analysis shows that the first-feeding methods, monthly income, social support, and feeding knowledge are good skills and personal inner dimensions predictor(P<0.05).Conclusion:1.Education background, monthly income,first-birth feeding methods, main caregiver attitudes, marital quality, social support, breastfeeding knowledge may be the influencing factors of BSE in second-birth pregnant women in Liaocheng.Targeted intervention strategies may be taken to improve the BSE level of second-child pregnant women and improve the rate of pure breast-feeding after childbirth.2.The higher the proportion of one-child exclusive breastfeeding, the higher the BSE and the scores of all dimensions of the second-birth pregnant women. In the future, the second-birth pregnant women who are fed formula milk as one of the formulas can be regarded as the key population for BSE intervention.3.Breastfeeding knowledge scores, marital quality, social support, and caregiver attitudes are all positively correlated with the BSE of second-pregnant women, suggesting that the construction of hospital pregnant women schools could be strengthened. At the same time, the second-birth pregnant women with low marriage quality and lack of spouse support can become one of our focus groups.Breastfeeding education targets should be extended from pregnant women to spouses, family members and the whole society.
