Survey On The Support Among Community Nurses Implementing Transitional Care

Objective Aims to construct a questionnaire about Implementing Transitional care by Community Nurses about community medical services in China.We have used literature review,semi-structured interviews,and Delphi expert consultation.Based on the relevant assessment scales on nursing support work,environment at home and environment abroad.We then used the questionnaire to survey the community nurses of community health service centers in Hangzhou to find out their status.This is to provide theoretical foundation for Implementing a successful and effective Transitional care in communities.It also provides reference for the development of Transitional care and the training of community nurses in the later period of the community.Methods 1.Based on many domestic and foreign literatures.The relevant theoretical framework for a nurse’s supportive work environment both at home and abroad.We then determined the basic framework and post-interview framework.We selected 11 appropriate community nursing managers and nurses and underwent interviews.Initially,we determined the preliminary items for the questionnaire about Community Nurses Implementing Transitional care.The initial questionnaire needed to verify through a two-round Delphi expert consultation.Many indexes including the expert familiarity coefficient,active coefficient,authority coefficient,variation coefficient,coordinate coefficient,test reliability and representativeness of expert consultation.After analysis,feedback was collected,term manifested,and representation was modified.The content contains three parts of a pre-test questionnaire,which include the general information for community nurses,a development present situation and Cognition Status of Nurses to Transitional care.The Support demanding questionnaire for Implementation of Transitional care by Community Nurses.The third part of the "Support demanding questionnaire for Implementing Transitional care by Community Nurses" contains 8 dimensions and 60 terms.2.After the pre-questionnaire survey of 180 community nurses from community health service centers in Hangzhou.We confirm reliability and representativeness of questionnaire via Cronbach’s A index,split-half reliability,structural validity analysis,content validity analysis.Then according to the statistical methods,we filtered the questionnaire entries,and finally the questionnaire was complete.3.The questionnaire on 360 community nurses from 18 community health service centers and we analyze the demand status for implementing Transitional care in Hangzhou.Result 1.Through the Delphi expert consultation,factor analysis method and project analysis method.We finally established the three-parts questionnaire.The third part,"Support questionnaire for Implementing Transitional care by Community Nurses"consists of 8 dimensions,(Work conforms,Career development support,peer support,hardware resource condition support,rights and support,leadership and management support,recognition,autonomy),58 entries.The first part of the "general situation questionnaire" and the second part "the present situation of Transitional care,and Cognition Status questionnaire":are selected by nurses in those actual situations respectively.The third part "Support questionnaire for Implementing Transitional care by Community Nurses "used the Likert-5 grading method.The following are the only choices under the Likert 5 scoring method,"very necessary","comparatively need","uncertainty","don’t need so much" and "don’t need completely".The total score of the questionnaire was 290.The higher the score,the greater the demand for the subject,and the lower the demand on the contrary.In this study,we used exploratory factor molecules to extract eight common factors,the cumulative variance contribution rate was 85.046%.2.The total Cronbach’s A index of support demanding questionnaire for implementing transitional care by community nurses(part 3)is 0.978.The 8 dimensions of Cronbach’s A index are higher than 0.818.The third part if the score was split-half reliability is 0.819;the content validity is 0.969,which shows the questionnaire has good reliability and validity.3.Through the questionnaire for the implementation of transitional care by community nurses,we find out the degree that community nurses implement transitional care are generally higher in Hangzhou,as all items have a mean more than4.The Age,education,length of community service,and family visits are major factors for implementing transitional care&support demands on community nurses.Conclusion The constructed questionnaire for implementing transitional care by community nurses has good reliability and validity,and it can be applied to support investigation.The investigation shows that the community nurses in Hangzhou currently have a well-structured human resources structure,age composition,overall academic qualifications、years of community employment,and Community nurses’ family visits are also good.We also find that the demand of transitional care by community nurses is generally higher in Hangzhou.Relevant departments can take corresponding measures and improvements according to the degree that community nurses’ demands to implement transitional care,so as to ensure the effective and smooth implementation of transitional care.
