Study On The Present Situation Of Traditional Chinese Medicine Service Of Community Health Service Center In Beijing

Objective:Based on interviews with experts, policy and literature analysis as well as the original subject to investigate the status of TCM(Traditional Chinese Medicine) service in community health service centers of Beijing, and find out the main problems existing in the process of developing TCM services.Methods:We used the literature research method to collect domestic and foreign literature, policies and regulations, normative documents and other information, to understand the relevant background knowledge and development status of community TCM services in Beijing and sort out the relevant index system, standard specification and existing problems of the development of community TCM service. We applied method of questionnaire survey to investigate the present situation of TCM services in community health service centers of Beijing, which included basic conditions of TCM, related indicators of TCM medical services, the development of TCM appropriate technology and the development of the basic public health services using TCM. Then we analyzed the existing problems of community TCM service in basic conditions, technical level, project development, economic impact etc.Results:1. TCM fundamental conditions:The community health service centers’TCM fundamental conditions in Beijing reached the basic construction standard and they basically had the capability to provide TCM services.The communities had an average of 3.7 TCM service departments,67.7% of the communities’TCM business housing area weren’t less than 150 square meters,89.6% of the communities set TCM Department,85.9% of the communities set up a single room for TCM decoction pieces,78.1% of the communities had no less than 300 types of TCM decoction pieces,79.7% of the communities equipped with more than five kinds of TCM decoction pieces equipment,95.3% of communities met the requirements of "except the basic instruments for acupuncture, cupping and skin scraping, other TCM diagnosis and treatment equipment equipped with not less than 4 kinds",76.6 % of the communities’TCM equipment equipped with reached more than 10 species.2. TCM service index:Increasing the capacity of community TCM service and the services index year by year, but the proportion reached construction standards of Beijing community TCM service areas and nationwide advanced units of community TCM were not high. The TCM’s community health service territories were small, the TCM business housing area, number of TCM department, out-patient volume of TCM, prescription quantity of TCM non-drug therapy and TCM decoction pieces in community health service centers had a very small proportion. The standard-reaching rate of TCM appropriate technology development and promotion of new species were both not so high,57.8% of the communities’ TCM decoction pieces service could not cover within the jurisdiction of the community health service station. The TCM revenue accounted for a low proportion of total business income.3. TCM appropriate technologies:92.2% of the communities’ TCM appropriate technology species number reached national demonstration community health service center construction requirements (more than 6),43.7% of the communities met the construction requirements of Beijing community TCM comprehensive service area (more than 10). Most of the TCM appropriate technology services carried out a little, the carry out frequency as a "general" and "less" accounted for the vast majority, The TCM appropriate technology service utilization rate was low. Most of the community health service center were able to carry out the acupuncture, moxibustion, massage, cupping, scraping, plastering therapy and ear-pressure therapy, but due to the impact of price factors, the medical personnel enthusiasm is not high and the number of times these treatments being used was not much. Because the impact of levels of personnel and technology, more than half of the communities didn’t carry out TCM service items such as acupuncture, osteopathy, acupuncture point injection, medicated, guidance and needle knife, and even if they had carried out it, the number of times was rarely, the application rate was very low.4.TCM public health services:TCM to carry out of the national basic public health services generally had limited funds and personnel. The TCM health education service basically reached the requirements of the national demonstration community health service center construction, but only 39.06% of the communities’ TCM health education prescription number reached Beijing TCM community construction index requirements. TCM health care projects such as common infectious diseases precaution, the prevention of stroke sequelae and physically disabled rehabilitation services,0-6 year old child healthcare, maternal healthcare, elderly people healthcare have carried out in the majority of community health service centers, communities to carry out the management of chronic diseases in TCM was mostly limited to hypertension, diabetes and stroke, the ratio to use TCM treating senile dementia,malignant tumor and osteoporosis was less than 40%.5. The difference between demonstration area and non-demonstration area is small:Low proportion of assessment on the contents of TCM in demonstration community health service index system, the differences between demonstration community health service centers and other non-demonstration community health service centers was not obvious, not embodying TCM plasticity.Conclusion:Beijing community health service centers have been basically established TCM service conditions, the indicators such as TCM revenue, TCM outpatient volume, TCM prescriptions and TCM appropriate technology application types increased year by year, The ability construction is constantly advancing. The community health service center basically carried out at least 6 kinds of TCM appropriate technology, most of them carried out acupuncture, cupping, scraping, plastering therapy, massage and other TCM services. TCM services have participated in community health care such as infectious diseases prevention, rehabilitation, health care, health education, key population health records, chronic disease management and other health services projects. Due to the limitation of TGM community funds, personnel, technical level and service price, most indicators of TCM services had a low proportion in the community health center services, the volume of TCM non-drug therapy and TCM decoction pieces was small, the standard-reaching rate of TCM appropriate technology development and promotion of new species were both not so high, more than half of the communities’TCM decoction pieces service could not covered within the jurisdiction of the community health service station. In community health service centers, most of the TCM appropriate technology services had a low utilization rate, both the amount of TCM health education prescription and the application of TCM in the treatment of chronic disease were not so much.
