At present,the trend of population aging in China is becoming more and more severe,and the elderly’s demand for elderly care and medical care is increasing.In this context,if we want to truly solve the problem of integrating medical care and elderly care,we should implement a community-based integrated medical care and elderly care system.Pension model.In this regard,this article draws on the relevant experience of domestic and foreign medical and elderly care services,and proposes strategies to promote the development and improvement of community medical and elderly care services,which to a large extent promotes the exploration process of community medical care and elderly care services in the academic community.It provides a favorable guarantee for the continuous development and improvement of community medical care and elderly care services,and to meet the people’s growing demand for diversified and multi-level health care services.This article fully integrates healthy aging theory,community care theory and welfare diversification theory,reads and sorts out a large number of domestic and foreign medical and elderly care services related documents,and also sorts out relevant government policy documents,and combines community medical care and elderly care services.The content and characteristics of the model carried out an in-depth analysis of the relevant situation of the H community in Suqian City,expounding the problems of the integrated elderly care services in the H community of Suqian City.For example,the elderly facilities are not perfect,the service level is uneven,and the departmental coordination mechanism is not sound.The service efficiency of the community’s “integration of medical and elderly care” has been improved,the elderly’s national health file information cannot be shared intelligently,cannot meet the diversified medical and elderly needs of the elderly,the implementation of the medical-care integration policy is not strong,the service funds are insufficient,and the service process is not smooth,etc.,And explained the reasons for the above problems.Finally,this article draws lessons from the UK’s community care for the elderly,the U.S.PACE model for elderly care,Japan’s long-term care for the elderly,Australia’s family doctor responsibility system and the Shanghai community’s combined medical care model,Beijing’s community home care model,Qingdao’s long-term care model,etc.Based on the characteristics of the H community in Suqian City,we put forward targeted countermeasures and suggestions to promote the development of integrated medical care and elderly care services in the H community in Suqian City,mainly including strengthening the community’s "integrated medical and elderly care" elderly service professional team construction and improving services Quality,realize the diversification of the supply of "medical and elderly care" elderly service providers,improve the service system,strengthen government functions,improve relevant policies and regulations,refine service content,establish industry standards,uphold a public welfare mentality,and explore market methods.
