| Objective:Through online and offline questionnaire survey and field survey of county general hospitals in the eastern,western,southern and northern regions of Guangdong Province,this study understands the current situation of orthopedic construction and management of county general hospitals in Guangdong Province,and analyzes and summarizes the difficulties and problems they face in the development and construction.Based on the experience of orthopedics construction and management in other countries and regions,this paper puts forward some specific suggestions and countermeasures to further standardize the orthopedics construction and management procedures of county general hospitals in Guangdong Province.It also provides scientific basis for continuous improvement of provincial orthopedics quality control center.Methods:Literature research.Referred to Chinese and English databases,summarized the construction and management of orthopedics in general hospitals at home and abroad,and searched for the latest policy documents related to the construction and management and control of orthopedics medical care issued by the central and local governments,and summarized and organized these documents for reference.Questionnaire survey.This study used the online and offline questionnaire and on-the-spot investigation,he survey subjects were 23 orthopedic departments of county level regional general hospitals in Guangdong Province,which had medical science and technology connection with the orthopedic research institute affiliated to a medical university in Guangdong Province,the research content is to understand the orthopedic hospital construction and management of "organizational management","facilities and equipment","medical technology level" and "medical care quality",under the condition of sorts through the data collected by questionnaire and analysis,grasp the status quo of the construction and management of county general hospital orthopedic,analysis of existing problems,and puts forward corresponding proposals.Mathematical statistics.IBM SPSS Statistics 22 and GraphPad Prism 7.0 statistical software were used for statistical processing of the survey data,and cartographic analysis was carried out to find out the hidden practical problems in the data,and objectively analyze the causes of the problems,so as to enhance the scientific nature of the research method.Results:A total of 23 questionnaires were sent out and 22 were recovered with an effective recovery rate of 96%.For the current situation of orthopedic construction and management in Guangdong county general hospitals,the survey showed that the total score of the questionnaire was set at 100 points.Among the overall scores of the surveyed hospitals,the highest final score was 96.5,the lowest score was 72,and the average score was 86.In general,taking Guangzhou,the capital of Guangdong Province as the center,the districts and counties were divided into four regions:east,south,west and north.The average score of orthopedic quality control and management of general hospitals in southwest Guangdong Province was higher than that in northeast Guangdong Province.In order to explore the main factors influencing the orthopedic overall situation,respectively analyzes the southwest and northeast county general hospital between the organization and management,facilities,medical technology and medical quality difference,upon examination,the southwest and northeast region between the "organization management"(P=0.002<0.05)and the "medical technology level"(P=0.002<0.05)difference was statistically significant,and "facilities"(P=0.061>0.05),and "quality" there was no statistical difference(P=0.634>0.05).The survey shows that the "facilities and equipment" and "medical quality" of the 22 hospitals surveyed in this research are relatively valued,especially the"medical quality" section,which generally gets a high score.The average scores of these two items in all hospitals were 17.3 points and 32 points,and 59.09%and 77.27%of the surveyed hospitals were higher than or equal to the average scores.The scores of "organization management" and "medical technology level" were generally low.The average scores of these two items were 23.1 and 13.7 in all hospitals,and 54.55%and 40.91%of the surveyed hospitals were above or equal to the average score.Conclusion:The overall situation of orthopedic construction and management of county general hospitals in Guangdong province is uneven,and there are differences in scores between east,west,north and south.In general,the hospitals surveyed in this study paid more attention to the construction and management of "facilities and equipment" and "medical quality" in the department of orthopedics,especially the section of "medical quality",which generally got a high score.The scores of"organizational management" and "medical technology" were generally low,and the degree of importance was slightly lower.Therefore,county general hospitals should strengthen the construction and management of orthopedic medical care,so as to improve the overall score and medical level of the hospital.The problems still exist in the construction and management of orthopedic department in Guangdong county general hospitals:1.The overall level of orthopedics construction and management in county general hospitals is uneven;2.The organizational structure of orthopedics construction and management is not perfect;3.The network management level is low and the informatization monitoring level is backward;4.Lack of medical professional and technical level makes it difficult to meet the practical needs;5.The proportion of beds and medical personnel in the orthopedic ward is unreasonable;6.Medical quality management and control lack of professional quality control organizations and quality control personnel.In view of the problems existing in the construction and management of orthopedic department in Guangdong County General Hospital,the following countermeasures are put forward:1.Strengthen the quality control of orthopedic medical treatment,improve the level of orthopedic construction and management of county general hospitals;2.Implement multi-level and three-dimensional management and control to improve the organizational structure of orthopedics construction and management;3.Establish a network information platform to improve the level of network monitoring and management;4.Pay attention to the technical training and continuing education of medical staff to improve the medical professional level;5.Increase the number of orthopedic hospital beds and medical personnel to improve the quality of orthopedic medical services;6.Set up a professional orthopedic quality control team to improve the quality control and management efficiency of orthopedic department. |
