ObjectivePromote the early detection of chronic diseases,reduce the risk of high-risk populations,continue to explore and optimize appropriate technologies and management models for chronic disease screening interventions that are suitable for China’s actual conditions and the Chinese population,and promote the establishment of long-term chronic disease screening interventions.Materials and methods1.A total of 4,372 health checkups who came to the Health Management Department of Nanfang Hospital from July 2015 to June 2017 and completed the lifestyle questionnaire assessment were selected.Chronic diseases were found through health checkups and lifestyle questionnaires.Assess the risk of chronic diseases for subjects,and give personalized lifestyle interventions.Through regular follow-up,dynamic monitoring of blood pressure,weight,blood glucose,blood lipids and other indicators,lifestyle adjustments and interventions,achieve the Continuous intervention and improvement of existing disease risk factors to study the role of individualized lifestyle intervention in improving chronic diseases.2.Selected from January 2016 to December 2017,the lifestyle questionnaire survey data of a total of 7,504 subjects who completedthe physical examination and completed the lifestyle questionnaire survey in the Health Management Department of Nanfang Hospital,and applied it as the subject The simplified lifestyle questionnaire questionnaire was designed by the author to fill out the questionnaire through the lifestyle questionnaire questionnaire platform to compare the differences between the original standardized lifestyle questionnaire and the simplified lifestyle questionnaire in collecting lifestyle risk factors.3.A total of 433 employees of a tobacco company who participated in the online lifestyle questionnaire were selected from January 2019 to May 2019 to evaluate how the subjects independently filled out the lifestyle questionnaire through the health management platform.ResultsDevelop personalized health intervention measures for the subjects who participated in the professional one-on-one lifestyle questionnaire.Through the improvement of lifestyle and regular follow-up,after 12 months of intervention,the general health risk factors in the lifestyle were found The 35594 risk factors before the intervention dropped to 31396 risk factors.The general health risk factors in the average lifestyle of each person dropped from 8.14 to 7.18.At the same time,the subjects did not receive injections but only improved their lifestyles,BMI,waist-to-hip ratio,blood pressure,blood sugar,uric acid,blood lipids and other indicators improved significantly,and the data were all statistically significant(all P<0.05).2.By applying the original standardized lifestyle questionnaire consultation information to the simplified lifestyle questionnaire filled out by the health management platform,the health risk factors screened out are analyzed.The simplified lifestyle questionnaire covers dietary behavior,dietary structure,exercise status,and smoking.Compared with drinking status,sleep and mental state,living environment module and standard version questionnaire,there are statistically significant differences in finding risk factors(P<0.05).3.Before the health promotion,a total of 16 employees participated in the online lifestyle questionnaire,and the participation rate was 3.7%.After starting health education for employees participating in the questionnaire in April 2019,the number of participants who completed the questionnaire rose to 203,and the participation rate rose to 46.9%.After health promotion,a total of 187 people completed the questionnaire,accounting for 92.1%of the total number of people filling in the questionnaire.ConclusionPersonalized lifestyle intervention has a significant effect on improving the risk factors of chronic diseases,which shows that personalized lifestyle interventions are effective and can improve the healthy cardiovascular behavior and factors of the population to a certain extent.The traditional one-to-one lifestyle intervention in medical and health care institutions cannot be adapted to the national conditions of China with a large population of chronic diseases and difficult management.Through improved questionnaires and the use of information technology to build a professional health management platform,the subject can be passively cooperated.Transform into an active participatory type,conduct comprehensive monitoring,analysis,and assessment of individual health status,provide health consultation and guidance,and intervene in health hazards,mobilize the enthusiasm of individuals,the whole and the whole society,and build a collection of health examinations,risk assessments,Treatment intervention,follow-up,health education,health archives,and effect evaluation are all in one new information-based and personalized health management service model for cardiovascular disease risk factors,realizing the whole life cycle health management of the whole society,and using limited resources to maximize Health effects.This new personalized health management model for cardiovascular disease risk factors is expected to be promoted nationwide.
