Under the double impact of the aggravation of the aging population and the hollowing out trend in rural areas,the health problems of the elderly in rural areas are increasingly prominent.Among them,the proportion of the elderly with chronic diseases is getting higher and higher,leading to a corresponding increase in the rate of disability and mortality.Therefore,it is particularly important for the elderly with chronic diseases in rural areas to carry out health management.The uneven distribution of medical resources in rural areas makes it a new direction to change the past passive disease treatment into active health management.Health management needs of rural elderly chronic diseases are prominent.Social work provides health management capability to enhance case services,which is conducive to reducing incidence rate and improving the quality of life.Based on the theory of social support network and the method of case work,this paper makes a comprehensive and detailed understanding of the elderly with chronic diseases in rural areas by using the methods of in-depth interview,literature and action research.It aims to improve their health awareness,link health resources and expand social support network,and educate health knowledge and train their abilities for agriculture to improve the health management ability of the elderly with chronic diseases in villages,targeted case services are provided.The ultimate goal is to reduce the incidence rate,disability rate and mortality rate of the elderly in chronic diseases in rural areas,improve the cure rate of diseases,improve the health level and quality of life of the elderly in chronic diseases in rural areas and their families,reduce medical expenses and promote rational allocation of social resources.Through eight case services,social workers and service objects analyze the supporting factors and obstacles to improve the health management ability,and achieve the improvement of health management ability by removing obstacles and obtaining support resources.Finally,through the observation and analysis of the practical process,it is found that the case work has prominent advantages in promoting the health management ability of the elderly with chronic diseases,and the individualized way can effectively concentrate resources;promote the service object and social workers to participate in the change process together,mobilize theenthusiasm of the service object,and improve the confidence of the service object;and it is conducive to the timely awareness and service of social workers the cognitive,psychological and emotional changes of the service objects;however,the available supporting resources for the elderly with chronic diseases in rural areas are extremely limited,which makes the implementation of case work more difficult,and the ability of social workers to fully link resources is facing a great test.In this practice,there are some deficiencies in the application of social workers' case work skills and strategies,and in the solution of ethical dilemmas.
