Objective: Based on the theory of social cognition,the present situation of health self-management of elderly patients with chronic diseases under the digital medical perspective is understood and mastered through questionnaire survey.Through multi-dimensional and multi-level investigation and analysis of the environment,cognition,behavior and overall level of health self-management of elderly patients with chronic diseases under the background of digital medical treatment,four aspects of health self-management of elderly patients with chronic diseases are analyzed.Comprehensively analyze the influencing factors of the health self-management of the elderly patients with chronic diseases,improve the health self-management ability of the elderly patients with chronic diseases,pay attention to and promote their health self-management behavior,and reduce the incidence and prevalence of diseases from the perspective of prevention.And integrate humanistic care into the process of health self-management of elderly patients with chronic diseases,understand the actual needs of patients,seek emotional resonance with patients,to provide a scientific basis for guaranteeing their quality of life in old age and promoting healthy aging.Methods: The elderly chronic disease patients in Nanhu Community,Dongfang Community,Huayang Community,Pengcheng community,Fuan community and Hongsheng community meeting the inclusion and exclusion criteria were selected by random convenience method for questionnaire survey,including general information questionnaire,adult health self-management ability questionnaire and social support rating scale.Statistical software SPSS22.0 was used for data analysis.Descriptive statistical analysis was used to describe different demographic characteristics,mean ± standard deviation((?)±s)was used to describe measurement data,independent sample T-test and one-way analysis of variance were used to compare the differences in health self-management ability scores of elderly patients with chronic diseases with different demographic sociological characteristics.Multiple linear regression method was used to analyze the influencing factors of health self-management in the elderly patients with chronic diseases.Results: 520 questionnaires were distributed,and 516 questionnaires were finally validated after all were collected and sorted out,with an effective rate of 99.23%.1.The score of health self-management behavior of elderly patients with chronic diseases from the digital medical perspective was(44.73±11.29)points;The score of healthy self-management environment was(32.47±8.14)points;Health self-management cognitive dimension score was(45.55±11.77)points;The total score of health self-management was(122.76±27.82)points.2.t test and one-way analysis of variance were used to compare the differences in scores of various dimensions of health self-management among elderly patients with chronic diseases with different demographic sociological characteristics.The results showed that:(1)The total score of health self-management of the elderly patients with chronic diseases was statistically different in gender,education level,physical condition,psychological condition,economic condition,type of chronic disease,duration of illness,number of children,degree of family concern,use of electronic equipment,frequency of using "digital medical services" and social support(P<0.05);(2)The scores of health self-management behavior of elderly patients with chronic diseases were statistically different in education level,physical status,psychological status,economic status,type of chronic disease,duration of illness,family care,online knowledge of chronic disease information,and social support(P<0.05).(3)The scores of health self-management environment of elderly patients with chronic diseases were statistically different in gender,educational level,physical condition,psychological condition,economic condition,type of chronic disease,duration of illness,number of children,residence of children,degree of family concern,use of electronic equipment and social support(P<0.05).(4)The cognitive dimensions of health self-management of elderly patients with chronic diseases were statistically different in gender,educational level,physical status,psychological status,economic status,type of chronic disease,duration of illness,family concern,frequency of using "digital medical services",and social support(P<0.05).3.The results of multi-factor analysis showed that the factors influencing the level of health self-management behavior of elderly patients with chronic diseases were: the degree of family concern,economic situation,duration of illness,type of chronic disease,social support,psychological condition,physical condition,educational level,F=69.399,P=0.000;The factors influencing the health self-management environment level of elderly patients with chronic diseases were: family concern,type of chronic disease,number of children,social support,physical condition,economic condition,duration of illness,education level(F=55.976,P=0.000);The factors influencing the cognition level of health self-management of elderly patients with chronic diseases were: physical condition,economic condition,family concern,psychological condition,social support,type of chronic disease,duration of illness,education level,F=54.599,P=0.000;The factors influencing the total level of health self-management of elderly patients with chronic diseases were: the degree of family concern,economic situation,duration of illness,physical condition,psychological condition,social support,type of chronic disease,education level,F=97.174,P=0.000.Conclusion:1.From the digital medical perspective,the total score of health self-management of elderly patients with chronic diseases was(122.76±27.82),showing a state of "high cognition and low behavior".2.The usage rate of digital devices among the elderly is generally low,and the self-management scores of the elderly who can use electronic devices and know about digital medical treatment and often use digital medical treatment are higher than those who do not use electronic devices and do not know about digital medical treatment.3.The influencing factors of health self-management behavior,environment,cognition and overall level of elderly patients with chronic diseases are: education level,physical condition,psychological condition,economic condition,type of chronic disease,duration of illness,use of electronic equipment and social support. |
