BackgroundThird-grade A hospital undertake sophisticated functions of diagnosis and treatment,education and scientific research,and their medical personnel are the important practitioners of these functions and play a mainstay role in third-class hospitals.Therefore,in order to enhance the role of the third-grade A hospital as the leader in the field of health system,it is necessary to pay attention to the people-oriented,stimulate the work morale and enthusiasm of medical staff,and improve the work enthusiasm.However,the enthusiasm of medical staff is faced with serious brain drain,prominent mental health problems,job burnout and other problems,so the medical staffs mental health,job burnout and turnover problems need to pay attention to.ObjectiveThe purpose of this study is to understand the current situation of mental health,job burnout and turnover intention of medical staff in Jinan third-grade A hospital,analyze the influence of demographic characteristics,job characteristics,health status and other factors on mental health,job burnout and turnover intention,and explore the influence of mental health and job burnout on turnover intention.MethodsUsing simple random sampling method,extraction of five third-grade A hospital in Jinan 20 3 third-grade A hospital,with equidistant stratified cluster sampling method,according to the title and stratification,and then according to the proportion of the number of layers,each layer is determined in the sampling,sample drawn in the same distance,determined to meet the requirements of survey of all the doctors,nurses and hospital pharmacists sample,a total of 800 people of a doctor in five hospitals,900 nurses,pharmacists,440,the survey questionnaire 2140,excluding questionnaire there are logic errors,recycling 2090 questionnaires,effective response rate was 98.03%.The self-made questionnaire was used to investigate the basic situation of medical staff,the service industry burnout scale(MBI-HSS)was used to measure the burnout status,and the depression,anxiety and stress scale(DASS-21)was used to measure the mental health status.Results(1)Basic information of respondents:There are a total of 2090 respondents,among whom there are more females,accounting for 72.9%of the total number.32.9%of the respondents have teaching positions;25.6%of the respondents had scientific research tasks;From point of view,the official at and side employed each accounted for 50%,average monthly salary in 7500-10000 yuan in the majority of respondents,accounting for 34.5%of the total number of investigation,age in 0 to 10 years in the majority of respondents,61.1%,32.4%of respondents had two or more chronic disease,most respondents sleep 7 hours a day,accounted for 40.5%.(2)Mental health,job burnout and turnover intention of medical staff:about 40%of the respondents showed anxiety symptoms of different degrees,and the very serious anxiety accounted for 9.3%of the total respondents.The occupational burnout of pharmacists is more serious.57.5 percent of the medical staff showed different degrees of burnout,16.8 percent of the medical staff have the intention to quit.(3)The influence factors of turnover intention:gender,education,scientific research,preparation,professional type,title,weekly working hours,doctor-patient disputes,working load,the salary satisfaction,daily sleep time,suffering from chronic diseases,the number of turnover intention have an impact,the doctor’s risk of a departure will produce 1.756 times that of pharmacists,the results have statistical significance;The risk of turnover intention of medical staff who had suffered from doctor-patient disputes was 1.754 times that of medical staff who had not suffered from doctor-patient disputes.Medical personnel who perceived a heavy workload had an increased risk of turnover intentions compared with those who perceived a light workload.(4)Mediating effect analysis:mental health is positively correlated with job burnout and resignation intention,while job burnout is positively correlated with resignation intention.Mental health has an indirect effect on turnover intention through job burnout as an intermediary,and the indirect effect of mental health on turnover intention(0.314 × 0.574=0.180).The total effect value of mental health on turnover intention is 0.525,that is,direct effect(0.345)+indirect effect(0.180)=0.525.Conclusions and SuggestionsThe mental health problem of medical staff is more serious,especially the anxiety problem and job burnout phenomenon is more prominent,the job burnout problem of pharmacist group is more serious;The turnover intention is high.Demographic characteristics,job characteristics and physical health status have a certain influence on mental health,job burnout and turnover intention of medical staff.The mental health of medical staff is significantly positively correlated with job burnout,mental health is significantly positively correlated with turnover intention,and the level of job burnout is significantly positively correlated with turnover intention.Job burnout has a significant mediating effect on mental health and turnover intention of medical staff.Based on the above conclusions,in order to reduce the turnover intention of medical staff,the following suggestions are put forward in this study:(1)improve the mental health level of medical staff;(2)reduce the job burnout of medical staff;(3)establish a reasonable salary system;(4)reduce the employment risk of medical staff;(5)improve the physical health level of medical staff. |
