| Objective:1.To explore the correlation between the normal range of myocardial enzymes levels and reproductive hormones,glucose and lipid metabolism in polycystic ovary syndrome.2.To evaluate the value of myocardial enzymes in predicting long-term CVD in PCOS patients,and to provide a new reference for the diagnosis and treatment of PCOS patients with cardiovascular diseases.Methods:This is a second analysis of 1000 PCOS patients in the PCOSAct project.Patients with missing CK and LDH records were excluded.Totally,937 patients with CK results and 939 patients with LDH results were grouped according to quartiles of CK and LDH levels.The grouping results were as follows:CK grouping:A:10-36U/L;B:37-49U/L;C:50-62U/L;D:63-99U/L;LDH grouping:A:4-50U/L;B:51-84U/L;C:85-116U/L;D:117-402U/L.The correlation between different levels of CK and LDH and baseline characteristics,reproductive hormones and glycolipid metabolism in PCOS patients was compared.Results:1.Baseline characteristics:In the four subgroups with CK level quartile,in the comparison of age,weight,BMI and systolic blood pressure,the body weight,BMI and systolic blood pressure increased with the increase of CK level,and there were statistical differences among the four groups(p<0.05,P<0.01);WHR increased gradually with increased levels of CK,but there was no statistical differences.The body weight,BMI,waist circumference,WHR and acanthosis nigricans score increased with the increase of LDH levels in the four sub-groups with LDH level quadruple,and the difference was statistically significant(p<0.05,P<0.01).2.Sex hormone indicators:T,FT and FAI increased with the increase of CK level in four subgroups with four equal levels of CK(p<0.05,P<0.01),and SHBG decreased with the increase of CK level(p<0.05).FT and FAI increased with the increase of LDH level(p<0.01)and SHBG decreased with the increase of CK level(p<0.01).3.Glucose and lipid metabolism indexs:TC,LDL,ApoAl and ApoB decreased with the increase of CK level in four sub-groups,with statistical significance(p<0.05,P<0.01),FPG decreased with the increase of CK level,but did not reach statistical difference(p>0.05).Among the four sub-groups,FPG,HDL and ApoAl gradually decreased with the increase of LDH level(p<0.01);ApoB/ApoAl increased with the increase of LDH level(p<0.01);TG and ApoB increased with the increase of LDH level,but there was no statistical difference(p>0.05).4.Correlation analysis:CK was positively correlated with T,FT and FAI,negatively correlated with age,FSH,SHBG,FPG,TC,HDL,LDL,ApoAl and ApoB.CK was positively correlated with T,FT and FAI,negatively correlated with age,FSH,SHBG,FPG,TC,HDL,LDL,ApoAl and ApoB;LDH was positively correlated with body weight,BMI,waist circumference,WHR,pulse,black spine skin score,FT,FAI,TG,ApoB/ApoA1,negatively correlated with SHBG,FPG,TC,HDL and ApoA1.Conclusion:1.The higher the level of LDH in the normal range,the more significant the obesity degree of PCOS patients.The higher the level of CK and LDH in the normal range,the more serious the disorder of sex hormone,especially androgen in PCOS patients.With the increase of CK and LDH in the normal range,the abnormal glucose and lipid metabolism has a serious trend.2.From the relationship between normal myocardial enzyme level and PCOS obesity,sex hormone disorder,abnormal glucose and lipid metabolism,it can be inferred that with the further deterioration of the above indicators,it will cause abnormal myocardial enzyme and increase the risk of cardiovascular damage in PCOS.Therefore,monitoring myocardial enzyme level has certain significance.At the same time,PCOS patients should control their weight and correct endocrine metabolic disorders.3.Myocardial enzymes may be an important early warning index for predicting PCOS complicated with CVD,but more and more comprehensive studies are needed to verify it. |
