Objective:Observe the clinical efficacy of 'Niejin-Maiwei' massage to prevent the digestive tract adverse reaction caused by chemotherapy regimens which include cisplatin. Methods:This study was a single-center,non-randomized controlled study.According to the inclusion and exclusion criteria,a total of 60 patients who were admitted to the Oncology Department of Guang'anmen Hospital of China Academy of Chinese Medical Sciences from March 2019 to January 2020 were included. They were divided into experimental group which has 28 individuals and control group which has 32 according to their wishes. No dropout occurred. The control group was routinely given ondansetron injection before chemotherapy. On the basic treatment of control group, the experimental group received an operation called 'Niejin-Maiwei' massage after daily use of cisplatin which continuously for 2-3 days.On the 2nd day of cisplatin use and on the 5th day after the end of cisplatin use,the MASCC antiemetic evaluation tool was used to evaluate nausea and vomiting. On the 1 st to 3rd days of chemotherapy, researchers will evaluate appetite, constipation and diarrhea within and between the two groups. At the end of the trial, researchers will compare the effective rate of TCM symptom improvement between the two groups. The levels of hemoglobin and albumin will be compared within and between groups before and after chemotherapy. The occurrence of adverse reactions between two groups were also evaluated.Results:1.Nausea and vomiting:The incidence of delayed nausea(42.9%vs.78.1%,P=0.005)and delayed vomiting(21.4%vs.46.9%,P=0.039)in the experimental group were lower than those in the control group, and the difference were statistically significant. The average score of delayed nausea(1.68 ± 2.278 points vs.3.84±2.908 points,P=0.003)and the average times of delayed vomiting(1.11±2.910 times vs.4.84±10.271 times,P=0.029)in the experimental group were lower than those in the control group, and the difference were statistically significant. However, the experimental group did not show any advantages over the control group in the control of acute nausea and vomiting(P>0.05).2.Appetite,constipation and diarrhea:For comparison within groups, there were both statistically significant differences in the experimental group and the control group in appetite grading on day 2 of chemotherapy and that on day 1 of chemotherapy(P=0.020;P=0.015).There was a statistically significant difference in appetite grade in the experimental group on the 5th day after the end of cisplatin use which was compared with the 3rd day of chemotherapy(P=0.007),but no statistically significant difference in the control group(P>0.05).When comparing between groups, the appetite grade of the two groups on the 1st day of chemotherapy, the 2nd day of chemotherapy, the 3rd day of chemotherapy and the 5th day after the end of cisplatin were compared, and the difference were statistically significant(P=0.007;P=0.003;P=0.038;P=0.001).When comparing the constipation of the two groups within the group, the difference was only statistically significant between the 3rd day of chemotherapy and the 5th day after the end of cisplatin use(P=0.017)in the experimental group, while the difference in the control group were not statistically significant(P>0.05).When comparing between groups at different time points, the differences were not statistically significant(P>0.05).When comparing the diarrhea of the two groups both within and between groups, the differences were not statistically significant(P>0.05).3.TCM symptom score:The effective rate of TCM symptom improvement in the experimental group was greater than that in the control group(57.1%vs.21.9%,P=0.005),and the difference was statistically significant4.Hemoglobin and albumin levels:When compared within the two groups, the average hemoglobin levels after chemotherapy both decreased compared with that before chemotherapy(113.54 ± 14.849 g/L vs.118.93 ± 14.954 g/L,P=0.009;105.91 ± 14.846 g/L vs.1 12.66± 14.788 g/L,P=0.002),and the difference were statistically significant. There were no significant differences in the average hemoglobin levels between the two groups(P>0.05).There were no statistically significant differences within either of the two groups in mean albumin level after chemotherapy and that before chemotherapy(P>0.05).When the mean albumin levels before chemotherapy were compared between the two groups, the difference was statistically significant(P=0.006);when the mean albumin levels after chemotherapy were compared between the two groups, the difference was not statistically significant(P>0.05).5.Adverse reactions:Adverse events in the blood system, digestive system and urinary system of the two groups were evaluated respectively, and there were no statistically significant differences between the two groups(P>0.05).Conclusion:For patients receiving cisplatin-containing chemotherapy,an operation called'Neijin-Maiwei' massage combined with ondansetron can significantly reduce the incidence of delayed nausea and vomiting and reduce the degree of delayed nausea and the times of vomiting. It can also relieve the appetite descent and constipation and promote appetite recovery. The safety of it has also been proved.
