Distribution And Correlation Of Traditional Chinese Medicine Constitution In Nausea And Vomiting During Pregnancy

Objective:Based on the basic principles of evidence-based medicine,this study observes the clinical symptoms of nausea and vomiting in early pregnancy women,conducts physique investigation with the help of Classification and Judgment Criteria of Physical Constitution of Traditional Chinese Medicine,and introduces relevant laboratory indicators and psychological scales to explore the distribution of physique,physique and psychology of women with nausea and vomiting during pregnancy.The correlation between laboratory micro-indicators is expected to provide a theoretical basis for clinical prevention and treatment of nausea and vomiting during pregnancy from physical and psychological aspects.Methods:Based on the basic principles of evidence-based medicine,this study observes the aspects.Results:1.Demographic dataA total of 120 women in the first trimester of pregnancy were surveyed,with an average age of 28.98±3.89 years,the oldest being 40 years old and the youngest being 21 years old.2.History and current concomitant diseases2.1 Past history and surgical history:Three pregnant women with previous history of disease were enrolled,including uterine leiomyoma,teratoma and appendicitis.In terms of surgical history,except for cesarean section and induced abortion,one patient had a history of surgery,appendectomy.2.2 History of marriage,childbearing and abortion:All the pregnant women in the group were married,and the pregnancy history ranged from pregnancy 1 to pregnancy 5.The proportion of pregnancy 1 was the highest,accounting for 47.50%.The number of deliveries ranged from zero to two,with the highest proportion of deliveries being zero,accounting for 57.50%.35 cases of spontaneous delivery,accounting for 68.63%,and 31.37% of cesarean section.In terms of abortion history,34 pregnant women had abortion history,accounting for28.33%.In terms of abortion mode,24 cases were induced abortion,accounting for70.59%;8 cases were drug abortion,accounting for 23.53%;4 cases were spontaneous abortion.3.PUQE score of nausea,vomiting and pregnancy57 cases had mild PUQE scores,accounting for 47.50%,of which 9 had 3 scores,accounting for 7.50%,60 had moderate scores,accounting for 50.00%,and 3 had severe scores,accounting for 2.50%.4.Symptoms related to TCMThe symptoms of nausea and vomiting in early pregnancy mainly include appetite,change of taste,thirst,cold and heat,salivation,dizziness,fatigue and fatigue.In the aspect of appetite change,the proportion of anorexia was the highest(86,71.67%);those with taste change(68,56.67%);those with acidity were the highest(29,56.86%);those with anorexia and greasy were the highest(39,57.35%);those with thirst and thirst(45,77.59%);those with cold and heat symptoms(85,70.83%)were the highest(36,80.00%).Among them,the highest proportion was fear of cold(49,57.65%),dizziness(67,55.83%),fatigue(97,80.83%),fatigue(105,87.50%)and salivation(36,30.00%);sleep disorders(46,38.3%)with the highest proportion of hypersomnia(28,56.52%);abnormal stool(44,36.67%)with the highest proportion of constipation(35,79.55%);abnormal urination(74,61.67%)with frequent urination.(74,100%)and frequent nocturnal urination(61,82.43%).5.Laboratory resultsFrom high to low,the proportion of blood group in this survey was: type O(38.37%),type B(27.91%),type A(22.09%),type AB(11.63%)and Rh(+)accounted for 98.84%;other indicators: increased hematocrit(66.32%),increased serum free triiodothyronine(37.37%),decreased gamma-glutamyl transpeptidase(40.00%),decreased prealbumin(32.61%)and vitamin C-positive urine.Sex(21.51%),of which'+-'accounted for the highest proportion(55.00%)and urine ketone body(20.43%),of which'+-' accounted for the highest proportion(42.11%).6.Personality Distribution ResultsPersonality types in descending order of frequency and frequency were: intermediate personality(21,17.50%),polyhemism(18,15.00%),biliary-polyhemoglobin(18,15.00%),polyhemoglobin-mucinous(17,14.17%),biliary-depressive(13,10.83%),mucinous(12,10.00%),mucinous-depressive(8,6.67%),depressive(7,5.83%)and biliary(6,5.00%).7.Physical Distribution ResultsAmong the 120 pregnant women enrolled in this group,114 could be clearly classified as constitutional type,and 6 were not one of the nine typical constitutional types.Four of them could not clearly judge the constitutional type,but tended to be constitutional.Six pregnant women who could not clearly judge the type of constitution were not analyzed for the time being.Among 114 pregnant women,58.77% were single constitution and 41.23% were double constitution.There are two to seven different conditions of concurrent clamp constitution,18 cases of concurrent clamp two constitutions,accounting for 15.79%,14 cases of concurrent clamp three constitutions,accounting for 12.28%,8cases of concurrent clamp four constitutions,3 cases of concurrent clamp five constitutions,and 2 cases of concurrent clamp six constitutions and seven constitutions,respectively.After separating 118 pregnant women who could clearly judge the constitution type and constitution tendency,the frequency of constitution type was in descending order: Yin deficiency quality(46.61%)> Qi deficiency quality(44.92%)> Qi depression quality(44.07%)> Yang deficiency quality(40.68%)> damp heat quality(38.14%)> phlegm dampness quality(36.44%)> Pinghe quality(25.42%)> blood stasis quality(20.34%)>Special quality(16.10%).Among them,the proportion of pregnant women with Yin deficiency and tendency to yin deficiency was the largest,about 46.61%.8.Physical RelevanceThis survey shows that the combination of asymptomatic and mild NVP is regarded as a "control group".After the combination of moderate and severe NVP is regarded as a "observation group",the "control group" is positively correlated with the quality of peace,and the "observation group" is positively correlated with the quality of depression.In terms of TCM symptoms,salivation and phlegm-dampness are the most correlated,cold fear and yang deficiency are the most correlated,and there is a significant positive correlation between fever and Yin deficiency,dizziness and dampness-heat,and fatigue and Qi deficiency,Yang deficiency and qi depression are all positively correlated.In terms of personality,Qi-depressive temperament was positively correlated with depressive temperament,bile-depressive temperament,and calm temperament was positively correlated with multi-blood temperament.In terms of laboratory indicators,FT4 was positively correlated with calm quality,neutrophil percentage was positively correlated with Qi deficiency and blood stasis,PA was positively correlated with Yin deficiency,and gamma-GT was negatively correlated with Damp-heat quality.Conclusion:(1)Nausea and vomiting are the characteristic manifestations of early pregnancy response.The incidence of nausea and vomiting is high.The severity of nausea and vomiting is mainly mild to moderate.Pregnant women generally lack attention to NVP,and the early treatment rate is low.Most early pregnant women have nausea and nausea,and vomiting occurs relatively less frequently.(2)The constitution of NVP pregnant women is diverse.Qi deficiency,Yin deficiency and Qi depression are the three main constitution types of NVP,while the mild NVP reaction is the lightest,and the NVP reaction of Qi depression is relatively serious.Pre-pregnancy conditioning biased constitution can be done,and the emotional state of pregnant women can be regulated from many aspects in clinical treatment.(3)The accompanying symptoms of NVP are diverse,involving diet,body,spirit and other aspects.Clinical treatment should pay attention to care.(4)Different personality types have biased constitution.Qi depression is positively correlated with depression,bile-depression,and calmness is positively correlated with polyplasm.We should pay attention to the interaction between them in clinical practice.(5)Pregnant women in early pregnancy may be accompanied by abnormalities in various laboratory indicators.The survey data show that FT4 is positively correlated with peace quality,and attention should be paid to differentiating FT4 from abnormal pathological state in clinical treatment.
