Objective: To explore the effect of TCM clinical nursing intervention with constant temperature and ginger moxibustion as the main nursing measures on nausea and vomiting induced by chemotherapy in breast cancer patients,in order to improve the quality of life of patients, and at the same time to induce nausea and vomiting caused by clinical chemotherapy. Methods: In this study,60 breast cancer patients who met the inclusion criteria were divided into observation group and control group according to the principle of random grouping,30 cases in each group. The control group was treated with routine comprehensive treatment and nursing of Western medicine.Including:(1)Application of conventional antiemetic drugs:intravenous granisetron hydrochloride(Taiji Group Sichuan Taiji Pharmaceutical Co.,Ltd.,national medicine quasi-H20030161)before and after chemotherapy on the day of chemotherapy,3 mg each time with a total dose of 6 mg.(2)Routine nursing: 1)Create a comfortable environment: keep the ward quiet, clean and tidy, and ensure fresh air.2)Nurses explain the role and purpose of medication to patients,do a good job of health education of chemotherapeutic drugs, alleviate patients' nervous fear.3)Vomiting response nursing: inform patients not to rush to eat in large quantities during vomiting, but to supplement water first. Can use warm sugar saline, swallow slowly;warm boiling water gargle, keep the mouth clean and comfortable.(3)Emotional nursing of TCM.(4)Chinese medicine diet nursing.Observation group: The observation group was given routine treatment and nursing in the control group on the basis of adding constant temperature and ginger grilled for nursing intervention.Method: Moxibustion with constant temperature and ginger partition:1)Time: the day before chemotherapy, the day of chemotherapy, and the 2nd-3rd day of chemotherapy, the time when the gas and blood flow of stomach meridian of Foot Yangming was vigorous(7:00-9:00)was selected.2)Acupoint: Zhongwan,Zusanli(bilateral),Shenque.3)Operating method: Use constant temperature moxibustion. The patient took supine position, exposed the moxibustion parts of Zhongwan,Shenque and Zusanli, cut ginger into slices 3-4 cm wide,2-3 cm wide and 0.2-0.3 cm thick, punctured with thick needles in the middle, put the prepared ginger slices on the moxibustion site, ignited thunder-fire moxibustion strips, put them into the constant temperature moxibustion apparatus, covered with a cover, and then put the constant temperature moxibustion apparatus on the moxibustion site and wrapped it in a large bath towel to keep the patient warm and comfortable.Moderate and non-scalding without scraping.4)Course and frequency of treatment: once a day,30 minutes a time, for 4 consecutive days. After the intervention, the patients in both groups were evaluated with Index nausea and vomiting and retching(INVR)scale. To explore the effect of constanttemperature and ginger moxibustion on preventing and treating nausea and vomiting caused by breast cancer chemotherapy, improving patients' compliance with chemotherapy and improving their quality of life. Results: Results: A total of 60 patients were included in this study. On the day of chemotherapy, the duration of nausea symptoms, the frequency of vomiting and retching symptoms, and the severity of nausea, vomiting and retching symptoms were compared between the two groups,P > 0.05.There was no statistical difference between the two groups, suggesting that the effect of the observation group and the control group in alleviating CINV in the above dimensions had no difference. However, the duration of vomiting and retching symptoms and the frequency of nausea symptoms in the observation group were lower than those in the control group, and P < 0.05,which had statistical significance, suggesting that the observation group had the effect of alleviating CINV in the above dimensions. On the second day after chemotherapy, the frequency of nausea and vomiting symptoms and the severity of nausea and vomiting symptoms were compared between the two groups,P > 0.05.There was no statistical difference, suggesting that there was no difference in the effect of CINV alleviation between the observation group and the control group in the above dimensions. However,in the duration of nausea, vomiting and retching symptoms, the frequency of occurrence of retching symptoms and the severity of retching symptoms, the score of the observation group was lower than that of the control group, and P < 0.05,with statistical significance, suggesting that the observation group had the effect of alleviating CINV in the above dimensions. On the 3rd day after chemotherapy, the duration, frequency and severity of nausea, vomiting and retching in each dimension were lower in the observation group than in the control group(P <0.05).The difference was statistically significant, suggesting that theobservation group had the effect of alleviating CINV in the above dimensions. On the 7th day after chemotherapy, the duration, frequency and severity of nausea, vomiting and retching in each dimension were lower in the observation group than in the control group(P < 0.05).It was significant that the observation group had the effect of alleviating CINV in the above dimension. From the 3rd to 7th day after chemotherapy, the effect of the observation group on alleviating CINV remained stable, while the observation group had the effect of alleviating CINV in the observation group. There were no obvious adverse reactions such as dizziness and moxibustion in the process of constant temperature and Ginger-separated moxibustion. Conclusion: 1.The overall effect of the observation group is better than that of the control group in alleviating CINV in breast cancer patients, and constant temperature Ginger-separated Moxibustion can effectively improve the nausea, vomiting and retching symptoms of breast cancer patients undergoing chemotherapy.2.Constant temperature and Ginger-separated Moxibustion also had significant advantages in improving delayed nausea, vomiting and retching symptoms, and there were no obvious adverse reactions such as dizziness and moxibustion in the observation group during the process of constant temperature and Ginger-separated moxibustion.3.Constant temperature and Ginger-separated Moxibustion is safe and effective in alleviating CINV in breast cancer patients, and it is worth popularizing in clinic.
