Professor Wang Mei's Experience In The Treatment Of 80 Cases Of Subacute Thyroiditis With Qinggan Xiaoying Zhitong Decoction

Purpose:Through observing the efficacy of Professor Wangmei in treating 30 patients with subacute thyroiditis(SAT)with the "Qinggan Xiaoxuan analgesic method" and summarizing the clinical experience and basic thoughts of Professor Wangmei on the treatment of SAT.It provides clinical ideas and unique insights for TCM treatment SAT.Material and method:From October 2016 to December 2018,a total of 80 patients who met the standards were collected,case data were taken,and patient information and condition were recorded.To standardize the collected case data into Excel 2007 to establish a database.Statistical analysis of the collected patient data is performed by using the method of self-comparison.Summarize the basic thought and clinical experience of Professor Wangmei's treatment of SAT.Results: 1.General information: 80 patients with SAT,with a total of 150 consultations,of which 12(18.75%)were males,68(35%)were females,and the ratio of males to females was 1:6.The oldest is 63 years old,the youngest is 25 years old,and the average age is 43.21±9.33 years old.The onset of SAT is dominated by young and middle-aged people.2.Clinical characteristics:(1)Febrile and thyroid pain characteristics: of the 80 SAT patients,only 15(18.75%)were initially diagnosed with fever,only 28(35.00%)with pain,and 37(46.25%)with pain and fever.Heat and heat: low heat(37.3?~38.0?)8cases(15.38%),moderate heat(38.1?~39.0?)32 cases(61.54%)high heat(39.1?~41.0?)12 case(20.08%).Thyroid pain: 9 cases(33.33%)of mild pain,10 cases(37.03%)of moderate pain,and 8 cases of severe pain(29.62%).Thyroid pain site: left pain in 12 cases(7.40%),right pain in 39cases(33.33%),bilateral pain in 14cases(11.11%),and alternate pain in 13 cases(48.14%).(2)Symptoms: In the acute period,there were 41 consultations,38 consultations(92.68%),3 consultations(7.31%),and 3consultations(7.31%).In the acute period,the main factor was fire.3.Evaluation of efficacy: 30 patients with acute SAT had the shortest fever withdrawal time of 2 days,the longest 21 days,and the average heat withdrawal time of 10.57±6.32 days.The pain relief time is the shortest 4 days,the longest 74 days,and the average pain relief time is 20.99±14.65 days.The pretreatment symptom score was 17.89±8.59,and the post-treatment symptom score was 1.82±2.98.There was a significant difference before and after treatment(P?0.05).According to the criteria for determining the curative effect of symptoms,18 cases were clinically cured,10 cases were effective,2 cases were effective,0 cases were ineffective,and the total effective rate was 100%.4.Commonly used drugs: Professor Wangmei treats the number of SAT flavors between 6 and 12,mostly 9 flavors.The most commonly used 15 kinds of drugs are Lianqiao,Zhebei,Bupleurum,Angelica,Bailiao,Zhimu,Xiacao,Jiajia,Jiangbanxia,Poria,Tianpollen,Baishu,Yanhusuo,Huangqin,gypsum.In line with the master's basic idea of clear liver and eliminate galls.Conclusion: 1.The 30 cases of SAT observed in this study are mainly female,and the most common among young and middle-aged people.Most of them are both hot and painful,and the degree of fever is mainly medium-heat,rare and high-heat.In the treatment of SAT with Qinggan Xiaoyan analgesic method,the average antipyretic time was 10.57±6.32 days,and the average analgesic time was 20.99±14.65 days.There was a significant difference before and after treatment(P?0.05),and the total effective rate was 100%.2.Professor wangmei believes that this disease belongs to the category of Chinese medicine "gall disease","gall gas","gall swelling" and other diseases.The occurrence of the disease is closely related to emotional factors,in addition to feeling the evil of wind fever and warm fever.The pathological site of SAT is in front of the neck,and the front of the neck is also the progressive site of the liver.The evil of wind and heat invades the veins of the liver.The hot evil and the blood and blood are gathered in front of the neck,resulting in lumps and swelling and pain;Emotional disorders such as worry for a long time,so that the liver is out of tune,the gas machine is depressed and angry,the phlegm gas condenses,hot blood stasis,before the neck knot,the formation of swelling.The antipyretic detoxification method and the antipyretic analgesic method are organically combined and used for the SAT attack of evil poison,ambush,fever,and thyroid pain.Clinical use of forsythia,Bupleurum,Astragalus,Yanhusuo and other antipyretic and analgesic products.
