The Clinical Experience Of The Treatment Of Subacute Thyroiditis In The Acute Phase Of The Treatment Of Toxic And Detoxification

ObjectiveBy observing the curative effect of "Outthrust evils and Toxin-resolving" method on the antipyretic and analgesic of the subacute thyroiditis(SAT)acute stage,to summarize the basic thought and clinical experience of Professor Liu's "Outthrust evils and Toxin-resolving"method in treating acute stage of SAT,to provide treatment ideas for pure Chinese medicine in the acute stage of SAT.Method1.From January 2015 to March 2018,a total of 32 cases with SAT acute stage inclusion criteria were collected,and patient data were taken.The medical records were faithfully recorded.2.Standardize the collected data into Excel 2007 to create a database.3.Analyze the collected data using self-contrast methods.4.To summarize the basic thought and clinical experience of Professor Liu's "Outthrust evils and Toxin-resolving" method in treating acute stage of SATResult1.General Information:32 cases of acute stage of SAT,a total of 156 times diagnosis,including 4 males(12.5%)and 28 females(87.5%),male to female ratio 1:7.The oldest was 62 years old,the youngest was 29 years old,and the average age was 45.28±9.37 years old.The acute stage of SAT was mainly young and middle-aged.2.Clinical characteristics:(1)Characteristics of fever and thyroid pain:In 32 cases of acute stage of subacute thyroiditis,only 2 cases(6.25%)were seen with fever,only 14 cases(43.75%)with pain,and 16 cases(50.00%)with pain and fever.The degree of heat:4 cases(22.22%)with low fever(37.3??38.0?),13 cases(72.20%)with moderate heat(38.1??39.0?),and 1 case(5.56%)with high fever(39.1 ??41.0?).Thyroid pain:10 cases of mild pain(33.33%),11 cases of moderate pain(36.67%),9 cases of severe pain(30.00%).Thyroid pain area:3 cases(10,00%)in left side pain,8 cases(26.67%)in right side pain,3 cases(10.00%)in bilateral pain,and 16 cases(53.33%)in alternating pain.(2)Syndrome characteristics:There were a total of 42 times diagnosis in the acute stage,and the syndromes were 40 times diagnosis(95.24%)for the Evil poison stasis in Shaoyang Syndrome,and 2 times diagnosis(4.76%)for the Invasion of Wind and Heat Syndrome.3.Therapeutic evaluation:The shortest anti-febrile time was 2 days,the longest was 21 days,and the average anti-febrile time was 10.47±6.29 days.The shortest pain relief time was 4 days and the longest was 73 days.The average pain relief time was 20.88±14.64 days.The symptom score before treatment was 17.69±8.49,and the symptom score after treatment was 1.81±2.97.There was a significant difference before and after treatment(P=0<0.05).According to the criterion for the therapeutic efficacy of syndromes,18 cases were clinically cured,10 cases were markedly effective,4 cases were effective,0 cases were ineffective,and the total effective rate was 100%.4.Drug analysis:Professor Liu treated acute stage of SAT with 5 to 12 herbs,mostly 9 herbs.The most commonly used first 15 drugs are Forsythia,scutellaria,chai hu,artemisia,fritillary bulb,reed root,semi-summer,north sea cucumber,radix powder,Fried fruit,ginger,semi-summer,codonopsis,poria,talc,ginger.It contains the meaning of Xiaochaihu Decoction and Haoqinqingdan Decoction.It is in line with the basic idea of "Outthrust evils and Toxin-resolving".Conclusion1.This study finds that female tend to have acute stage of SAT,especially young and middle-aged.Most of them have both fever and thyroid pain.The degree of fever was mainly moderate heat.High fever was rare.There was a significant difference before and after treatment(P=0<0.05)in the "Outthrust evils and Toxin-resolving" method on the antipyretic and analgesic of the SAT acute stage.The total effective rate was 100%.2.Professor Liu believes that the acute stage of SAT disease location is in Shaoyang.It is hidden evil stasis in Shaoyang,and it is becoming heat toxin.lt should be treated from the"Evil poison stasis in Shaoyang".The basic treatment method is "Outthrust evils and Toxin-resolving" method.As the same time,we should take care of Vital qi and use Xiaochaihu Decoction or Haoqinqingdan Decoction as main recipe."Outthrust evils and Toxin-resolving"method is an organic combination of outthrusting evils method and detoxification method,which is used in the acute stage of SAT for fever and thyroid pain.We always use fructus forsythiae,scutellaria baicalensis,radix bupleuri,sweet wormwood.
