According to the relevant meta-analysis results,the prevalence of hypertension in China increased from 9.8%(95% CI: 6.3%-14.9%)to 30.0%(95% CI: 26.7%-33.6%)during the1980 s to the first 10 years of the 21 st century,However,according to the epidemiological survey of various types of hypertension,the awareness rate,diagnosis rate and control rate of hypertension among adults in China have not improved significantly,and are still at a low level.As one of the important target organs of hypertension damage,hypertensive renal damage has become a common chronic disease in nephrology department under the situation of poor control of hypertension.According to the data,about 17.9% of hemodialysis patients are renal failure caused by hypertension,up 6% from 10 years ago.Hypertensive renal damage has become one of the main causes of chronic kidney disease(CKD)and its progression to end-stage renal disease(ESRD).Modern medical treatment of hypertensive renal damage advocates stable hypotension combined with lipid-lowering therapy,delaying the progress of CKD.Intractable hypertension in patients with nephropathy requires a combination of antihypertensive drugs.However,due to renal dysfunction,atherosclerosis and ischemic kidney changes,the application of some antihypertensive drugs,such as angiotensin converting enzyme inhibitors and angiotensin II receptor blockers,is limited,which makes it difficult to effectively control blood pressure and aggravates the progress of CKD.In recent years,domestic scholars have made some achievements in assisting hypotension,improving symptoms and delaying the deterioration of renal function by applying traditional Chinese medicine in combination with conventional therapy.Professor Yuanfang thinks that the blood flow in kidney is abundant,which can be differentiated and treated according to the theory of blood-vessels of traditional Chinese medicine,regards“Mai Zhang”as the name of hypertensive renal damage.She concludes that the causes of “Mai Zhang”are related to improper diet,emotional disorder,chronic illness,fatigue,oldness and other factors.Six excesses factors are the inducement and aggravation factors.The mechanism of disease is disharmony of qi and blood,disharmony of blood veins and fullness of vessels.Pathological nature is divided into deficiency and excess,static blood,dampness turbidity,and toxic pathogens obstruct the vessels as excess syndrome;spleenand kidney weakness and liver deficiency can not promote qi and blood,qi and blood stagnate vessels as deficiency syndrome.The pathogenesis of this disease is complex,and the clinical common syndrome of deficiency and excess is mixed.The location of the disease is in the blood vessels,which is closely related to the function of zang-fu organs.The disease was divided into five types: deficiency of kidney essence,deficiency of liver yin,deficiency of spleen,blockage by static blood and obstruction of dampness turbidity.The main method of treatment is "regulating qi and blood,unlocking the vessels",aiming at maintaining the blood vessel peace,according to its deficiency and excess treatment,supplementing deficiency and reducing excess when necessary.To provide new thinking and experience of syndrome differentiation for the treatment of hypertensive renal damage with traditional Chinese medicine.
