Purpose:To explore the method of arthroscopic reconstruction of the posterior cruciate ligament by using the autogenous hamstring tendons to retain the residual fiber and the residual bundle branch bundles,systematically evaluate the postoperative knee function and observe its clinical efficacy.Material and method:Thirty-six cases of patients with posterior cruciate ligament injury admitted to Shenyang Orthopaedic Hospital between November 2015 and March 2017 were selected.All surgical methods were performed under arthroscopy with autogenous hamstring tendons to retain stump fibers and residual bundle branches to reconstruct the posterior cruciate ligament.The femoral ends were suspended with a zygomatic plate,and the tibial ends were retracted with absorbable screws and fixed with metal cannulated screws.Pegs are double fixed.Actively prevent and treat complications after surgery,and early systemic rehabilitation training.According to the preoperative and postoperative 12-month follow-up thigh circumference,knee ROM and PDT indexing,objective assessment of knee function was performed;Lysholm score,IKDC score and IKDC2000 knee joint examination table were performed on the ipsilateral knee joint.Subjective function evaluation of the system.Results:Thirty-six patients in this group were followed up for 12 months.The follow-up period was 12-27 months.The mean follow-up period was(20.02±4.33)months.No serious complications occurred during the follow-up period.Objective evaluation of all patients: the average diameter of the thigh circumference was(48.94±5.38)cm before operation,and(52.49±5.84)cm on the 12 th month after operation;the average ROM score of the affected knee was 68.19±.13.90 degrees,after an average of 12 months(134.31 ± 6.23)degrees;preoperative ipsilateral PDT index: 30 degrees in 3 degrees,2 degrees in 6 cases,no 1 degree and 0 degrees;postoperative 12 Monthly PDT indexing: 0 degrees in 32 cases,1 degree in 4 cases,no 2 degrees and 3 degrees.Subjective function evaluation results of all patients: Lysholm score was 41.58±4.73 on the affected side,and was 89.39±4.81 on the 12 th month after surgery.The Lysholm score was excellent at 12 months after operation.The good and good rate was 97.22%.The preoperative IKDC score was averagely divided into(32.65±8.44)points,and was averagely divided into(83.41±5.29)12 months postoperatively.In the IKDC2000,the functional status of the knee examination table was graded: In the preoperative 36 cases,26 cases were D-grade,10 cases were C-grade,and no B-grade and A-grade;among 36 cases,30 cases were A-grade,and 6 cases were Grade B,no C and D grades.Preoperative and postoperative 12 months(P<0.01)differences were significant.Conclusion:Arthroscopic reconstruction of the posterior cruciate ligament with autogenous hamstring tendon retaining residual fibers and residual bundle bundles is a simple technique that has the advantages of easy operation,less damage to the body,less postoperative complications,and safety and reliability.The early recovery of knee function is worthy of clinical promotion and further study.
