Objective:To investigate the effect of arthroscopic endobutton titanium plate in the treatment of tibial avulsion fracture of posterior cruciate ligament(PCL).Methods:The cases were selected from 28 patients with PCL tibial avulsion fractures admitted between April 2015 and April 2019,including 21 males and 7 females.The age ranged from 13 to 65 years,with an average of 37.32 ± 12.35 years.There were 15 cases of traffic injuries,9 cases of sports injuries,and 4 cases of bump injuries.The surgical methods were all treated with conventional arthroscopy anterior medial and lateral approach and posteromedial approach.Subjective evaluations were performed before and6 months after surgery using Lysholm scores and American Special Surgery Hospital knee scores(HSS).KT-1000 was used to objectively assess knee stability.Results:In the follow-up of 6 months,2 patients were positive in the first degree drawer test,the rest were negative.The mobility of the two patients was less than 120°and the rest were more than 120°.There was no instability of the knee joint.The subjective score of knee joint was normal distribution,and the variance was homogeneous(P > 0.05).The preoperative knee joint mobility ranged from 70 °to 115 °,with an average of 95.36±12.90°.The 6-month postoperative joint mobility ranged from 110°to 145°,with an average of 129.64 ±9.62 °.The postoperative period was higher than that before operation(t = 14.57,P<0.001).The 6-month follow-up HSS score of 85.82 ±3.74 for knee joint was significantly higher than 33.89±4.34 before surgery,and the difference was statistically significant(P<0.001).The 6-month follow-up of knee joint Lysholm score 91.07 ±2.46 was significantly higher than 40.25 ±2.44 before surgery,and the difference was statistically significant(P<0.001).KT-1000 measures the backwardmovement of the tibia of the affected knee and the contralateral side.When the knee was bent at 90°,the affected side was 2.51±0.16 mm,and the healthy side was 2.45±0.15 mm.There was no significant difference(P> 0.05).Conclusion:Arthroscopic treatment of PCL tibial avulsion fracture with endobutton titanium plate has a reliable effect.It is a surgical method with high accuracy,small complications and good bone healing.It can thoroughly explore and debridement,more accurate fixation and suturing,and the fixation is reliable,relatively simple,safe and minimally invasive,and imitated biomechanical fixation.It does not need secondary surgery,and has overall advantages over traditional incision Obvious.It can benefit the patients.
