| BackgroundNon-specific low back pain(NLBP)is a common and frequently-occurring disease in orthopaedics.With the aging of the population and the change of modern lifestyle,the incidence of low back pain shows two extreme trends:younger and older.In conservative therapy,wearing belt protector can relieve the pressure of intervertebral disc and joint,relieve the tension of lumbar dorsal muscle and relieve symptoms through its supporting and restraining effect on lumbar spine.Traditional Chinese medicine is widely used in clinic because of its unique advantages of simplicity,convenience,cheapness and effectiveness.Xuanbi lotion is composed of 12 traditional Chinese medicines,such as Tengjin Cao,Zanthoxylum bungeanum,Lulutong and Tougucao.It has the functions of dispelling wind,dispelling cold and removing dampness,promoting blood circulation and dredging collaterals and relieving pain.It is suitable for Bi syndrome of cold and dampness obstruction and blood stasis obstruction collaterals.It has been proved in previous basic and clinical studies that its external application can treat degenerative osteoarthri.Although there are many kinds of belts on the market,they are not well targeted and can not meet people's needs.Therefore,based on the concept of ergonomics,this study designed and developed a more practical belts-improved belts.The waistband can not only play a more in line with the needs of modern doctors and patients,but also be compatible with the external application of traditional Chinese medicine.In the previous study of Xuanbi lotion,there was no clinical study on NLBP.Therefore,this study systematically observed the clinical efficacy and safety of Xuanbi lotion combined with improved belt protector in the treatment of NLBP,and provided the basis for its further clinical research and application promotionObjective1 Design of waistband based on Ergonomics.2 To evaluate the clinical efficacy and safety of Xuanbi lotion combined with modified belt protector in the treatment of nonspecific low back pain.MethodCollecting the T10-S1 CT tomographic images of normal volunteers'waist and back,using Mimics 10.0 software to build the three-dimensional geometric model of the normal lower back,fitting the human-machine contact surface of the belt by SolidWorks 2016 software,cutting the surface of the main support points of the preset waist,and obtaining the design parameters of the belt support strip;in the design of the main body of the belt,in order to make the belt more stable.The main body extends to the sacrococcyx to form an arc-shaped restraint belt,and the center of the belt is designed with Chinese medicine wet compress,which consists of a fixed layer,dressing layer and isolation layer.By observing 30 cases of non-specific low back pain which met the inclusion criteria of this study,all patients were treated with Xuanbi lotion and improved belt braking.The treatment cycle lasted 2 weeks.Xuanbi lotion was applied twice a day for 28 times.The Oswestry disability index(ODI)and degree of pain(visual analogue scales VAS)were observed and recorded on the day of case screening and the 3rd,5th,7th,14th day after treatment,and 1,2nd month follow-up at 7 time points after the end of treatment.The overall efficacy was evaluated by ODI evaluation criteria for low back pain.The data were analyzed and processed by SPSS12.0 statistical software package.VAS score and ODI score were expressed as mean(standard deviation)(x±s),and single factor repeated measurement analysis of variance or rank sum test were used,respectively,P<0.05,indicating that the difference was statistically significant.ResultThirty patients included 16 males(53.33%)and 14 females(46.67%)aged from 23 to 48 years,including 15 patients aged 20 to 30 years,11 patients aged 31 to 45 years,and 4 patients aged 46 to 55 years,with an average of 31.33 years,with an average course of 16.67 months.The overall effective rates were 6.67%,60.00%,80.00%,93.33%,93.33%and 86.67%after 3 days,5 days,7 days,at the end of treatment,1 month and 2 months,respectively.The overall effective rates were 0%,10.00%,26.67%,33.33%,63.33%and 46.67%respectively.The VAS scores at each time point after treatment and at two follow-up periods were lower than those before treatment,with statistical significance(P<0.05);ODI scores at each time point after treatment and at two follow-up periods were lower than those before treatment,with statistical significance(P<0.05).Conclusion(1)Modified waistband has practicability in clinical application.(2)Xuanbi lotion combined with improved belt protector is effective,safe and stable in improving symptoms,signs and clinical pain of patients with non-specific low back pain,which is worthy of clinical application. |
