Ward nursing operation changes dynamically with the number of patients,level of care and treatment,which is a typically complex and multiple operation process.It is a complicated process to make a systematic ergonomic analysis of the workload of the nursing staff.In this study,orthopedic ward is taken as the research object,and the ergonomic analysis of the workload of nursing staff is realized through the simulation analysis of the operating process,which provides technical support for measuring and improving the operating intensity of the nursing staff.The field investigation is carried out with the methods of interview and observation.The nursing operation flow and the overall operation flow of the ward are clarified and the basic data are collected.The study on ergonomic work load and nursing work load is carried on,and the simulation method is selected to study on the complex work load of multiple personnel.The factors such as continuous working time,rest time,nurse-patient ratio and bed nursing ratio are determined as the main indexes to measure the nursing work load.The flow chart is used to develop the models of nursing operation,shift operation and ward operation.The discrete event system model is established by Petri net.Using Flexsim software platform,a simulation model of orthopedic ward nursing operation is established based on discrete event system model.SPSS data statistical analysis software is used to fit the distribution function of the data collected in the field(patient arrival time,patient operation time,patient out-patient examination time,nurse disposal operation time,etc.),and the corresponding parameters in the simulation model are set.According to the number of patients,the care level,the number of nursing staff,the number of related medical resources and other factors,18 kinds of operation schemes are developed and simulated.Based on the analysis of the simulation data,the key indexes of operating load in each scheme are calculated.The simulation results show that the average daily working time of ward nurse seriously exceeds the ergonomics requirement for the average daily working time.The nurse-patient ratio and the bed protection ratio greatly exceed the relevant provisions of the state.Under the condition of ensuring the requirements of rest time and daily working time,the optimal allocation of nursing staff should reach 26 nurses in the ward/4 nurses in the night/2 nurses in the registry.The working process of the registry and the ward should be modified into a parallel mode in which multiple nurses handle one patient.Performance of improved the system can meet the requirements of patient care services in variety situations.The above work shows that the method of workload analysis based on simulation is feasible to develop ergonomics analysis of nursing staff’s complex work.The results of this study provide useful methods for the objective evaluation of the workload of hospital nursing staff and regulation improvement. |
