Object:Pneumonia is a common and frequently-occurring disease of the respiratory system.ICU Patients with pneumonia are always in a relatively serious condition with a higher mortality rate.Worsestill,with the continuous resistance of pathogens and the unreasonable use of antibiotics,the treatment of ICU patients with pneumonia has become a challenging task clinically.In the meanwhile,the clinical efficacy of TCM in assisting western medicine in the pneumonia treatment has also been continuously proven,which has attracted the attention to the combination treatment of TCM.By controling the herbs of Erchen decoction and adding it into Western medicine treating of ICU patients with pneumonia,the paper observed the clinical efficacy,and further explore the clinical effectiveness of TCM phlegm and sputum excretion in the treatment of ICU patients with pneumonia,in order to provide a new method for relieving symptoms of patients,broaden the treatment approaches,hope to better serve the clinical treatment by combining traditional Chinese and western medicine.Method:This study selected 36 inpatients from the Surgical Intensive Care Unit of Beijing China-Japan Friendship Hospital from October 2018 to January 2019,all of which met the diagnostic criteria of Western medicine for pneumonia and the diagnostic criteria for phlegm-damp obstructing lung in TCM.According to the principle of randomization,contrasting and single-blind,they were respectively included into the contrasting group and the treatment group,with 18 cases in each group.The contrasting group was treated with western medicine;the treatment group was treated with Western medicine,together with Erchen decoction.The treatment duration of both groups was 7 days.The required monitoring data were respectively recorded on the 1 st day and 7th day of the treatment.After treatment,the writer contrasted the clinical efficacy,TCM symptom scores,procalcitonin,C-reactive protein,white blood cell,percentage of neutrophils,arterial oxygen partial pressure,carbon dioxide partial pressure,chest X-ray and sputum volume of two groups,in order to compare the advantages of two treatments.The clinical data collected were statistically analyzed with SPSS 20.0.The measurement data were expressed by the standard deviation:mean plus or minus(X±S).The normality test was performed first.If the normal distribution was met,the baseline data was analyzed by independent sample t test.The indexes before and after treatment in the group was analyzed by paired sample t test.The difference between the groups was analyzed by independent t test.If the normal distribution was not met,the nonparametric test would be used.If the sample size of any group was<30,and the Mann-Whitney test would be used.The count data or composition ratio is determined by the X2 test.If P<0.05,there was a statistical difference;if P<0.01s there was a significantly statistical difference;Result:1.Clinical efficacy:the effective rate of the contrsting group was 68.8%,while that of treatment group was 82.4%.The statistical comparison P=0.398>0.05,with no statistical difference.2.TCM symptom score:the effective rate of the contrsting group was 56.3%,and that of treatment group was 88.2%.The statistical comparison P=0.046<0.05,with a significantly statistical difference.3.Inflammatory index:Both the contrasting group and the treatment group can significantly reduce the values of procalcitonin,C-reactive protein,white blood cells,and neutrophils.With statistical analysis,there is no statistical difference in reducing procalcitonin and white blood cells of two groups.On the other hand,there was a statistical difference in the reduction of C-reactive protein and the percentage of neutrophils,where the treatment group was superior to the contrasting group.4.Arterial blood gas:the partial pressure of oxygen in both the contrasting group and the treatment group increased and the partial pressure of carbon dioxide decreased.After statistical analysis,there were statistical differences,with treatment group being superior to the contrasting group.5.Inflammatory absorption rate in chest radiograph:The effective rate of the contrasting group was 76.5%,and that of the treatment group was 56.3%.In statistical comparison,P=0.282>0.05,with no statistical difference.6.Sputum discharge:the amount of sputum in the contrasting group and the treatment group both significantly reduced.in statistical analysis,P=0.031<0.05,with a statistically different.The treatment group was superior to the contrasting group.Conclusion:1.In terms of clinical efficacy,there is no significant difference between the western medicine treatment group and the treatment group with TCM;however,there is a statistical difference between two groups in improving the clinical symptoms of pneumonia patients.The treatment group is better than the contrasting group.The treatment group with TCM can significantly improve the clinical symptoms of patients with pneumonia.2.In the promotion of sputum discharge,the treatment group with TCM is superior to the western medicine treatment group,with a statistical difference,indicating that the TCM Erchentang has good effect in phlegm reducing and discharging.It may be based on this effect that the lung ventilation function has been improved to some extent.3.In reducing the inflammatory index,both the western medicine treatment group and the treatment group with TCM can reduce the inflammatory index,although there is no statistical difference between the two groups in reducing the PCT and WBC.However,in terms of CRP and NEUT%,there was a statistical difference between two group,with the former being superior to the latter.
