Summary Of Professor Yu Xiaoping's Clinical Experience In Treating Bronchial Asthma Based On Traditional Chinese Medicine Inheritance Support System (V2.5)

Objective:The purpose of this research is to summarize Professor Yu Xiaoping's clinical experience in treating bronchial asthma.The Methodology of this research is done by collecting clinical medical records and relevant data such as lung function test report,exhaled nitric oxide(FeNO)test report,radiology report,blood test report,etc.After this is done,the information gathered will be run through Traditional Chinese Medicine Inheritance Support System V2.5(TCMISS)platform software,which then transform it into statistics of asthma diagnosis,symptoms,treatment and drugs.These statistics data will be analyzed based on regular similarities for clinical diagnosis.These statistics will help scholars to determine possible treatment and drugs that corelates with Chinese and Western medicine.Method:The research in these thesis includes:(1)Literature researchLiterature research based on medical journals that elaborates on the affiliation of pathogenesis,syndrome differentiation and treatment of bronchial asthma from traditional Chinese medicine.(2)Clinical ResearchThe methodology of clinical research includes:1)Collecting medical records of patients with medical records of bronchial asthma,that meet the Western medicine diagnostic criteria,and exclude incomplete medical records.2)Standardization and categorizing patients' medical records:all symptoms,syndromes and prescriptions in the medical record.3)The data collected at no 2 above,is then inserted into Microsoft Excel and this will be run at TCMISS(V2.5).4)The result from TCMISS will be in the form of statistical data.This data will then be analyzed based on its regular similarities and it will help scholars to better understand and prepared for the treatment plan for their patients.Result:(1)General Information:This study collects 93 Asthma patients from total of 519 medical cases.Male to female ratio of 1.00:2.44,a total of 26 patients(27.96%)of age group of 60 to 69 years old.(2)Symptom analysis:The main symptoms consist of dyspnea,wheezing,coughing and sputum which includes daily sputum quantity,sputum color and characteristic,and difficulty expectoration.Highest frequency of the main symptoms is coughing(70.06%),follow up by phlegm(68.89%).The main result of the secondary symptoms by groups are nasal discomfort,mouth discomfort,pharyngeal discomfort,and chest rib discomfort,and pulmonary related discomfort.Tongue and pulse result is dark red tongue,thin white coating,and thready pulse.(3)Disease analysis:519 medical cases mostly occur in asthma acute attack stage,followed up by asthma chronic stage,a few of asthma recovery stage.The top 3 rankings of the combined disease were:bronchiectasis,hypertension,and allergic rhinitis.Type of syndromes,is divided into three category,heat-phlegm accumulation in the lung syndrome,damp-phlegm obstructing lung qi syndrome,and excess deficiency combination syndrome.Therefore,the main treatment is to dispel pathogen,and it is divided into dispel cold and wind,ventilate lung and relieve dyspnea,and clear heat and resolve phlegm.Deficiency main treatment is to replenish the lungs and strengthen exterior qi;invigorate spleen and reduce phlegm;replenishing kidney qi and reinforced fire to strengthen yang;and nourishing yin and promoting blood circulation.(4)Drug analysis:519 medical cases has a total of 686 prescriptions,including 224 drugs,a total of 13995 frequency.686 prescriptions exclude Western medicine and proprietary Chinese medicines.There are 541 Chinese medicine prescriptions,including 192 medicines,the average number medicine in a prescription is 26herbs,without 5 additional herbs.There is a total of92 first-consultation prescription,with a total of 137 herbs,and a total of 2159 frequencies.Medicinal property consists of four natures,five flavors and the meridian tropism.The result of nature and flavor is:the bitter cold herbs is the most frequently used,followed by the pungent warm and sweet neutral.The result of meridian tropism:relate to lungs meridian is the most,followed by liver meridian,spleen meridian and stomach meridian.It reflects Professor Yu Xiaoping's treatment of bronchial asthma,by using bitter cold and pungent warm medicine to treat lungs and pacify liver.Also using neutral and sweet herbs to strengthen and protects the spleen and stomach,usually using Zuojin Wan,consists of Huanglian(Rhizoma Coptidis,golden thread),Wuzhuyu(Fructus Evodiae,medicinal evodia fruit)and Walengzi(Arcae concha).(5)Study of the correlation of regular herbs medication:To obtain the core herb prescription of Professor Yu Xiaoping,setting the accuracy level to 0.9,by adjusting different support level,and review with Professor the most appropriate support level result is 35%,consist of 610 herbs combination involving 18 herbs.There are 117 pairs of two herbs combination used medicinal,superficies-releasing medicinal,heat-clearing medicinal.Asthma chronic stage mainly use dispel pathogen and tonify deficient as the major treatment principle.Research results shows that during dispelling pathogen,tonifying herbs is gradually increasing,such as Nanshashen(Adenophorae Radix),Duanlonggu(Stir-fire Os Draconis,dragon bone)-Duanmuli(Stir-fire Concha Ostreae,oyster shell).Asthma recovery stage mainly use tonifying deficient as the major treatment principle.Professor believes that although asthma did not recurrence,but the root of asthmastill needs to be treated at the same time.Therefore,phlegm-resolving medicinal is still ranked first,followed up by tonifying medicinal such as Huangqi(Radix Astragali,milkvetch root),Wumei(Fructus Mume,smoked superficial.(8)The study of treatment methods based on Asthma different syndrome types:Based on the results above,Professor consider the medication for asthma patient's individual physical characteristic combining with 3 different syndrome types,which is heat-phlegm accumulation in the lung syndrome,damp-phlegm obstructing lung qi syndrome,and excess deficiency combination syndrome.?.Heat-phlegm accumulation in the lung syndrome treatment based on core prescription,adding Yinqiaosan as heat-clearing and toxicity-removing formula,which is uses jinyinhua(Flos Lonicerae,honeysuckle flower)and lianqiao(fructus forsythia,weeping forsythia capsule)as the representative.?.Damp-phlegm obstructing lung qi syndrome treatment based on core prescription,adding Sanzi Yangqin decoction or Erchen decoction,which consists of tinglizi(Semen Lepidii seu Descurainiae,pepperweed seed),zisuzi(Fructus Perillae,perilla fruit),chenpi(Pericarpium Citri Reticulatae,dried tangerine peel),banxia(Rhizoma Pinelliae,pinelliae tuber),fuling(Poria,india bread),etc.?.Excess deficiency combination syndrome treatment based on core prescription,uses the most suitable herbs according to lung,spleen,and kidney organ's qi,yin and yang deficiency.Lung-qi deficiency uses Yupingfeng san as additional formula.Spleen-qi uses Liujunzi decoction as additional formula.Kidney-qi deficiency uses neutral tonifying and strengthening kidney-qi such as xuduan(Radix Dipsaci,Himalayan teasel root),etc.Kidney-yang deficiency uses Xianling formula which is based on Yougui decoction as invigorating blood as a minor treatment.Fourth,is the importance of treating lungs and nose diseases at the same time.Fifth,is the importance of treating lungs and skin diseases at the same time.Conclusion:(1)Professor Yu Xiaoping pays a lot of attention to the collection of clinical medical history:the severity of the symptoms,the degree of severity,the combination of the disease and the evolution of the disease.It is interesting to note that,Professor Yu Xiaoping often use the philosophy of Sun Tzu the Art of War(that is to attack or retreat or guard)when administering prescriptions depending on the condition of the patients.Sometimes particular combination of drugs,often 2 or 3,is used to speed up recovery of the patients.Special attention is made to ensure that no drugs given to the patients can have side effects to the spleen and stomach.(2)There are three main types of syndrome for the treatment of bronchial asthma.The first syndrome is heat-phlegm accumulation in the lung syndrome.The second syndrome is damp-phlegm obstructing lung qi syndrome,and the third syndrome is excess deficiency combination syndrome.(3)During treating asthma,Professor Yu Xiaoping pay special attention to damp-phlegm as the root cause of asthma.(4)When administering prescriptions,there are 5 characteristic treatment principle of Professor Yu Xiaoping in treating asthma based on her long-term clinical experience.First,is the guidelines to use Mahuang.Second,is the use of insects medicinal.Third,is the use of invigorating blood as a minor treatment.Fourth,is the importance of treating lungs and nose diseases at the same time.Fifth,is the importance of treating lungs and skin diseases at the same time.(5)The use of TCMISS is also proven to be not stable as well as buggy and some function are not user friendly.For instance,during working with TCMISS,when there are too much data entered in the programs,it will often freeze and unable to save the entered data.This is an issue that have to be addressed as scholars will often do repetitive work of trying to insert the data again and ensuring it is saved correctly.In addition to this,it is recommended to add a filter function to the search function in the system as it is often difficult to search for a specific symptom in the system.Scholars often use Microsoft Excel when needed to search for specific details.This study founds a clinical observation difference in efficacy between the use of Scorpio and Scorpio powder which is worth for further research.
